RESUMO

The experience in diagnosis and treatment of 469 patients with gastroduodenal ulcer bleedings is presented. Sensitivity of endoscopy in detection of bleeding source was 98.8%, in detection of ulcer size -- 93.4%, specificity -- 80.6%. It is demonstrated that the most frequently recurrence of bleeding occurs when ulcer size is more than 1 cm. Location of ulcer on posterior, posterior-superior and posterior-inferior walls of the duodenal bulb is an unfavorable prognostic symptom (rebleeding occurs in 17.1, 30.0 and 12.5% cases, respectively). Types Forrest-Ia and Forrest-IIa are especially dangerous for recurrence (21.4 and 15.6%, respectively). In types Forrest-Ib and Forrest-IIb rebleeding occurs less frequently (4 and 6%, respectively). In types Forrest-Ia and Forrest-Ib rebleeding occurs mainly on the first day, rebleeding in types Forrest-IIa occurs more often on the first-second day, however it is possible up to day 7. Rebleeding in types Forrest-IIb occurs also mainly during the first two days. Endoscopic hemostasis (electrocoagulation and injection therapy) was performed in 40 patients. In 6 (15%) cases hemostasis was not achieved. In 8 (20%) cases endoscopic hemostasis led to a temporary effect, i.e. rebleeding was seen within 24-72 hours after endoscopic procedure. Stable hemostasis with endoscopy was achieved in 26 (65%) patients.

RESUMO

The investigation is based on an analysis of results of treatment of 102 patients after an emergency resection of the stomach for complicated gastroduodenal ulcers. The probe was inserted into the jejunum intraoperatively. The period of enteral probe nutrition (EPN) was 5-7 days. At the early postoperative period in patients who had undergone the emergency resection of the stomach EPN is an effective method of nutrition maintenance as compared with the infusion-transfusion therapy. The method is simple and open to general use, and is economically advantageous under conditions of any hospital.

RESUMO

The results of examination of 167 patients suffering from gastric and duodenal ulcers complicated by bleeding and perforation in a rehabilitation period are presented. High contamination by HP bacteria was established in gastroduodenal bleeding especially in duodenal ulcer (94.45%), as well as direct correlation between the contamination range and blood loss degree. The presence of HP infection during a control check-up examination suggests that the course of anti-ulcer therapy should last not less than 4-6 weeks. In treating patients suffering from ulcer bleeding conservative therapy is preferable. Stomach resection is considered as a method of choice. Lethality makes up 0.94 and 6.25%, respectively.

RESUMO

Two types of neurons are distinguished proceeding from their morphology and electrophysiological characteristics during early days of development in culture. The "mature" cells prove to be motoneurons and die by the 5th-7th days in vitro. They possess a sodium and two-component potassium currents. Neuroblasts represent another type of cells. They are not able to generate the action potential and potassium current consists only of one component. These cells are differentiated during cultivation.

RESUMO

Development of spontaneous synaptic activity was investigated during first 2.5 weeks in culture by means of the "hole-cell" version of the patch-clamp technique. Glycine-mediated giant postsynaptic currents (PSCs) were the most prominent property of the synaptic activity pattern. Excitatory PSCs were not observed until 14 days in culture. The role of culture conditions for the maturation of spontaneous activity has been shown. Possible mechanisms of the burst activity generation are discussed.

RESUMO

Glycine-activated whole-cell currents of the chick spinal cord neurons have been investigated in a chicken embryo during different periods in culture. Shifts in the concentration dependence curve of mature and immature cells are shown. Mature cells prove to be motoneurons and die 5-7 days later in vitro. immature cells are neuroblasts differentiating in culture. Glycine sensitivity of low-differentiating cells increased more than 10 times by the end of the 14th day in culture. Desensitization kinetics of the glycine-activated currents decelerated with age. A crucial role of spontaneous electric activity was shown for maturation of the agonist-activated conductance.

RESUMO

Spontaneous glycine-mediated synaptic currents in culture of the spinal cord cells of chicks were analyzed by fitting exponential function to their decay. Some PSCs were of small amplitude. Their decay was single-exponential. Time constant of the decay increased with depolarization and decreased with a rise of temperature. The decay of the giant PSCs was double-exponential and had a weak potential dependence. Only slow component of the decay was sensitive to changes of the temperature. Time constant of the slow component increased in the presence of strychnine. The removal of the agonist from receptors was considered as an explanation for the variability and double-exponential character of the decays.

RESUMO

The method of dietetic interview was used in a strictly standardized epidemiological investigation of a random sample of males aged from 30 to 59 years (n-2537) in Kiev. A dependence has been revealed between the use of alcohol and disorders in the character of nutrition unfavourable in respect to coronary heart disease. A relationship has been established between the alcohol use and the incidence rate of such risk factors of coronary heart disease as hyperlipoproteinemia, excessive body mass, smoking, and the incidence of myocardial infarction. No relationship was recorded between alcohol kind and dose and the incidence of risk factors of coronary heart disease.
