In the world literature there are descriptions of about 60 cases of the Caroli disease representing a congenital dilatation of intrahepatic bile ducts. Unlike true cysts of the liver the cystoid dilatation of the ducts in Caroli disease contains bile and features of chronic cholangitis prevail in the clinical picture of the disease. The diagnostics is difficult. Intraoperative cholangiography is the most informative method.

214 patients with pyloric ulcer were examined. All of them underwent endoscopic examination of the upper parts of the gastrointestinal tract. Malignant pyloric ulcer was detected in 20 patients; 19 of them were operated on. Histological examination of the resected material established malignant transformation of chronic ulcer in 9 patients out of 20 (4.4% of all cases of chronic pyloric ulcer). A concept of pylorobulbaric ulcer is suggested. It was found that this type of ulcer may undergo malignant transformation, too. Endoscopy proved to be the major diagnostic method in identification of malignant pyloric ulcer. The results of endoscopic examination with aiming biopsy provide the basis for determining the extent of surgical procedure. Endoscopic symptoms of malignant pyloric ulcer are discussed. It is noted that the latter were absent in one third of cases. No malignancies were detected in those cases during surgery, either.

The article is concerned with endoscopic investigations in different types of hemorrhage from the gastric mucosa, acute erosion, zone of anastomosis and in disturbed evacuation from the gastric stump and after creation of choledochoduodenal anastomoses. Typical clinical examples are described. The authors propose useful practical recommendations.

Two observations of an aberrant pancreatic gland in the gallbladder wall revealed by histological examination of the surgical material are presented. In one case the aberrant pancreatic gland was represented by the exocrine part and ducts in the other by adenomyoma.

46 endoscopic electroexcisions of gastric polyps were performed in one case this procedure was complicated with bleeding, that would not be arrested via an endofibroscope. This operative procedure is described, and also some considerations concerning endoscopic electroexcision of gastric polyps are given.
