RESUMO

Regulatory changes proposed by the US Department of Agriculture in 1994 promised to bring progressive changes to school meals. However, lobbying by interest groups resulted in substantial changes to the final rule. This analysis retrospectively examines the federal school meals policy-making process during 1992 to 1996. Key questions address why the policy changed and what the role of interest groups was in affecting the shape, pace, and direction of the policy. The study provides suggestions for using the experiences of 1992 to 1996 to guide future advocacy efforts and for adapting the approach for application to other food and nutrition policies.

In the last five years, home health agencies have become increasingly interested in telemedicine as a potential means to meet the future healthcare needs of their aged and chronically ill clientele. This case study examines the organizational and environmental conditions that affected the implementation of a telemedicine program in one rural home healthcare organization. Several factors restricted the utilization of telemedicine, including Medicare's Prospective Payment System and corresponding documentation (Outcome Assessment and Information Set), the organization controlling grant funding for the program, and several environmental factors. Findings suggest that in rural communities, older homecare patients may have less opportunity to benefit from telemedicine. The study demonstrates the importance of environmental and organizational factors when implementing a telemedicine program. Recommendations are offered for home healthcare organizations considering development of telemedicine programs.

This publication describes the experiences of those Member States of the WHO European Region that were among the first to develop food and nutrition policies as part of the regional strategy for health for all. It presents a vast array of experience, insights and points of view on current issues in nutrition policy-making across Europe. These problems, activities and hopes will occupy decision-makers and the public alike, as they take part in the process of defining the new Europe of the next century. The book addresses a wide audience: policy-makers and leaders in industry, agriculture, health, education, the environment, consumer affairs and the media, and indeed anyone who may be interested in food and nutrition. Its purpose is to acquaint readers with the experiences and lessons to be learned from other countries, and to encourage thoughtful discussion about what the rapidly changing context of Europe means for the nutrition and health of all Europeans, whose well-being is now more than ever interdependent.
