RESUMO

El uso clínico del mindfulness adaptado a la psicosis ha sido rechazado por algunos profesionales de la Salud Mental debido a prejuicios y falta de información. Sin embargo, la evidencia actual ha demostrado que las Intervenciones Basadas en Mindfulness (MBI) son seguras y tienen numerosos beneficios en los trastornos del espectro psicótico. Las MBI son adaptables a diferentes contextos y situaciones clínicas, incluido el deterioro cognitivo, y aunque no constituye su objetivo principal, pueden facilitar la reducción de la sintomatología negativa en la psicosis. Las MBI permiten a las personas con psicosis enfrentar las experiencias alucinatorias desde una perspectiva diferente, fomentando la flexibilidad, la regulación emocional y la aceptación. Es importante adaptar las intervenciones a las características de la psicosis, así como distinguir entre los diferentes tipos de prácticas, para aplicarlas de manera diferencial según la sintomatología específica y las características de la persona.(AU)


The clinical use of psychosis-adapted mindfulness has been rejected by some mental health professionals due to biases and lack of information. However, current evidence has demonstrated the safety and numerous benefits of mindfulness-based interventions (MBIs) in psychotic spectrum disorders. MBIs are adaptable to diverse clinical contexts and situations, including cognitive impairment, and although it is not their primary objective, they can facilitate the reduction of negative symptomatology in psychosis. MBIs enable individuals with psychosis toapproach hallucinatory experiences from an alternative perspective, fostering flexibility, emotional regulation, and acceptance. It is crucial to tailor interventions to the characteristics of the psychosis, as well as to differentiate between the various types of practices, in order to apply them differentially depending on the specific symptoms and characteristics of the individual.(AU)

Untargeted metabolomics is a powerful tool with high resolution and the capability to characterize a wide range of bioactive natural products from fruit and vegetable by-products (FVB). Thus, this approach was applied in the study to evaluate the phenolic compounds (PC) by metabolomic screening in five FVB after optimizing their extraction. The total phenolic content and antioxidant activity analyses were able to select the best extractor (SM) and ultrasonication time (US) for each FVB; methanol was used as a control. Although ultrasonication yielded a lower number of PC identifications (84 PC), the US extract was the most efficient in total ionic abundance (+21% and +29% compared to the total PC and SM extracts, respectively). Ultrasonication also increased the phenolic acid (+38%) and flavonoid classes (+19%) extracted compared to SM, while the multivariate analyses showed the control as the most dissimilar sample. FVB extracted from the same parts of the vegetable/fruit showed similarities and papaya seed presented the most atypical profile. The application of the metabolomics approach increased the knowledge of the bioactive potential of the evaluated residues and possibilities of exploring and valorizing the generated extracts.

BACKGROUND: A range of safe and effective vaccines against SARS CoV 2 are needed to address the COVID 19 pandemic. We aimed to assess the safety and efficacy of the COVID-19 vaccine SCB-2019. METHODS: This ongoing phase 2 and 3 double-blind, placebo-controlled trial was done in adults aged 18 years and older who were in good health or with a stable chronic health condition, at 31 sites in five countries (Belgium, Brazil, Colombia, Philippines, and South Africa). The participants were randomly assigned 1:1 using a centralised internet randomisation system to receive two 0·5 mL intramuscular doses of SCB-2019 (30 µg, adjuvanted with 1·50 mg CpG-1018 and 0·75 mg alum) or placebo (0·9% sodium chloride for injection supplied in 10 mL ampoules) 21 days apart. All study staff and participants were masked, but vaccine administrators were not. Primary endpoints were vaccine efficacy, measured by RT-PCR-confirmed COVID-19 of any severity with onset from 14 days after the second dose in baseline SARS-CoV-2 seronegative participants (the per-protocol population), and the safety and solicited local and systemic adverse events in the phase 2 subset. This study is registered on EudraCT (2020-004272-17) and ClinicalTrials.gov (NCT04672395). FINDINGS: 30 174 participants were enrolled from March 24, 2021, until the cutoff date of Aug 10, 2021, of whom 30 128 received their first assigned vaccine (n=15 064) or a placebo injection (n=15 064). The per-protocol population consisted of 12 355 baseline SARS-CoV-2-naive participants (6251 vaccinees and 6104 placebo recipients). Most exclusions (13 389 [44·4%]) were because of seropositivity at baseline. There were 207 confirmed per-protocol cases of COVID-19 at 14 days after the second dose, 52 vaccinees versus 155 placebo recipients, and an overall vaccine efficacy against any severity COVID-19 of 67·2% (95·72% CI 54·3-76·8), 83·7% (97·86% CI 55·9-95·4) against moderate-to-severe COVID-19, and 100% (97·86% CI 25·3-100·0) against severe COVID-19. All COVID-19 cases were due to virus variants; vaccine efficacy against any severity COVID-19 due to the three predominant variants was 78·7% (95% CI 57·3-90·4) for delta, 91·8% (44·9-99·8) for gamma, and 58·6% (13·3-81·5) for mu. No safety issues emerged in the follow-up period for the efficacy analysis (median of 82 days [IQR 63-103]). The vaccine elicited higher rates of mainly mild-to-moderate injection site pain than the placebo after the first (35·7% [287 of 803] vs 10·3% [81 of 786]) and second (26·9% [189 of 702] vs 7·4% [52 of 699]) doses, but the rates of other solicited local and systemic adverse events were similar between the groups. INTERPRETATION: Two doses of SCB-2019 vaccine plus CpG and alum provides notable protection against the entire severity spectrum of COVID-19 caused by circulating SAR-CoV-2 viruses, including the predominating delta variant. FUNDING: Clover Biopharmaceuticals and the Coalition for Epidemic Preparedness Innovations.

A basic sPLA2 (D49) from the venom of snake Agkistrodon piscivorus leucostoma (AplTX-II) was isolated, purified and characterized. We determined the enzymatic and pharmacological profiles of this toxin. AplTX-II was isolated with a high level of purity through reverse phase chromatography and molecular exclusion. The enzyme showed pI 9.48 and molecular weight of 14,003 Da. The enzymatic activity of the AplTX-II depended on Ca2+ pH and temperature. The comparison of the primary structure with other sPLA2s revealed that AplTX-II presented all the structural reasons expected for a basic sPLA2s. Additionally, we have resolved its structure with the docked synthetic substrate NOBA (4-nitro-3-octanoyloxy benzoic acid) by homology modeling, and performed MD simulations with explicit solvent. Structural similarities were found between the enzyme's modeled structure and other snake sPLA2 X-Ray structures, available in the PDB database. NOBA and active-site water molecules spontaneously adopted stable positions and established interactions in full agreement with the reaction mechanism, proposed for the physiological substrate, suggesting that NOBA hydrolysis is an excellent model to study phospholipid hydrolysis.

El presente artículo surge del proceso de investigación titulado"Identificación de las pruebas objeti-vas y subjetivas utilizadas por fonoaudiólogos para la evaluación y diagnóstico de la disfagia", realizado en Universidad Santiago de Cali. Se propone debatir sobre los encuentros y disonancias en torno a la evaluación fonoaudiológica de la disfagia para lo cual se realizó un estudio cuantitativo, ob-servacional descriptivo transversal, con el uso de encuesta estructurada. La población de estudio correspondió a fonoaudiólogos que laboraban en el área de alimentación, específicamente en la disfagia, en la ciudad de Santiago de Cali (Colombia), entre enero de 2014 y junio de 2015. Los resultados obtenidos permiten visualizar cómo se encuentra la Fonoaudiología frente al hacer profesional en la evaluación de la disfagia, los modelos conceptuales que cimentan la praxis; pero también posibilitan llegar al análisis del quehacer Fonoaudiológico en relación a su experti-cia y formación. Los resultados que se presentan, desde los encuentros y las disonancias, abren el camino a repensar posiciones conceptuales y gremiales en pro de la visibilización de la profesión en el área de la salud


This article arises from the research process called "Identification of the objective and subjective tests used by speech therapists for evaluation and diagnosis of dysphagia" held in Santiago de Cali University. To answer the problem question a quantitative, descriptive observational cross-sectional study was conducted using structured survey. The study population corresponded to speech therapists who work in the food area, specifically in dysphagia, in the city of Santiago de Cali (Colombia) during second half, 2014 - first half, 2015.The results allow to visualize how the Fonoaudiología is facing the professional make the evalua-tion of dysphagia, conceptual models that underpin the practice; but also enable analysis pho-noaudiological reach actions related to their expertise and training. The results presented from encounters and dissonances, open the way to rethink conceptual and union positions towards the visibility of the profession in the world of health

El síndrome SAPHO (sinovitis, acné, pustulosis, hiperostosis y osteítis) comprende un conjunto de manifestaciones cutáneo-osteoarticulares. Se han descrito algunas complicaciones graves que pueden aparecer durante la evolución de la enfermedad, como la trombosis venosa, principalmente en pacientes que desarrollan afectación inflamatoria grave de la pared torácica anterior. El objetivo de la presente revisión fue analizar los casos descritos en la literatura médica relacionados con la presencia de complicaciones trombóticas en pacientes diagnosticados de síndrome SAPHO e intentar establecer los probables factores de riesgo y su posible mecanismo patogénico. Se analizaron 11 artículos publicados de casos clínicos aislados o series de casos, con un total de 144 pacientes, que describen en total 15 casos de trombosis venosa. Se exponen las características clínicas de estos pacientes, se evalúa si cumplen los criterios de clasificación ASAS para espondiloatritis axial y periférica, y se resalta la necesidad de realizar un diagnóstico y tratamiento precoces (AU)

KEY MESSAGE: Diploid strawberry and potato transformed with a transposon tagging construct exhibited either global (strawberry) or local transposition (potato). An activation tagged, compact-sized strawberry mutant overexpressed the gene adjacent to Ds. As major fruit and vegetable crops, respectively, strawberry and potato are among the first horticultural crops with draft genome sequences. To study gene function, we examined transposon-tagged mutant strategies in model populations for both species, Fragaria vesca and Solanum tuberosum Group Phureja, using the same Activation/Dissociation (Ac/Ds) construct. Early somatic transposition during tissue culture occurred at a frequency of 18.5% in strawberry but not in potato transformants. Green fluorescent protein under a monocot promoter was a more reliable selectable marker in strawberry compared to potato. BASTA (gluphosinate herbicide) resistance served as an effective selectable marker for both species (80 and 85% reliable in strawberry and potato, respectively), although the effective concentration differed (0.5% for strawberry and 0.03% for potato). Transposons preferentially reinserted within genes (exons and introns) in both species. Real-time quantitative PCR revealed enhanced gene expression (670 and 298-fold expression compared to wild type in petiole and leaf tissue, respectively) for an activation tagged strawberry mutant with Ds inserted about 0.6 kb upstream from a gene coding for an epidermis-specific secreted glycoprotein EP1. Our data also suggested that endopolyploid (diploid) cells occurring in leaf explants of monoploid potato were the favored targets of T-DNA integration during transformation. Mutants obtained in these studies provide a useful resource for future genetic studies.
