RESUMO

Chronic dacryocystitis is due to an obstruction in the nasolacrimal duct, with subsequent infection of the lacrimal sac. The goal of surgery is to reestablish intranasal drainage of the lacrimal sac. Classic dacryocystorhinostomy (DCR) requires an external incision and drilling through the lacrimal bone into the middle meatus. In our study a 600-micron neodymium:YAG (Nd:YAG) fiber with a blunt hemispherical tip is inserted via the lacrimal puncta. An intranasal ostium is created with the laser under intranasal endoscopic control. Silicon tubes are then left in place for 6 months. We have performed 49 procedures over the past 2 1/2 years, with a success rate of 85% after one surgical procedure, which is commensurate with standard DCR. This procedure provides a simple, bloodless, incisionless alternative to standard DCR.

RESUMO

Capsulotomy using a cystotome and continuous tear capsulorhexis has dramatically improved the quality of modern cataract surgery. The authors have devised a microscalpel that has a semicircular configuration and can turn 360 degrees and score or cut the anterior capsule. The blade also has the ability to salvage a radial tear that may occur during capsulorhexis by cutting the extension from the opposite direction and continuing the "peeling off" of the capsule.

RESUMO

We analyzed human tears by ultraviolet (UV) spectrophotometry for transmittance of UV light. Ultraviolet light was readily transmitted through human tears in the UV-A (320-400 nm) and UV-B (290-320 nm) ranges. Tears partially absorbed UV light in the UV-C (100-290 nm) range. The clinical implications of these findings include prevention of phototoxic ocular damage and effects upon excimer laser corneal photoablative techniques.

RESUMO

The clinical, light microscopic, ultrastructural, and immunohistologic findings of a neurilemmoma encroaching upon the nasolacrimal duct are presented. This is the first reported case of this rare tumour of the nasolacrimal duct where the diagnosis has been confirmed by electron microscopy and immunohistochemical techniques.
