RESUMO

The surgical procedure is the most important in the combined treatment of the liver colorectal metastatic injury. The aim of our investigation was to determine the efficacy of liver resection together with regional chemotherapy(R/C) at solitary injury of liver. 48 patients with metastatic damage of the liver from the colorectal cancer were observed. Different degree of liver resection with R/C was performed. Outcomes of partial resection of the liver metastasis combined with primary tumor resection and 5-FU chemotherapy in the observed cases were quit satisfactory (5 year survival-14,6%).

The aim of the study was the evaluation of significance of immunopathogenic mechanisms in the pathogenesis of minimal change nephrotic syndrome and the study of the ultrastructural changes of peripheral blood formed elements. The observation was carried out on 64 patients with steroid sensitive minimal change nephrotic syndrome. During the manifestation and recurrence of the disease, in the majority of the patients, the results of immunological studies revealed the reduction in both total and absolute count of T-lymphocytes on the background of decreased immunoregulatory index (T(4+)/ T(8+)). In the peripheral blood statistically reliable reduction in the percent value of T(3+) and T(4+) phenotype lymphocytes and elevation of number of T(8+)-lymphocytes were observed. On the background of normal values of B-lymphocytes appeared increase of IgM and decrease of IgG and IgA. In most cases significant elevation in the value of IgE - (244+10,3) and low values of INF-gamma, ultrastructural changes of blood monocytes were apparent. The peculiarities of ultrastructural changes of eosinophils and basophiles especially were pronounced by cytomembrane degranulation in form of decrease in electric density.

Retrospective study was performed to evaluate the efficacy of combined methods of treatment in patients with colorectal cancer with hepatic metastases. 57 patients (I group - control) undergone regional chemotherapy (RC) with hepatic arterial or intraportal 5Fluorouracil (5FU). From 84 patients 48 (II group) undergone liver resection + RC (5FU), 24 (III group) - hepatic arterial embolization +RC (5FU), and 12(IV group) with multiple bilobar (<3 cm) metastases- alcoholization of metastatic nodules +RC (5FU). Overall 5-year survival for the II group was 14.8% without operative or postoperative deaths. The median survival was 19 months (range 13-27 months) and 21 months (range 15-26 months) for II and III groups respectively, which is not different from the survival rate of patients who received only RC. Although, quality of life was much better after combined treatment. Resection of colorectal liver metastases followed by regional chemotherapy can provide long-term survival in selected patients.

RESUMO

Cholestasis inhibits expression of early response genes in rat hepatocytes stimulated to proliferation and restricts the process related to reparative regeneration. The occlusion of general bile duct induces bile acid accumulation, which suppresses 11-beta-hydroxysteroid-dehydrogenase activity. Hence, the concentration of corticosterone is increased, which suppresses the expression of early response genes in rat hepatocytes. The restoration of liver mass occurs due to proliferation of reserved cells. An unusual mitotic peak of these cells is registered 34 h after the operation.

RESUMO

The ultrastructure of peri- and extra-infarction areas was examined in the left-ventricular myocardium of rats after swimming exercise. The ultrastructural study of cardiomyocytes and blood capillaries has suggested that regular exercise under experimental myocardial infarction improves blood supply to the heart muscle, prevents fibrillogenesis and activates compensatory-adaptive response of the intracellular regeneration type within components of the capillary wall and cardiomyocytes.
