RESUMO

Background: Pembrolizumab is a guideline-recommended, both first- and second-line treatment option for microsatellite-instability-high (MSI-H)/mismatch repair-deficient (dMMR)advanced colorectal cancer patients. The aim of the present study is to investigates the health and economic outcomes of three treatment strategies with or without pembrolizumab in MSI-H/dMMR advanced colorectal cancer to define the best treatment strategy from the perspective of the US payer. Methods: A microsimulation model was developed to estimate the cost and effectiveness of three treatment strategies: 1) pembrolizumab used as first-line, 2) pembrolizumab used as second-line and, 3) chemotherapy. Life years (LYs), quality-adjusted LYs (QALYs) and lifetime costs were estimated. Results: The model projected that patients receiving pembrolizumab in the first-line setting gained 5.579 QALYs; this value was 1.501 and 3.941 QALYs more than that for patients receiving pembrolizumab in the second-line setting and chemotherapy, respectively. First-line pembrolizumab strategy dominated second-line pembrolizumab strategy. Compared with chemotherapy, first-line pembrolizumab strategy yielded an incremental cost of $50613.7, which resulted in an ICER of $13441 per QALY. Conclusion: For patients with MSI-H/dMMR advanced colorectal cancer, reserving pembrolizumab for second-line line use is dominated by its first-line use, and first-line use of pembrolizumab is cost-effective compared with chemotherapy.

RESUMO

Human epidermal growth factor (hEGF) is a typical member of the growth factor family that activates epidermal growth factor receptors. It is synthesized and secreted by multiple tissues and organs of the human body, regulating the cell proliferation, differentiation and migration via binding to receptors and activating a series of signaling pathways. In recent years, the research on hEGF has been extended to its role in human physiology and pathology, especially in tissue regeneration and wound healing. This paper reviews the research progress of hEGF, briefly describes its gene and protein structure and characteristics, mechanisms and biological effects, with the emphasis on the roles and influences in the healing of gastrointestinal ulcers, skin wound repair and tumor pathology.

RESUMO

OBJECTIVE: To facilitate the practice of the activity of "hospital administration year" and to improve the medical service quality. METHODS: Reformation was made actively by optimizing the administrative organization structure and introducing the model of secondary hospital service in our hospital after the trusteeship by the Shanghai 6th People's Hospital. RESULTS & CONCLUSIONS: Due to the above practice, the consciousness on medication safety, the quality of medical care services as well as the degree of patients' satisfaction had been increased gradually, which contributed to a sound, ordered and sustainable development of our hospital.

RESUMO

AIM: To investigate the effect of tea polyphenols (TP) on alcohol-induced liver injury in mice.METHODS: 40% alcohol(0 5 mL) and TP (5 mg) were administered intragastrically, the liver MDA level and serum alanine transaminase (ALT) activity were determined. The effect of TP on MDA level in alcohol-treated liver in vitro was also examined.RESULTS: Pretreatment with TP significantly inhibited alcohol-induced liver MDA increase in mice in vivo and in vitro , the increase in serum ALT induced by alcohol was also reduced by pretreatment with TP (0 5 mg). TP at a dose of 5 mg, administered 1 h after alcohol treatment, also suppressed increase in liver MDA level stimulated by alcohol. CONCLUSION: These results demonstrated that TP has a protective effect on alcohol-induced liver injury in mice.
