The aims of this study were to investigate the medical and dental care of children born in Mayo with cleft lip and/or palate in the period 1999-2007. Thirteen subjects were identified--nine males and four females. Galway was the main locus for cleft surgical referral and care, with 10 subjects receiving treatment by the same surgeon. A total of 54% of subjects required ENT care, with 62% receiving speech and language therapy. DMFT and dmft were zero and 0.037, respectively. Mean age at first dental visit (to a private or community dental clinic, specialist paediatric dental practice or Western Health Board orthodontic service) was 21 months. This represents progress in the objective of early access and provision of dental care for this special care dentistry needs group.

Axenfeld-Rieger syndrome (ARS) is a rare, autosomal dominant condition characterized by ocular, craniofacial, dental, and periumbilical abnormalities. Relatively little information exists on this syndrome within the dental literature despite the fact that midface hypoplasia and maxillary hypodontia are classical presenting features of this syndrome. This is a case report of a 7-year-old Caucasian female with ARS who presented with significant ocular and dental anomalies. She was also found to have osteopenia. Her dental condition is described, her immediate treatment is shown, and her long-term treatment needs are discussed.

While Marcus Gunn phenomenon (MGP) is well documented in the medical literature, little data exist within the dental literature. This is a case report of an adolescent with MGP who presented for orthodontic treatment and required bite-opening in order to treat her malocclusion. No operative complications were experienced and orthodontic treatment has been uneventful. Although MGP may be uncommon in a dental context, dentists and other oral health professionals can play a significant part in its detection and diagnosis.

Tongue piercing has many associated risks. This is a case report of a patient who, to avoid parental disapproval, hid the dorsal aspect of a lingual stud device. Subsequently, the dorsum repaired. The device could no longer be removed manually and warranted surgical removal. Despite being in situ for over two years, no further complications arose, but all efforts to obtain patient agreement for removal of the device failed.

AIM: To investigate caries experience and initial access to dental services in a group of children with cleft lip/palate in the west of Ireland. DESIGN AND METHOD: Cross sectional study with prospective data capture and matched control. Details of children born with a cleft were obtained from all health professionals likely to be involved in delivering care to these children. Existing databases were cross-referenced to eliminate duplication or missed patients. A matched control sample was recruited from 14 schools in the region. RESULTS: A sample of 90 cleft affected children (48 male, 42 female) with any category of cleft born between 1980-1996 (i.e. 16 years) was compared with a control group of 100 non cleft children (60 male, 40 female). The DMF index was determined by a trained and calibrated clinician. Twenty-two percent (n=20) of the cleft group were caries free compared to 41% (n=41) in the control group. The combined dmf/DMF for the cleft group was 2.09 compared to 1.50 for the control (P<0.05). Separate analysis of the dmf and DMF between the two groups indicated that the difference lay in the caries found in the deciduous dentition of the cleft group. The first dental visit was at 4 years of age for the cleft group. CONCLUSIONS: Cleft affected children in the region did not receive adequate and regular dental care at the appropriate time. In view of the significantly greater risk of dental disease in clefting, particularly in the deciduous dentition, all cleft affected children should be referred for comprehensive and continued preventive dental care from the first year of life.

The aim of this study was to identify and investigate care for children born, over a 17-year period (1980-1996), with cleft lip and/or palate (CL/P) in the Western Health Board region of Ireland. One hundred and six cleft-affected children were identified. The incidence was 1.14 per thousand (1:871) live births. Epidemiological findings in relation to cleft type, gender, family history, and location followed international trends. The establishment of a national Register with mandatory reporting of children with CL/P in Ireland would ensure greater consistency in data collection and may help improve co-ordination of care.

Dental ankylosis may be a significant complication in orthodontic clinical practice. This case report describes the management of a malocclusion, complicated by an ankylosed maxillary central incisor, which arose during orthodontic treatment, following an acute traumatic injury. The use of the ankylosed incisor in successfully managing the significant Class II division 1 malocclusion is described.

OBJECTIVE: To report the effectiveness of dental panoramic radiography in identifying features pathognomonic for cleidocranial dysplasia. METHODS: Panoramic radiographs of nine male Caucasian patients with cleidocranial dysplasia are analysed. RESULTS: In addition to the established dental complications of failure of eruption of the permanent dentition and multiple supernumerary teeth, morphological abnormalities of the maxilla and mandible, particularly in the ascending ramus and coronoid process were present. CONCLUSION: Dental panoramic radiography is a valuable adjunct in confirming the diagnosis of cleidocranial dysplasia.

This case report reviews the variability of dental eruption and the possible sequelae. Dental eruption of the permanent teeth in cleft palate children may be variable, with delayed eruption the most common phenomenon. A case of premature dental eruption of a maxillary left first premolar is demonstrated, however, in a five-year-old male. This localized premature dental eruption anomaly was attributed to early extraction of the primary dentition, due to caries.

A case report of a congenital midline sinus in the upper lip of a 13-year-old girl is presented. Theories proposed regarding the aetiology of this rare anomaly are discussed.

This is a case report of a Caucasian female who presented with an unusual combination of dental anomalies: short roots on the maxillary central incisors and premolars, talon cusps, dentes invaginati, low alveolar bone heights, tubercles of Carabelli on the maxillary first and second permanent molars, with pyramidal root morphology in three of the second permanent molars. None of the anomalies alone are particularly uncommon but they have not previously been reported together. The occurrence of the anomalies is probably incidental as the conditions are aetiologically unrelated.

This initial audit of 600 recently assessed Eastern Health Board orthodontic patients suggests that a large number of them (47 per cent) requires referral for routine restorative and preventive dental care. Closer links are needed with general dental practitioners and community dental surgeons to resolve these needs. The trend for a high referral of females and Class 11 Division 1 malocclusion type correlated well with studies in other countries.

This paper reviews the incidence, etiology and location of supernumerary teeth with emphasis on premolar supernumeraries and examines the management of supernumerary premolars of three patients undergoing orthodontics. These cases demonstrate that the management of premolars is assessed individually and treatments based on potential complications, which may occur during the orthodontic and surgical management of the dentition. Progress and posttreatment radiographs are recommended for the assessment of late forming supernumerary teeth.

A large intracoronal radiolucent lesion in an unerupted permanent molar was found during the routine assessment of a young male Caucasian prior to orthodontic treatment. The tooth was extracted. Histological examination indicated the lesion was caused by external resorption. The defect extended widely into the enamel and dentine, and was repaired in part by bone. The pulp chamber was not involved. The aetiology of these lesions is often obscure but in this case it appeared to have originated in the floor of two developmental pits on the occlusal surface of the tooth.
