RESUMO

Patients with varicose veins of the lower extremities with osteoarthritis of the knee often experience improvement in knee joint symptoms after endovascular treatment. We considered that it was important to decide the operation indication of lower extremity varices, to know the correlation between the two diseases in the treatment of varicose veins. To investigate the postoperative improvement of knee symptoms related to varicose veins with knee osteoarthritis, we conducted a questionnaire survey for a total of 12 months, from December 2014 to May 2015 and from October 2018 to March 2019. The participants were 35 patients (7 men and 28 women) with varicose veins complicated with knee osteoarthritis. We classified knee osteoarthritis according to a grading scale and compared the improvement of knee symptoms after endovenous thermal ablation. The higher the knee grade, the lower the degree of improvement. However, the improvement was observed in all knee osteoarthritis grades, and as a whole, 25 patients (71.4%) have experienced improvement of subjective symptoms. For patients with knee osteoarthritis, we strongly recommend surgical treatment of the varicose veins regardless of the progression of knee grade. OBJECTIVE: Although neoadjuvant chemotherapy(NAC)has been recognized as an important option for improving the clinical outcome of patients with advanced gastric carcinoma, convincing evidence that it prolongs life and brings about a good prognosis are both lacking. We retrospectively evaluated the efficacy and safety of NAC in ten patients with advanced gastric cancer. METHODS: A total of ten patients with advanced gastric cancer, who received NAC with the combination of S-1 and cisplatin in our hospital from April 2008 to March 2010, were retrospectively investigated. RESULTS: A total of 5 patients responded to neoadjuvant chemotherapy, and 2 patients showed a complete regression of the primary gastric carcinoma. Four of the 5 patients who responded had solid-type poorly-differentiated adenocarcinoma. CONCLUSION: NAC with the combination of S-1 and cisplatin was suggested to be effective for advanced gastric carcinoma, especially for solid-type poorly differentiated adenocarcinomas(por1).

Case 1 was a 58-year-old woman diagnosed with unresectable liver metastases from advanced rectal cancer. We performed laparoscopic low anterior resection for sigmoid cancer. As the first-line treatment, FOLFOX+bevacizumab(BV)was applied for 16 courses. The second-line treatment, FOLFIRI plus BV, was applied for four courses. However, the disease progressed with worsening liver metastases. The sequencing of K-RAS genes from the biopsy specimens of sigmoid colon cancer revealed an expression of a wild-type K-RAS. As the third-line treatment, panitumumab was applied. After 8 courses of this chemotherapy regimen, a significant reduction in the size of liver metastases was observed. Case 2 was an 81-year-old man diagnosed with unresectable liver metastases from advanced rectal cancer. We obliged the patient by performing laparoscopic rectal resection. As the first-line treatment, XELOX plus BV was applied for 10 courses. As the second-line treatment, IRIS was applied for 6 courses. However, this failed to prevent him from having a progressive disease. As the third-line treatment, panitumumab was applied for 2 courses, and a significant reduction in the size of liver metastases was observed. Our findings suggested that panitumumab has great potential for effective treatment of patients with unresectable stageIV colorectal cancer.

RESUMO

The patient was a 77-year-old woman with multiple liver metastases of sigmoid colon cancer. She underwent low anterior resection for sigmoid colon cancer. After surgery, she selected oral administration of UFT and LV for liver metastases and multiple lymph node metastases. After two courses, the liver metastases had markedly diminished. Thirty-two months later, liver metastases had disappeared on computer tomography. This therapy was continued for five years, and recurrences are no longer shown. Severe adverse effects were not observed. Oral anti-cancer drugs can serve as effective therapy for advanced colorectal cancer of old patients.

RESUMO

The scaffold type Leeds-Keio artificial ligament (LK) for ligament reconstruction of the knee provides sufficient strength to require minimal sacrifice of autogenous tissue. This implant works not only as prosthesis but also as scaffold onto which natural tissue grows from the synovium. LK has been in clinical use for anterior cruciate ligament (ACL) reconstruction since 1982, although the operative procedure has been modified and has undergone significant development since that time. Recently, we developed radio frequency-generated glow discharge (RFGGD)-treated LK ligament (LKII, previously indicated as Bio-LK) to improve tissue induction and reported that cell proliferation and cell attachment to artificial fibers increased considerably with this hydrophilic treatment. In this study, we report the findings of reconstructed ACL using LKII, which has been in clinical use since 2003. At reconstruction, LKII was covered with the remnant of the original ACL as much as possible in order to preserve the nerve ending system. Thirteen cases with over 12 months' postoperative history were reviewed. Knee stability was regained after reconstruction without any complications such as joint effusion and chronic synovitis. In one case, postoperative arthroscopy showed that the reconstructed ACL was completely covered with newly formed tissue at 8 weeks postoperatively. Biopsy revealed abundant fibroblasts, collagenous fibers, and vessels around the artificial fibers without marked inflammatory findings. Transmission electron microscope study showed abundant thin collagen fibers, which demonstrated regular orientation to some extent. Fibroblasts were observed with extensive amount of rough endoplasmic reticulum. According to these results, we consider LKII to be superior to LK in tissue induction and maturation.
