RESUMO

Objetivo: La enucleación prostática mediante láser de holmio (HoLEP) es una alternativa a la adenomectomía prostática para el tratamiento quirúrgico de la hipertrofia benigna de próstata. Analizamos nuestra curva de aprendizaje de dicha técnica y la comparamos de forma secundaria con las adenomectomías de próstata. Materiales y métodos: Estudio retrospectivo y comparativo donde se incluyeron los 100 primeros casos de HoLEP realizados en nuestro centro y los últimos 50 casos de adenomectomías retropúbicas. Se recogen los datos del paciente, de la intervención, del acto anestésico, de las variables perioperatorias, de las complicaciones anestésicas y de las variables postoperatorias con un seguimiento durante 6 meses. Se analizó la curva de aprendizaje sin mentor del HoLEP y se compararon las características del HoLEP en 2 fases diferenciadas (fase de aprendizaje y fase de estabilización) con las últimas adenomectomías retropúbicas de próstata realizadas. Resultados: La anestesia intradural fue la técnica más frecuente. Las necesidades de transfusión, el tiempo de estancia (p < 0,01) y la morbilidad postoperatoria fueron menores en los HoLEP que en las adenomectomías. Sin embargo, el grupo de las adenomectomías retropúbicas tenía un mayor volumen prostático inicial (p < 0,001) y un menor tiempo quirúrgico (p < 0,001). Se observó un mejor rendimiento quirúrgico (p < 0,001) y una menor incidencia de complicaciones en el Grupo HoLEP-B (superada ya la curva de aprendizaje) frente al grupo HoLEP-A. Conclusión: En nuestro centro el HoLEP se ha introducido como una opción válida frente a las adenomectomías retropúbicas abiertas, con muy buenos resultados en términos de morbilidad y reducción de estancia hospitalaria. Respecto a la curva de aprendizaje consideramos en torno a los 50 pacientes (sin mentor) como un punto de corte adecuado. La anestesia regional es una buena elección como técnica anestésica

However, the retropubic adenomectomy group had larger initial prostate volumes (P < .001) and shorter surgical times (P < .001). Better surgical performance (P < .001) and a lower incidence of complications were observed in the HoLEP-B group (once the learning curve had been overcome) compared with the HoLEP-A group. Conclusion: In our center, HoLEP was introduced as a valid alternative to open retropubic adenomectomy, with excellent results in terms of morbidity and reduced hospital stay. In terms of the learning curve, we consider that approximately 50 patients (without mentor) is an appropriate cutoff. Local anesthesia is a good choice for the anesthesia technique

OBJECTIVE: Involvement of extralymphatic tissue in chronic lymphocytic leukemia (CLL) is uncommon and that of the epididymis and spermatic cord is rare. This paper describes an unusual case with metastasis to both epididymis and the spermatic cord as the first sign of reactivation of CLL. METHODS/RESULTS: A patient with CLL is described. The first sign of disease progression was the involvement of both epididymis and the spermatic cord. The literature on tumors of the epididymis and spermatic cord and CLL is briefly reviewed. CONCLUSION: Metastatic tumors of the epididymis and spermatic cord are rare. The importance of the pathological findings in making the diagnosis is underscored.

OBJECTIVES: To review the main features of malignant pheochromocytoma and its relationship to von Recklinghausen's neurofibromatosis. METHODS/RESULTS: We report on a case of malignant bilateral pheochromocytoma with hepatic and pulmonary metastasis, associated with von Recklinghausen's neurofibromatosis. Surgery achieved good results and the patient is well seven years postoperatively. CONCLUSIONS: Approximately 10% of the pheochromocytomas are malignant and treatment is principally by surgery. (131) metaiodobenzylguanidine may be useful in cases with metastasis, residual tumor or local relapse.

RESUMO

OBJECTIVES: The possible solutions to a case of ureteral duplication complicated by obstruction of one of the collecting systems are reviewed. METHODS/RESULTS: We report an uncommon case of ureteropelvic junction obstruction in the lower system of an incomplete duplication of the collecting systems that was resolved by pyeloureterostomy. CONCLUSIONS: Anastomosis of the lower renal pelvis to the upper ureter may provide a simple solution for the functional recovery of an important renal segment.

RESUMO

OBJECTIVES: We report on a patient with an increase in testicular size that had been initially diagnosed as neoplasm. METHODS: The ultrasound and histological findings, the atypical clinical features and possible causes are analyzed. RESULTS: Segmental infarction with diffuse testicular atrophy. CONCLUSION: Infarction of testis may atypically present as a testicular mass that may lead us to suspect a testicular neoplasm since the age of presentation of both types of lesions are similar.

OBJECTIVES: To analyze the long-term results of treatment of urethral strictures by internal endoscopic urethrotomy, a simple, easily reproducible procedure that carries a low morbidity and requires a short period of convalescence. MATERIAL AND METHODS: We report our experience of 78 cases submitted to urethrotomy as primary treatment. The results at 19.7 months mean follow-up (range 6 months--5 years) are presented. A maximum of three Sachse procedures were performed if stricture recurred. RESULTS: The complications were rare. Although the results were poor in only 21% of the patients 6 months postoperatively, 46% had recurrent strictures. Moreover, nearly 50% of these strictures recurred after a second or third urethrotomy. CONCLUSIONS: The nature of the stricture, determined by spongiofibrosis, is the main prognostic factor. Internal urethrotomy is indicated in urethral strictures without exorbitant spongiofibrosis, otherwise stricture will recur after internal urethrotomy and other treatment modalities will be required.

OBJECTIVES: The present study was conducted to review the epidemiological factors of patients with renal cell carcinoma. METHODS: We reviewed the familial, social, cultural, toxic, occupational and pathological factors of 229 patients who were diagnosed as having renal cell carcinoma between 1975 and 1992 at our hospital. RESULTS: As in other series, we found a greater predisposition in male patients around the seventh decade. However, unlike other studies, we observed a greater predisposition in the lower social and cultural levels and rural population. Of the epidemiological factors related to adenocarcinoma of the kidney, smoking was the most common in this series (41.8%), followed by high blood pressure (15.7%) and toxic-occupational factors (8.2%). We found no correlation between these factors and tumor stage or survival. CONCLUSIONS: Our data show a greater predisposition for renal cell carcinoma in male patients, around the seventh decade, in the lower social and cultural levels and rural population. Smoking, high blood pressure and occupational exposure to toxic agents were the most common epidemiological factors.

RESUMO

Though renal leiomyoma is an unusual tumour, symptomatic tumours are even more uncommon. They often raise problems with differential diagnosis from malignant tumours which not even the most advanced imaging techniques manage to elucidate. On the other hand, their sarcomatous degeneration has been described. Considering these premises, doubts arise on their therapeutical approach, on which we here present our own views.

Herein we describe a case of penile incarceration due to a nut at the base of the penis which was resolved by cutting the metallic object. The literature on penile strangulation describes different foreign bodies and methods of management of constrictive penile band injury.

RESUMO

Ureteral iatrogenic lesions occur with variable frequency in the different types of surgery, mainly in uncomplicated abdominal hysterectomy. This paper contributes 29 patients (2 male and 27 female) with several types of lesions, secondary to gynaecological, intestinal and vascular surgery. The series produced 16 cases of clamping, 6 ureterovaginal fistulae, 6 ureteral sections (5 total and 1 partial) and 1 ureterouterine fistula. The terminal ureter was the most frequently damaged. The diagnostic and therapeutical methods used in this type of lesions are analyzed.

We report an additional case of ectopic ureter opening into the seminal vesicle in a patient who had consulted for nephric colic from calculus in dysplastic kidney and urinary tract infection. The clinical features, the diagnostic and therapeutic aspects of this rare malformation are briefly reviewed.

RESUMO

Over the last few years, a better anatomical and functional knowledge of the adrenal glands has allowed us to diagnose and treat optimally most of these gland's conditions. A variety of medical (endocrinology, anaesthesiology) and surgical specialties have been used to this aim. This paper reviews the adrenal gland's diseases treated in our unit over the last 9 years. Pre-operative management, ways of approach, morbidity and mortality as well as results are analyzed and compared with those obtained by other authors.

Presentation of two new cases of vesico-uterine fistulae secondary to cesarean operation. In one case our route for surgical approach is explained as a novelty; also, the time-frame within which it was performed, one month, since it is usual to recommend a minimum of three months. In the second case an in-dwelling vesical catheter was sufficient to achieve closure. The article also reviews the existing literature on the different aspects: signs, symptoms and therapy, of this infrequent pathology.

RESUMO

The renal adenocarcinoma is not a frequent tumour within the scope of multiple neoplasias, at least in the clinical series. Conversely, it appears at a higher relative rate in the post-mortem series. This paper submits a review of renal adenocarcinoma associated in life to other malignant neoplasias, enumerating their major clinical and epidemiological data. Also, it reviews the literature with regard to frequency and etiology, applying the data to our series. The paper concludes that hypernephroma is not a tumour frequently associated with others in life, given its tendency to be secondary to other primary ones, therefore its development depending on survival to the initial tumour. It also emphasizes the need for adequate anamnesis aimed to detect multi-cancer syndromes and the etiology of multiple neoplasias. Further studies are needed to understand the scope of multiple neoplasias and their relative risk.
