RESUMO

The article deals with the results of treatment of 60 patients with severe pneumonia and acute infectious destructions of the lungs. Hemosorption improved the results of treatment of patients with this pathological condition due to rapid recovery from the state of toxicosis, stimulation of immunity, and improvement of microcirculation. The dynamics of changes of the immunological values are discussed, the indications for conducting hemosorption in patients with severe pneumonia and infectious destructions of the lungs and the criteria of its efficacy are determined.

RESUMO

54 patients with acute pneumonia were examined. There were 36 men and 18 women aged 16 to 72 years. Use was made of the clinico-roentgenological, bronchological and laboratory methods including biochemistry and immunoassays to assess the gravity of the patients' condition. The system protease inhibitors, the concentration of immunoglobulins in blood serum, circulating immune complexes, functional activity of neutrophils and monocytes of peripheral blood were under study. The major part of the patients (45 persons) did not develop destructive lesions, in 9 patients, pneumonia was complicated by suppurative-destructive lesions. In both patients' groups, the disease (in 90.3% of cases) ran its course with the signs of active virus infection. The risk factor for the destruction development was the rise of the laboratory gravity index to over 5-6 marks, a tendency towards the decrease of the phagocytic activity of monocytes and immunoglobulin M, the presence of unbalance in the system protease inhibitors at the expense of inadequate reduction of the antiproteolytic potential.

RESUMO

Based on an analysis of results of treatment of 20 patients with critical pneumonias and acute infectious destructions of the lungs by the method of hemosorption the authors make a conclusion that use of hemosorption promoted a rapid control of state of intoxication, decrease of the level of middle molecule toxins and lowering of the laboratory and clinical estimates of severity. Results of the treatment have confirmed good effects of hemosorption.

RESUMO

An analysis of case histories of 225 patients with bronchiectasis subjected to lung resections of various volume has shown high efficiency of isolated and associated resection of the basal pyramid with an intact apical segment. The apical segment remaining intact gives positive effects on the rearrangement of the architectonics of the bronchial tree preventing a considerable displacement of the remaining bronchi. The postoperative period in the patients who had an additional treatment of the wound surface of the sixth segment was more favorable as compared with the patients with open wounds.

The data are presented on studying the production of antibodies to influenza A (H3N2) virus by lymphoid cells of the amygdaline tonsils and mediastinal lymph nodes in vitro during stimulation of these cells with influenza virus or lipopolysaccharide of typhoid bacteria.

RESUMO

On the basis of the complex clinico-functional examination of 78 patients before and after operations for bronchiectasis the authors consider that performing preserving resections, segmentectomies in particular, still remains the main principle of the surgical treatment of adult patients with bronchiectasis. Complete clinical recovery may be achieved in half of the operated patients. A third of the patients showed an improvement in their condition.

RESUMO

The analysis of data obtained in the investigation of functions of the pulmonary blood flow in 44 patients prior to and following segmentary and combined resections of the lungs fulfilled for bronchiectasis has shown that almost all the patients with bronchiectasis have the disturbed hemodynamics of the small circulation circuit. The direct relation was shown between the duration of the disease, spread of the pathological process and the degree of lung hypertension. It has been found that a resection within the limits of 6 lung segments fails to exert a negative influence on hemodynamics of the small blood circulation circuit and allows prevention of progressing pulmonary hypertension in most of the patients.
