RESUMO

Mean gestational sac volumes of 9 singleton pregnancies induced by human menopausal gonadotropin (hMG) measured by ultrasound were compared with mean gestational sac volumes obtained from 37 normal singleton pregnancies, 6 to 8 weeks from the last menstrual period. HMG-induced gestational sacs were smaller at 6 and 7 weeks of gestation, compared with normal pregnancies. Despite the initial lag in gestational sac volume, hMG-induced pregnancies subsequently demonstrated appropriate fetal growth as measured by crown-rump length measurements between 8 and 12 weeks. These pregnancies also demonstrated normal fetal growth patterns in the second and third trimesters of pregnancy, as assessed by clinical evaluation and ultrasound. These preliminary data suggest that early growth development in hMG-induced pregnancies may be different from that in spontaneously occurring pregnancies.

Artifacts interfering with sonographic imaging of the uterine cervix in pregnancy are described. Increasing the amount of fluid in the urinary bladder closes the cervix and alters the configuration of the lower uterine segment, while increased intra-uterine pressure results in reciprocal changes. Angulation of the transducer and manual pressure thereon distort sonographic findings. It is concluded that, contrary to recent reports in the literature, sonography cannot reliably demonstrate changes in cervical compliance during pregnancy. Vaginal examination remains the ultimate diagnostic modality for the diagnosis of the incompetent cervix, while sonography may be used as an adjunct only.

RESUMO

The prenatal sonographic presentation of Hirschsprung's disease is described. This report strongly suggests that bowel distention in Hirschsprung's disease may begin in the late third trimester of intrauterine life. The importance of early diagnosis and treatment cannot be overemphasized. Sonography may greatly contribute to the early diagnosis of Hirschsprung's disease.

Fetal inner orbital diameter and outer orbital diameter were measured in the occipital transverse and occipital posterior positions in 180 normal pregnancies. The outer orbital diameter was found to be closely related to the biparietal diameter. After a nomogram had been constructed, 463 patients whose fetuses were at risk for various anomalies were studied in two cases, hypotelorism was suspected because the orbital measurements fell below the 95% confidence limits, and this diagnosis was confirmed at the time of delivery. The conclusion is that, in situations in which fetal head position precludes accurate biparietal diameter determinations, the outer orbital diameter can often be obtained and used to estimate that variable and, therefore, gestational age. Furthermore, the nomogram constructed has proved to be a useful tool in diagnosing hypotension in utero. Theoretically, this should also be true of fetal hypertelorism.
