RESUMO

Most optimal methods of plasty with the maximum use of intact skin of a healthy leaf and adjacent area as a trapezoid cellulo-cutaneous flap are proposed. Different variants of the operation are proposed as one-, two-, three- and many-flaps plasty with local tissues and one- and two-flaps combined plasty. The choice is dependent on the localization and degree of contracture. Plasty by the methods proposed allows to adequately eliminate marginal contractures of large vessels, syndactylia, microstomia and narrowing of the lid slit.

RESUMO

Based on their experience with the treatment of 93 patients with postburn adduction contracture of the shoulder the authors propose a classification related with localization of scars. Marginal contractures are successfully treated by plasty with a trapezoid skin-fat flap from the axillary cavity with/without a combination with the displacement of scarry tissues or free skin plasty; the medial contracture--by one or several pairs of trapezoid skin-fat flaps cut from leaves on the sides of the scar fold; the total contracture with the absence of intact skin in adjacent fields can be adequately eliminated by mobilization a quadrangular island in projection of the dropped axillary cavity to the normal level of the cupola, fixation of narrow sides to the wound margins on the anterior and posterior surface of the humeral articulation and closure of wounds on the shoulder and chest with the splitted skin; the anterior-posterior contracture is eliminated either in the same way as the marginal one, or by cutting a quadrangular flap in the center of the axillary cavity as in the total contracture.

RESUMO

Operations were performed on 10 patients with persistent ulcers of the scars in the zone of Achilles tendon. Plasty with L-shaped whole thickness flap raised from the area of the external surface of the foot, ankle and lower third of the shin with a proximal pedicle was fulfilled in 9 patients after a radical dissection of ulcerated scars. In one patient plasty was fulfilled by translocation of a two-pedicle flap. Different variants of operations and results of treatment are described. A conclusion is made that scarry-trophic ulcers in the zone of Achilles tendon can be successfully treated by plasty with a whole thickness flap raised in the area of the lower third of the shin, external ankle and the lateral surface of the foot.
