RESUMO

Several COVID-19 vaccines, some more efficacious than others, are now available and deployed, including multiple mRNA- and viral vector-based vaccines. With the focus on creating cost-effective solutions that can reach the low- and medium- income world, GreenLight Biosciences has developed an mRNA vaccine candidate, GLB-COV2-043, encoding for the full-length SARS-CoV-2 Wuhan wild-type spike protein. In pre-clinical studies in mice, GLB-COV2-043 induced robust antigen-specific binding and virus-neutralizing antibody responses targeting homologous and heterologous SARS-CoV-2 variants and a TH1-biased immune response. Boosting mice with monovalent or bivalent mRNA-LNPs provided rapid recall and long-lasting neutralizing antibody titers, an increase in antibody avidity and breadth that was held over time and generation of antigen-specific memory B- and T- cells. In hamsters, vaccination with GLB-COV2-043 led to lower viral loads, reduced incidence of SARS-CoV-2-related microscopic findings in lungs, and protection against weight loss after heterologous challenge with Omicron BA.1 live virus. Altogether, these data indicate that GLB-COV2-043 mRNA-LNP vaccine candidate elicits robust protective humoral and cellular immune responses and establishes our mRNA-LNP platform for subsequent clinical evaluations.

Over the past two decades, automatic facial emotion recognition has received enormous attention. This is due to the increase in the need for behavioral biometric systems and human-machine interaction where the facial emotion recognition and the intensity of emotion play vital roles. The existing works usually do not encode the intensity of the observed facial emotion and even less involve modeling the multi-class facial behavior data jointly. Our work involves recognizing the emotion along with the respective intensities of those emotions. The algorithms used in this comparative study are Gabor filters, a Histogram of Oriented Gradients (HOG), and Local Binary Pattern (LBP) for feature extraction. For classification, we have used Support Vector Machine (SVM), Random Forest (RF), and Nearest Neighbor Algorithm (kNN). This attains emotion recognition and intensity estimation of each recognized emotion. This is a comparative study of classifiers used for facial emotion recognition along with the intensity estimation of those emotions for databases. The results verified that the comparative study could be further used in real-time behavioral facial emotion and intensity of emotion recognition.

BACKGROUND: Vitiligo is a disorder of dyspigmentation that can impact quality of life. While narrow-band ultraviolet B (NBUVB) is an effective treatment for vitiligo, a subset of patients are unable to respond to phototherapy as they cannot photoadapt. However, nonsteroidal anti-inflammatory drugs (NSAIDs) have been shown to increase the minimal erythema dose. PURPOSE: To determine whether ibuprofen allows non-photoadapters to respond to therapeutic doses of NBUVB and maintain photoadaptation. METHOD: Four patients unable to tolerate NBUVB or excimer past a dose of 1000 mJ/cm2 were enrolled in the study and given ibuprofen 400 mg prior to phototherapy, which was performed 2-3 times a week. Patients were followed up to 72 treatments to demonstrate photoadaptation and maintenance of response to phototherapy. Patients were clinically monitored by serial photographs approximately every 12 treatments. Response to phototherapy was monitored by tracking the dose of NBUVB received at each session. Maintenance of response was monitored for six treatments after discontinuing the ibuprofen. Percent change in pigmentation was also recorded. RESULTS: Three out of four subjects enrolled in the study were able to increase their doses of phototherapy to a therapeutic range, and subjects continued to photoadapt for six treatments after discontinuing ibuprofen. Two subjects achieved repigmentation during their course of phototherapy. CONCLUSION: Ibuprofen may be a safe alternative to corticosteroids for select patients with vitiligo that are unable to photoadapt. It appears that the ability to photoadapt continues once ibuprofen is discontinued, negating the need for chronic use. Enabling photoadaptation allows patients to achieve therapeutic doses of NBUVB phototherapy, leading to repigmentation and improved outcomes. Extensive possibilities of applications have made emotion recognition ineluctable and challenging in the field of computer science. The use of non-verbal cues such as gestures, body movement, and facial expressions convey the feeling and the feedback to the user. This discipline of Human-Computer Interaction places reliance on the algorithmic robustness and the sensitivity of the sensor to ameliorate the recognition. Sensors play a significant role in accurate detection by providing a very high-quality input, hence increasing the efficiency and the reliability of the system. Automatic recognition of human emotions would help in teaching social intelligence in the machines. This paper presents a brief study of the various approaches and the techniques of emotion recognition. The survey covers a succinct review of the databases that are considered as data sets for algorithms detecting the emotions by facial expressions. Later, mixed reality device Microsoft HoloLens (MHL) is introduced for observing emotion recognition in Augmented Reality (AR). A brief introduction of its sensors, their application in emotion recognition and some preliminary results of emotion recognition using MHL are presented. The paper then concludes by comparing results of emotion recognition by the MHL and a regular webcam.

BACKGROUND: Reconstruction of postsurgical scalp defects can be difficult and time-consuming using a conventional bilayered technique. A specialized closure using a pulley suture can assist in closing wounds under high tension and can decrease time and cost for the surgeon. OBJECTIVE: To determine if closing scalp defects with a single-layered closure using pulley sutures would result in decreased time but equivalent scar cosmesis compared to bilayered closures. MATERIALS AND METHODS: A total of 21 patients with postsurgical scalp defects were randomized to a bilayered or a pulley group, and time was measured for each closure. Scar appearance was assessed using the Patient and Observer Scar Assessment Scale at 2 weeks, 2 months, and 6 months postsurgery. Before and after photographs were also assessed by a blinded dermatologist using the visual analog scale. RESULTS: Compared to a bilayered closure, the pulley technique resulted in significantly reduced closure time (p < .001). Even though patient overall scores at 2 weeks and observer total score at 6 months were superior in the pulley group, the visual analog scale scores were similar between the 2 groups. CONCLUSION: Scalp reconstructions using a single layer of pulley sutures result in time and cost reduction and similar scar appearance compared to bilayered closures.

Psoriasis is an inflammatory skin condition that affects approximately 2 % of people worldwide. Topical treatments, systemic treatments, biologic agents, and phototherapy are all treatment options for psoriasis. Ultraviolet (UV) B phototherapy is most appropriate for patients with >10 % affected body surface area who have not responded to topical treatments. This review outlines the use, dosage, safety, and efficacy of narrowband UVB (NB-UVB) and targeted phototherapy. NB-UVB and excimer laser are effective treatment options for psoriasis; they are administered two to three times weekly until clearance followed by maintenance treatment before discontinuation. Long-term data on NB-UVB indicate that it has a good safety profile. NB-UVB is commonly used with adjunctive topical treatments such as emollients, calcipotriene, cortico-steroids, retinoids, and tar. NB-UVB can be used in selected patients with traditional systemic agents such as methotrexate, mycophenolate mofetil, and cyclosporine, although the duration of the combined treatment should be kept to a minimum and patients need to be closely monitored. Acitretin can be safely used with phototherapy, but robust data on the combination use of biologic agents or phosphodiesterase inhibitors with phototherapy are lacking.

INTRODUCTION: Mohs micrographic surgery (MMS) is successful and cost effective, but may be time consuming as patients are required to wait for final wound repair until margins are clear. We propose for selected cases the "Mohs and Close technique" (MCT), in which the defect is immediately repaired after tumor resection rather than waiting until margins are clear. METHODS: MCT was only performed on tumors that had clearly de ned borders, low risk histology, whose resulting defect after exci- sion required either a primary or partial closure, and whose repair wouldn't change to a different repair option if further stages of exci- sion were necessary. Tumor data was recorded for all cases. Time elapsed from tumor resection to completion of wound closure was recorded with and without performing MCT for comparable wounds. RESULTS: MCT was performed for 456 of 898 cases. Time required without MCT was significantly longer than with MCT when only one stage was performed (P<0.001). There was no statistical difference (P=0.3358) between the two separate techniques for cases which required 2 or more stages. CONCLUSION: MCT significantly reduces the time needed for selected Mohs cases that require only one stage of excision and therefore can increase the efficiency of MMS. BACKGROUND: The prevalence of surgical site infection (SSI) is low with Mohs micrographic surgery (MMS). It has not been determined whether sterile gloves (SG) or nonsterile gloves (NSG) should be used for resection and reconstruction during MMS. OBJECTIVE: To compare the SSI rate with the use of SG and NSG for MMS, including reconstruction, and to determine whether SG help prevent SSI. MATERIALS & METHODS: Data were collected and recorded for Mohs cases in which SG or NSG were used. Infected cases and SSI rate for SG and NSG were also recorded. Chi-square analysis was performed to compare SSI. RESULTS: There were 1,004 tumors in 942 patients in the SG group and 1,021 tumors in 941 patients in the NSG group. The prevalence of infection was 0.50% in the SG group and 0.49% in the NSG group (p = .82). The cost of gloves was $5.66 for one SG case and $1.63 for one NSG case. CONCLUSION: The prevalence of infection with SG and NSG was almost identical. The cost to use SG was 3.5 times as great as for NSG. The use of NSG for MMS and reconstruction is safe and cost effective.
