RESUMO

Acute fatty liver disease of pregnancy AFLP is an obstetrical emergency, with severe complications that may include death. Management of AFLP is challenging and include plasma exchange which helps to improve the prognosis for both mother and fetus and delay liver transplantation.

INTRODUCTION: inferior vena cava IVC injury is rare with lethal outcomes, the clinical signs depends on the location and associated injuries, andt he treatment might be endovascular, surgical. CLINICAL CASE: A 25 years with no medical history was admitted to the emergency department after a car accident. After intubation and hemodynamic stabilization, the computerized tomography CT scan showed hepatic laceration with a rupture of the IVC in the retro-hepatic portion, he was admitted to the operation room for damage control laparotomy; the patient died 12 h after the operation despite appropriate management. CONCLUSION: IVC are rare and lethal, the CT scan remains the gold standard and the evolution of endovascular techniques decreased the mortality rate.

INTRODUCTION: Hydatid cyst is an infectious disease caused by « ECHINOCOCCUS GRANULOSIS ¼, it can be asymptomatic or manifests depending on size, location, and relation with other organs, or by complication like rupture. This latter might occur spontaneously or post-traumatic, and it might manifest with an anaphylactic shock, a life-threatening situation. IMPORTANCE: Anaphylactic shock is a rare cause of traumatic shock state. CASE PRESENTATION: A 30 years old man with no medical history was admitted to the emergency room after a car accident, on his admission, his Glasgow coma scale was 10/15 with a blood pressure of 80/30 mmHg, he was intubated and stabilized hemodynamically. The full-body CT scan showed no abnormalities except on the thoracic level where it showed a well-limited rounded formation with regular contours containing hydro-aeric level related to ruptured hydatid cyst. After ruling out the diagnosis of hemorrhagic, hypovolemic shock, the diagnosis of anaphylactic shock due to a post-traumatic rupture of the hydatid cyst was maintained. CONCLUSION: Post-traumatic rupture of the hydatid cyst is a rare emergency that requires early diagnosis and management. Surgery remains the principal treatment of ruptured hydatid cyst. Anaphylactic shock is a life-threatening situation with various symptoms; epinephrine is the cornerstone for management of this type of shock.

RESUMO

Pneumopericardium is a rare complication of a blunt thoracic trauma. It is defined as the presence of air in the pericardial sac. There are just a few cases described in the literature. This article brings pneumopericardium to light, reinforcing the importance of considering it within the blunt chest trauma and remarking its management with a careful monitoring for the patients whose stable or even asymptomatic with spontaneous ventilation because of the risk of tension pneumopericardium and cardiac arrest. Diagnosis is often difficult, and it can be life-threatening by the occurrence of gas tamponade. We report the case of a 48 years old patient victim of a severe traumatism with pneumothorax and pneumopericardium; he was stable with spontaneous ventilation.

RESUMO

Fahr syndrome (FS) is defined by the presence of bilateral and symmetrical intracerebral nonarteriosclerotic calcifications, localized in the central gray nuclei. We report the case of a patient who presented a postoperative delay in awakening and whose biological and radiological investigations revealed a FS.

RESUMO

Parathyroid carcinoma is a very rare malignant tumor of the parathyroid gland. This cancer poses a great diagnostic and therapeutic difficulty due to its rarity and the absence of a characteristic clinical and paraclinical picture. The diagnosis is histological but is not always easy. Surgery remains the only curative treatment, and cervical radiotherapy can be discussed. Good prognostic factors are complete monobloc tumor resection, and bad prognostic factors are the presence of lymph node metastases at diagnosis, distant metastases, and nonsecreting carcinomas.

RESUMO

A renogastric fistula is a rare complication defined by abnormal communication between the kidney and the stomach. We report the case of a patient admitted to the intensive care unit for hemorrhagic shock following upper gastrointestinal bleeding whose radiological investigations revealed a fistulated renal hematoma in the stomach.

Osler-Weber-Rendu disease (OWRD), called hereditary hemorrhagic telangiectasia, is an uncommon genetic illness with the dominant autosomal transmission. It cannot be easily or quickly diagnosed because of both its infrequency and its various associated symptoms. As far as its symptoms are concerned, the patient experiences recurring epistaxis, mucocutaneous telangiectasia, and arteriovenous malformations that can lead to severe undesirable symptoms. In our case, we report a 32-year-old female that was diagnosed with postpartum preeclampsia and whose paraclinical examinations showed that she suffers from hereditary hemorrhagic telangiectasia disease. Management of OWRD includes systematic diagnosis of visceral arteriovenous malformations (AVMs) in regular intervals, measures to prevent complications, and symptomatic treatment.

RESUMO

Guillain-Barré syndrome (GBS) is an acute peripheral neuropathy that manifests with ascending symmetrical and progressive weakness or paralysis with the absence of reflexes. The incidence of this syndrome in pregnancy and postpartum periods is similar to the general population; however, few published clinical cases have been described. Here, we report the case of a 26-year-old patient who developed GBS with respiratory distress two weeks after her baby's delivery that required long-term mechanical ventilation. The patient's clinical conditions were significantly improved after the use of immunoglobulins and rehabilitation, and she was discharged. Physicians should be aware of the possibility of developing GBS during pregnancy and postpartum periods.
