Examination of 45 patients with acute oral poisonings by psychopharmacological agents who underwent for the body's detoxjication intestinal lavage using salt enteral solution that was isoionic to chymus and isoosotic to plasma has revealed its preventive action on the development of tissue hyperhydration and its corrective effect of plasma ionic composition. It has been ascertained that after intestinal lavage, there are significant increases in partial arterial blood oxygen pressure by 80% and in arteriovenous oxygen difference by 90%.

Serotonin adipinate in a dose of 40-60 mg was injected by drip infusion to 30 patients with acute poisoning by psychotropic drugs for correcting circulatory disorders. The drug corrected hemodynamic disorders in the lesser and greater circulation by increasing the stroke volume and normalizing total peripheral vascular resistance and central blood volume.

Use of sodium hypochlorite in combined therapy of patients with alcoholic delirium complicating acute poisoning with psychotropic drugs, caustic poisons, alcohol and its surrogates resulted in an appreciable decrease in the duration of alcoholic delirium.

Ninety-five patients with acute poisonings with psychotropic agents, barbiturates, amitriptyline, leponex, cholinolytics, and organophosphorus insecticides were treated by gastro-enteroadsorption making use of activated coal. In order to enhance propulsive function of the gastrointestinal tract, serotonin adipinate was repeatedly administered in a dose of 5 to 20 mg. Evacuation of the enteroadsorbent in the rectum was enhanced fivefold in comparison with a control group of similar patients administered traditional drugs (proserine, pituitrin, cerucal). As a result of such therapy, the toxicogenic phase of poisoning was shortened and mortality in the observed group reduced twofold in patients poisoned with psychotropic agents, 5.4 times in those with barbiturate poisoning, and 3.5 times in those with organophosphorus poisoning.

Esophagogastroduodenoscopy was carried out with diagnostic and therapeutic purposes in patients with acute poisonings with noncaustic chemicals during the acute period. Diagnostic endoscopy was performed in 142 patients. Gastrointestinal hemorrhages were detected in 96 (67.6%) cases. In 31 patients endoscopic intervention was performed to arrest gastrointestinal bleeding and prevent its recurrence. In 305 patients a tube for intestinal lavage was introduced in the small intestine with an endoscope by various methods.

The paper gives evidence for the fact that serotonin and serotonin receptors are responsible for the occurrence of primary contractions of smooth muscles, primary cardiac contractions and primary brain electric activity (EEG genesis). It is concluded that the function of the smooth muscle, brain and heart cannot take place without serotonin and its receptors. This suggests that the serotonin receptors are life ones.

The paper outlines the previously unknown smooth muscle insufficiency in terms of its etiology, pathogenesis, diagnosis and treatment. It has been shown that drug administration leads to smooth muscle insufficiency which is eliminated by serotonin both in vitro and in vivo. The clinical manifestations of smooth muscle (SM) insufficiency are eliminated by serotonin, irrespective of whether it has been caused by the use of drugs or impaired smooth muscle innervation. The mechanism of vascular rhythmic oscillations in the microcirculatory bed, the so-called endogenous vasomotility (EV) has been decoded. The mechanism of EV regulation is due to the fact that platelets constantly (continuously) adsorb serotonin from the enterochromaffin cells of the gastrointestinal tract and constantly (continuously) release it into the microcirculatory bed, thereby providing the continuum of entry of the stimulant to the SM fibers which are able to contract at this time. The summation of these contractions of smooth muscle contractions in the microcirculatory bed maintains their vascular tone and yields the pattern of EV. The paper also describes the earlier unknown properties of hemoglobin and myoglobin to cause smooth muscle spasm and to accelerate platelet destruction. A correlation between the EV and vascular platelet hemostasis is described in terms of these properties. The continuous mechanism of EV is impaired and it transforms to intermittent (final) vascular platelet hemostasis.

A combined method for body detoxification including extracorporeal purification of the blood outflowing from the intestine with sorbents and emergency evacuation of the small intestine contents by means of a tube was employed in acute experiments on dogs after oral administration of the lethal doses of veronal. Tht use of the method interferes with the poison supply from the gastrointestinal tract to the systemic blood flow, thereby leading to acceleration of body detoxification.
