RESUMO

Urinothorax is an unusual cause of pleural effusion. To describe a case with urinothorax secondary to urinary system trauma. 41-year-old male readmitted to our hospital 16 days after a right percutaneous nephrolithotomy with fever, shortness of breath, chest pain and multiloculated pleural effusion confirmed by CT scan, resolved with surgical treatment. Readmitted twenty-one days later with recurrent empyema and diagnosis of reno-pleural fistula, treated with new thoracotomy and double J and urinary catheter placement. The suspected diagnosis and the multidisciplinary approach allowed to solve this rare complication.


El urinotórax es una causa inusual de derrame pleural. Se describe un caso de urinotórax secundario a trauma del sistema urinario. Varón de 41 años que reingresa en el hospital a los 16 días de someterse a una nefrolitotomía percutánea derecha, con fiebre, disnea y dolor torácico. Se confirma por tomografía derrame pleural multitabicado, que se resolvió con tratamiento quirúrgico. Reingresa 21 días después con empiema recidivante y se diagnostica una fístula renopleural, que fue tratada con retoracotomía y colocación de catéter doble J y sonda vesical. La sospecha diagnóstica y el manejo multidisciplinario permitieron resolver esta rara complicación.

Yerba mate (Ilex paraguariensis St. Hil.) has been studied for its important biological activities mainly attributed to phenolic compounds. This study evaluated the antimicrobial activity of methanolic and ethanolic extracts of yerba mate against food pathogens, such as Staphylococcus aureus, Listeria monocytogenes, Salmonella Enteritidis and Escherichia coli through minimum inhibitory (MIC) and bactericidal (MBC) concentrations, in addition to the determination of chemical composition by gas chromatography with mass spectrometry (GC-MS) and phenolic content. The most effective extract had its activity evaluated under different pH conditions by growth curve analysis. All microorganisms except E. coli were inhibited. The ethanolic extract showed the lowest MIC/MBC (0.78/0.78 mg/ml), the highest phenolic content (193.9 g.GAE/kg) and the presence of chlorogenic acid derivatives, especially 3-O-caffeoylquinic and caffeic acid. This extract was able to inhibit microbial growth at pH 7 and 8.

BACKGROUND: Operative and nonoperative treatment of anterior cruciate ligament (ACL) injuries is often associated with a lack of proprioception and persistent muscle weakness of unknown origin. METHODS: This long-term study in the cat experimentally compares both neural and muscular activity in the articular nerves of the knee (PAN and MAN), quadriceps and hamstrings, in the chronic unstable knee, and in the reconstructed knee. We also investigated changes in neuromuscular response due to the mechanical competence of the graft, comparing stable and unstable reconstructed knees. RESULTS: We found increased periarticular muscle activity during anterior tibial translation in chronically unstable knees. Both reconstructed and non-reconstructed knees lost fast reactive activity in the articular nerves. When stability was recovered after reconstruction, the knees showed a more adjusted - although incomplete - muscular reaction. INTERPRETATION: ACL-injured knees in the cat, with or without reconstruction, show definite abnormalities in neuromuscular reaction in the long term. Regaining stability with a competent graft in the reconstructed knee is crucial for reduction of this anomalous reaction.
