RESUMO

Even if the first radical prostatectomy was performed more than ninety years ago by Hugh Hampton Young, it's only during the 20 last years that an anatomical approach to this technique has been developed, to reduce the number of complications routinely described. The evolution of the procedure allow us how to make the operation safer, to perform it in a bloodless field and increase the possibility to preserve continence and potency. We would like to show you how two simple technical variations for the control of dorsal vein complex and the vesico-uretral anastomosis can, at our opinion, decrease the number of operative and postoperative complications. A retrospective review was performed on 168 clinical localised prostate cancer patient (stages T1 to T2 N0 M0) treated in our hospital between December 1987 and March 1998. All these patients underwent retropubic radical prostatectomy with pelvic lymphadenectomy. The preoperative clinical stage was T1 for 48.2% and T2 for 51.8%. The mean age of the patients at operation was 64.4 years and the mean PSA level at diagnosis was 12.4 ng/ml. Our results revealed that 97% of the patients are now perfectly continent and that the continence was immediately recovered by 16% at the removal of the bladder catheter.

We present the medium-term results of 33 patients treated with Urolume Plus urethral stent between August 1990 and June 1996 in 9 Belgian and Luxembourg centers. They all had previous treatments for bulbo-membranous urethral stricture, but without sustained benefit (dilatation, internal urethrotomy, or/and urethroplasty). The mean age of the stricture was 5.4 years (+/- 4). The stent was inserted easily during a short hospitalisation. Median time of follow-up is two years (6 months to 4 years). Maximum flow rate at last follow-up was 20.7 ml/sec (+/- 7), and 83.3% of the patients were satisfied with the stent. In 70% of the cases, the stent achieved its purpose of maintaining a good urethral lumen. In 5 patients (15.15%), stricture recurred inside the stent and in 2 patients (6.06%) a new stricture appeared on another site. Among these 7 patients, 5 were satisfactorily treated by endoluminal resection (4 cases) or dilatation (1 case). There were 2 real failures (= 6%): one patient who has refused any complementary endoscopic treatment and the other one who is still undergoing repeated urethral dilatations. Having respected the right indications for this device, we are satisfied of the results. It is a good alternative after failure of other treatments for bulbar-membranous stricture.

RESUMO

Authors report a case of right ureteral triplication complicated with right hydronephrosis of the upper kidney and vesico-ureteral reflux concerning the middle kidney. Surgical technique is illustrated and a brief review of literature completes this case-report.

In the service, sixty patients underwent a radical prostatectomy between 1987 and 1994. The last 35 candidates to radical prostatectomy were prospectively analyzed in urodynamic parameters preoperatively and two months afterwards. Five patients present an incontinence: two total incontinence and three moderate incontinence. In two of the patients there was a detrusor instability, in two others an urethral instability and in the last patient, we discover a diminution of the bladder functional capacity on the postoperative urodynamics. So, the question is now to determine if incontinence is only caused by a sphincteric insufficiency or by a bladder or urethral dysfunction. The preoperative urodynamics are important to appreciate the quality of sphincters and the quality of the bladder; it's also an important meeting between the patient and the surgeon. The postoperative urodynamics seem to be reserved to the patients who have some problems, so to search an explanation for the incontinence. All our patients underwent rehabilitative physiotherapy, which seems to help them to recover their continence.

RESUMO

We describe one case of hemangiopericytoma touching a 44-year-old man. It is a rare vascular tumor originating from pericytes surrounding capillaries. The main typical features of it are the difficulty in doing an histologic diagnosis and the impossibility to make out a reliable prognosis. The therapy of choice is a surgical procedure sometimes in combination with radio- and/or chemotherapy.

RESUMO

Endoscopic sphincterotomy, AMS 800 artificial urinary sphincter and enlargement cystoplasty for treating a neurologic dyssynergic bladder. Association of endoscopic sphincterotomy made on a first operating time, with AMS 800 artificial sphincter and enlargement cystoplasty is a good choice for treating selected patients with a neurologic dyssynergic bladder. The author report his experience and his technique in a case of congenital malformation of the medullary cone.

RESUMO

Between 1963 and 1978 the authors operated 371 patients for adenocarcinoma of the colon. The operability rate was as high as 99%. Sixty-three percent of the group were women. The average age was 66 years. A resection could be done in 86.5% of the cases; of those 75% were considered curative. Only 6.7% were stage A. Right-sided lesions were treated with a right hemicolectomy. On the left side a left hemicolectomy was the operation of choice. A segmental resection was performed when local and general factors were unfavorable. The operative mortality (within 30 days) was 12.5% for the total group for "curative" resections the mortality was 9.9% all cases considered and 7.3% for these operated upon between 1971 and 1978. The survival rate for all operated patients is 34.8% (corrected 42.1%) 5 years later and remains about the same at 10 years (29.3%) (corrected 41.9%).
