RESUMO

The article deals with the comprehensive expertise of quality of diagnostics of bronchial asthma in ambulatory polyclinic institutions of pediatric profile. The following techniques were applied: the retrospective analysis of all cases of bronchial asthma among children of City of Yugorsk of Hanti-Mansiisky autonomous okrug (n = 92), expertise of the causes of late diagnostics and possible ways of optimization of earlier diagnostics of bronchial asthma in ambulatory polyclinic institution (20 experts), testing the pediatricians working in children polyclinics (n = 95) on the knowledge of dealing with bronchial asthma. The late diagnostics is ascertained in 95% of cases and average period of diagnostics delay consisted 3 days. The leading role of inadequate theoretical background of pediatricians as a "triggering device" in progressive development of disease is established. The necessity to develop various forms of ongoing training of physicians of primary care for optimizing modern diagnostics of bronchial asthma in polyclinic conditions is justified.

RESUMO

The analysis of health indicators related to children population revealed that the corresponding lower morbidity and higher mortality in rural areas as compared with urban areas is regitered due to underregistration of the nonsevere forms of pathology in children population dwelling outside the zone of the rural district pediatric services. The mentioned indicators correlated with such social and medical organizational characteristics of rural regions limiting the availability of the high-quality medical care as the poor road and transport communication with the district town, the remoteness from the oblast town, the paediatrician's provision and the level of their professional qualification, the percent of children from socially ill-being families and the density of population. The established patterns demonstrate that to enhance the quality of medical care of children living in the rural areas it is necessary to resolve professional, social and medical organizational problems simultaneously. Thereupon, appears the need to elaborate a well-grounded differentiated approach to health resources planning with an imperative taking into account the characteristics of the rural regions.

For the 1970-1997 yrs. period 1357 patients were operated on for an ulcer disease using vagotomy, in 361 (26%) simultant operations were carried out, including in 340--for the abdominal cavity organs diseases and in 21--for intraoperative complications.

RESUMO

The diagnostic potentialities of tetrapolar chest rheography were examined in children with heart pathology, undergoing bicycle ergometry tests of different power. The method applied to 25 patients with organic heart lesions enabled latent heart failure to be diagnosed in 8 of them. The method can be used for the estimation of the treatment and rehabilitation measures with special reference to the follow-up of 119 children suffering from myocardiodystrophy.
