OBJECTIVE: The purpose of this study was to compare cytologic findings of fluid from ovarian cysts with ovarian histologic findings. STUDY DESIGN: Ovaries submitted for pathologic examination were grossly examined for ovarian cysts. Fluid was removed by needle aspiration from intact ovarian cysts and prepared for cytologic examination. The cytologic findings were categorized as benign, malignant, indeterminate, and nondiagnostic. Histologic classification was assigned using the guidelines established by the World Health Organization. A single pathologist evaluated each cytologic specimen and was blinded to the gross appearance and histologic findings of each ovary. Cytologic diagnoses were compared with the histologic diagnoses. RESULTS: The study material consisted of 105 ovaries from 98 women. A comparison of the ovarian histologic findings with the cytologic diagnosis was performed in 89 of 105 cases. Histologic examination of the ovaries revealed 89 benign ovarian tumors and 13 ovarian carcinomas. The sensitivity of ovarian cyst cytologic evaluation was 25%, and the specificity was 90%. The false-positive rate for ovarian cytologic evaluation was 73%, and the false-negative rate was 12%. CONCLUSIONS: Cytologic examination of aspirated ovarian cyst fluid does not accurately predict ovarian histologic findings.

RESUMO

With advances in minimally invasive techniques, the safety and efficacy of laparoscopic adrenalectomy is well documented. Compared with open surgery, the laparoscopic approach results in shorter hospital stay, reduced morbidity, and more rapid recovery. In the setting of pheochromocytoma, tumor resection is feasible although somewhat more difficult because of catecholamine liberation and frequent large lesion size. We report the successful laparoscopic resection of a para-aortic, extra-adrenal paraganglioma using ultrasound guidance.

Primary epididymal malignancies are uncommon, and epididymal adenocarcinomas are exceedingly rare. We report a case of primary adenocarcinoma of the epididymis in a 62-year-old man and review the world literature on epididymal adenocarcinoma.

A 30-year-old black man came to the emergency department with gross hematuria, prostatitis, and urethritis 4 months after immigrating to this country from the Sudan in Africa. Urinalysis revealed hematuria and no ova or parasites. An intravenous urogram showed normal kidneys and ureters. The patient had cystoscopy with biopsies of an inflamed and ulcerated bladder mucosa. The nematode Schistosoma haematobium and schistosome eggs were identified without evidence of urothelial malignancy. The patient was treated with praziquantel and is currently asymptomatic. Over 200 million people are infected with schistosomiasis worldwide, yet this disease is rarely encountered in this country. The differential diagnosis and assessment of patients must include exposure to uncommon diseases that are endemic to other geographic regions.

Hemangioma of the esophagus is a rare tumor. Fewer than 100 cases have been found in the literature. The present case report describes a patient who experienced a sudden massive hematemesis and demonstrated unusual changes on the computed tomographic scan.

OBJECTIVE: The optimal management of cervical intraepithelial neoplasia after cold knife conization remains controversial. Reliable predictors of residual dysplasia in the cervix after cold knife conization have not been consistently identified. This study was initiated to examine the accuracy of the traditional factors used to predict residual dysplasia in hysterectomy specimens after cold knife conization. STUDY DESIGN: A retrospective 10-year chart review identified a cohort of 1272 patients who underwent cold knife conization, of whom 311 had a subsequent hysterectomy within 1 year of conization. Residual disease was defined as cervical intraepithelial neoplasia or cancer in the hysterectomy specimen. All cone specimens were completely submitted for pathologic examination, and the following factors were analyzed for their predictive value: degree of dysplasia, margin involvement, endocervical gland involvement, and status of the endocervical curettage. The predictive value of age, race, gravidity, parity, socioeconomic status, cigarette smoking, and marital status were also examined. The chi 2 test, t test, and logistic regression were used for statistical analysis. RESULTS: Dysplasia or cancer were identified in 1066 (84%) of the 1272 patients who underwent cold knife conization. Of the 311 patients having a subsequent hysterectomy, 106 (34%) had residual disease in their hysterectomy specimen. By multivariate analysis only increasing age and degree of dysplasia were predictive of residual disease. The odds ratio of residual disease in the hysterectomy specimen for a 25-year-old woman was 2.7 (95% confidence interval 1.6 to 4.4) compared with a 40-year-old woman whose odds ratio was 4.9 (95% confidence interval 2.2 to 10.8). The presence of dysplasia in the cold knife conization specimen conferred an odds ratio of 12.1 (95% confidence interval 2.7 to 54.5) of identifying residual disease. Dysplasia involving the ectocervical margin, endocervical margin, and endocervical glands was not predictive of disease in the hysterectomy specimens. Endocervical curettage was not performed in 44% of the patients, preventing reliable statistical evaluation. Further analysis indicated that residual disease was found in 32% of the hysterectomy specimens with negative margins, in 31% with no endocervical gland involvement, and in 23% with a negative endocervical curettage sample. CONCLUSIONS: The presence or absence of dysplasia in the cold knife conization ectocervical margin, endocervical margin, and endocervical glands was not predictive of residual dysplasia in post-cold knife conization hysterectomy specimens. Increasing age and severity of disease in the cone specimen were the only factors that accurately predicted residual dysplasia. The traditional factors used to justify hysterectomy after cold knife conization may not be valid on the basis of these results.

The authors report four cases of patients in desperate clinical situations where cyanoacrylate adhesive (Krazy Glue) was successfully used to control hemorrhage. Clinical observations were supplemented with bacteriological studies which showed that commercially available cyanoacrylate adhesive showed lack of bacteriological contamination.

This preliminary report describes 5 consecutive patients operated on for drug-resistant ventricular tachycardia (VT). All were successfully treated with laser photocoagulation ablation alone. The continuous-wave neodymium:yttrium-aluminum garnet (Nd:YAG) laser (wavelength, 1.06 micron) was chosen because of its capability for controlled deep tissue penetration, which can be adjusted by manipulating the power and exposure time of the beam. All patients had severe coronary artery disease. Preoperative left ventricular ejection fractions were low (0.18 to 0.29). Risk factors associated with increased failure rates by conventional surgical approaches were frequent: absence of discrete left ventricular aneurysm (5 patients) and multiple VT morphologies with disparate sites of origin (4 patients). All patients recovered fully. VT was not inducible prior to discharge, and no patient was placed on a regimen of antiarrhythmic drugs. Current direct surgical approaches to drug-resistant VT have markedly improved operative results compared with indirect procedures. However, failures and mortality remain high. Laser photocoagulation obviates some of the problems associated with conventional methods. It is similar to cryotherapy in that the structural integrity of affected tissues is maintained. In contrast to cryosurgery, however, laser photocoagulation is achieved more rapidly and with more precise myocardial destruction. One of the most promising features of laser coagulation is that it is administered to the perfused normothermic heart. Consequently, each morphological form of induced VT is observed to disappear as its area of origin is systematically located by mapping and then ablated.

The cerviscope has been proposed as a screening tool for use in conjunction with the Papanicolaou smear as a system offering many of the same advantages as colposcopy and cytology. One hundred twenty-five patients were evaluated with the cerviscope who had been referred to this colposcopy clinic with a diagnosis of dysplasia. The purpose of this study was to evaluate the instrument's potential as a screening adjunct in the context of simultaneous evaluation by colposcopy. Thirty-nine percent (49 patients) were unevaluable due to technical problems (defective cervicogram, nonvisualized squamocolumnar junction). Significantly, 67% (26 patients) of the 39 patients reported as having negative cervicograms had a biopsy-proven dysplasia (19 mild, 4 moderate, 3 severe). By the chi 2 method, this is statistically significant (P less than 0.001).

Two patients with osteogenic sarcoma of the proximal tibia were treated pre-operatively with intensive chemotherapy with high dose methotrexate (HDMTX) with citrovorum factor rescue (CFR) and adriamycin (ADR). One patient died before surgery. The other underwent en bloc resection of the primary tumor with prosthetic replacement of the involved tibia, following by adjuvant chemotherapy consisting of HDMTX with CFR and ADR, but died of metastases. Complete control of the primary or metastatic tumor was not achieved. It is emphasised that intensive multiple drug chemotherapy should be administered with extreme caution. The histologic findings are carefully analysed in relation to objective tumor response and toxic chemotherapy effects.
