RESUMO

This study evaluated the state of environmental health in the Republic of South Africa using the strengths, weaknesses, opportunities, and threats (SWOT) analysis framework. The internal and external factor evaluations were conducted by reviewing existing literature searched using a combination of keywords and boolean functions. The weighted score for the strengths and weaknesses was 0.79 and 1.04, respectively. The external factor evaluations findings revealed that environmental health has an equal amount of threats and opportunities, however, the threats outweigh the opportunities. The total weighted score for the internal and external factor evaluations was 1.83 and 2.25, respectively, which were below the average value of 2.5. The internal-external factors matrix indicated that environmental health in the Republic of South Africa needs to be refocused and re-aligned to public health protection. Failure to address the weaknesses and threats and optimise the strengths and opportunities may further weaken the provision of environmental health services.

Particulate matter (PM) of different sizes and elemental composition is a leading contributor to indoor and outdoor air pollution in residential areas. We sought to investigate similarities between indoor and outdoor PM2.5 in three residential areas near a ferromanganese smelter in Meyerton to apportion the emission source(s). Indoor and outdoor PM2.5 samples were collected concurrently, using GilAir300 plus samplers, at a flow rate of 2.75 L/min. PM2.5 was collected on polycarbonate membrane filters housed in 37 mm cassettes coupled with PM2.5 cyclones. Scanning electron microscopy coupled with energy-dispersive spectroscopy was used to study the morphology, and inductively coupled plasma-mass spectroscopy was used to analyse the elemental composition of the PM2.5. Mean indoor and outdoor PM2.5 mass concentrations were 10.99 and 24.95 µg/m3, respectively. Mean outdoor mass concentration was 2.27-fold higher than the indoor concentration. Indoor samples consisted of irregular and agglomerated particles, ranging from 0.09 to 1.06 µm, whereas outdoor samples consisted of irregular and spherical particles, ranging from 0.10 to 0.70 µm. Indoor and outdoor PM2.5 were dominated by manganese, silicon, and iron, however, outdoor PM2.5 had the highest concentration of all elements. The ferromanganese smelter was identified as the potential main contributing source of PM2.5 of different physicochemical properties.

Only 15% of the global population has access to occupational safety and health services. In Africa, only 5% of employees working from major establishments have access to occupational health services (OHS). Access to primary health care (PHC) services is addressed in many settings and inclusion of OHS in these facilities might increase efficiency in preventing occupational diseases. A cross-sectional study was conducted in four Southern African Development Community (SADC) countries aiming at assessing the availability of OHS at PHC facilities and the organization of OHS. We conducted a literature review to assess the provision and organization of OHS services. In addition to the review, a total of 23 doctors from Zambia were interviewed using questionnaires in order to determine the availability of OHS and training. Consultations with heads of ministries were done in four SADC countries. Results showed that in the SADC region, OHS are fragmented and lack a comprehensive approach. In addition, out of 23 PHC facilities, only two (13%) provided occupational health and PHC. However, OHS provided at PHC facilities were limited to TB screening and audiometric testing. Our study showed a huge inadequacy of trained occupational health practitioners. This study supports the World Health Organization's advocacy to integrate OHS at the PHC level.

A D-grade type coal was burned under simulated domestic practices in a controlled laboratory set-up, in order to characterize the emissions of volatile organic compounds (VOCs); namely, benzene, toluene, ethylbenzene, and xylenes (BTEX). Near-field concentrations were collected in a shack-like structure constructed using corrugated iron, simulating a traditional house found in informal settlements in South Africa (SA). Measurements were carried out using the Synspec Spectras GC955 real-time monitor over a three-hour burn cycle. The 3-h average concentrations (in µg/m³) of benzene, toluene, ethylbenzene, p-xylene, and o-xylene were 919 ± 44, 2051 ± 91, 3838 ±19, 4245 ± 41 and 3576 ± 49, respectively. The cancer risk for adult males and females in a typical SA household exposure scenario was found to be 1.1 and 1.2 respectively, which are 110- and 120-fold higher than the U.S. Environmental Protection Agency (EPA) designated risk severity indicator (1 × 10-6). All four TEX (toluene, ethylbenzene, p-xylene and o-xylene) compounds recorded a Hazard Quotient (HQ) of less than 1, indicating a low risk of developing related non-carcinogenic health effects. The HQ for TEX ranged from 0.001 to 0.05, with toluene concentrations being the lowest, and ethylbenzene the highest. This study has demonstrated that domestic coal burning may be a significant source of BTEX emission exposure.
