RESUMO

During ergometric examination of 92 patients with ischemic heart disease and 35 healthy individuals blood coagulation was studied by recording thromboelastograms and electrocoagulograms and determining blood platelet aggregation, fibrinolytic activity and levels of fibrinogen and free heparin in blood. In 65 patients these values in ergometry were determined after treatment with obsidan. It was shown that during physical load there is an intensification of hypercoagulation processes embracing all phases of blood coagulation with simultaneous activation of anticoagulation mechanisms; in contrast to healthy individuals patients with ischemic heart disease are marked by prevalent hypercoagulation. Shifts in the system of blood coagulation depend on the volume of physical load. Heparin and fibrinolytic activity increases up to a certain load level, but when this is surpassed hypercoagulation processes prevail. This may be used as an additional criterion in determining the level of the training regimen in physical rehabilitation of patients.

RESUMO

The correlational connection between some factors of risk and ergometric indices in patients with ischemic heart disease was studied. The results of an examination covering 188 patients and 56 practically healthy persons which involved the use of a graded stepwise increasing loading on a veloergometer under a monitored control of the pulse rate, ECG and arterial pressure were analyzed. To the risk factors were referred hypercholesterinemia, hypertension, smoking, obesity and hypodynamia. With a rising number of risk factors a significant and progressive decline of the patients' physical performance capacity was ascertained. The most essential correlational connection between the risk factors and the maximally tolerable load and chronotropic and aerobic reserves was educed.

An evaluation of some standardized and relative indices of ergometry is presented: heart rate and oxygen consumption as percentage of individual values, indices of chrono-, isotropic, aerobic and coronary reserves. The investigation is based on the study of 113 normal individuals, 363 hypertonic patients, and 100 patients with ischaemic heart disease. In all cases the maximum tolerable workload was tested by way of its step-wise increase, conducted under pulse, arterial pressure, ECG and oxygen consumption control. Some patients were examined after Obsidan and Dopegit therapy. The use of the relative values of the heart rate and oxygen consumption was shown to neglect the role of individual factors (sex, age, body weight). The determination of the relative augmentation of these indices, as well as of the arterial pressure permits to precise the state of functional reserves of the cardiopulmonary system, and can be used for the diagnosis and evaluation of therapeutic results.
