RESUMEN Introducción. En Venezuela existen pocos reportes que describan las bases genéticas del potencial patogénico y filogenético de las cepas de Escherichia coli provenientes de hospitales. Objetivo. Determinar la diversidad genética de cepas extraintestinales de E. coli productoras de las betalactamasas TEM, SHV y CTX-M asociadas a la atención de salud. Materiales y métodos. Se estudió una colección de 12 cepas extraintestinales de E. coli con sensibilidad disminuida a las cefalosporinas de amplio espectro. La sensibilidad antimicrobiana se determinó por concentración inhibitoria mínima. La detección de los grupos filogenéticos, de los factores de virulencia y de los genes que codifican la resistencia antimicrobiana se hizo mediante la técnica de reacción en cadena de la polimerasa y la relación clonal se estableció mediante reacción en cadena de la polimerasa de elementos palindrómicos extragénicos repetitivos (Repetitive Element Palindromic-PCR, rep-PCR). Resultados. Todas las cepas analizadas presentaron resistencia a las cefalosporinas, y resistencia conjunta a quinolonas y aminoglucósidos. La distribución filogenética evidenció que los grupos A y B1 fueron los más frecuentes, seguidos por D y B2; en este último, se detectaron todos los factores de virulencia evaluados, y el gen más frecuente fue el fimH. En todas las cepas analizadas, se encontró bla CTX-M, con predominio de las bla CTX-M-8, y en dos de estas cepas se evidenció la presencia simultánea de bla CTX-M-9, variantes bla CTX-M-65 y bla CTX-M-147. Conclusión. Las cepas estudiadas demostraron diversidad genética y albergaron diferentes genes de virulencia y betalactamasas de espectro extendido (BLEE) sin predominio de ningún filogrupo en particular. Este estudio constituye el primer reporte de la variante bla CTX-M-65 en Venezuela y de la variante bla CTX-M-147 en el mundo, en cepas no relacionadas genéticamente aisladas de hospitales, situación que merece atención y la racionalización del uso de los antimicrobianos.

This is the first report of bla CTX-M alleles in Venezuela and in the world associated to non-genetically related strains isolated in health care units, a situation that deserves attention, as well as the rationalization of antimicrobials use.

In this study, the distribution of phylogenetic groups and the genetic detection of virulence factors in CTX-M-15 ß-lactamase-producing uropathogenic Escherichia coli (UPEC) strains were analyzed. Twenty eight strains were isolated between January 2009 and July 2011 from patients with urinary tract infection (UTI) who attended the Public Health Laboratory at Mérida, Venezuela. Determination of phylogenetic groups and detection of six virulence genes, fimH, fyuA, kpsMTII, usp, PAI and papAH, were performed by PCR amplification. Fifteen of the 28 isolates were mainly located in the phylogenetic group A, followed by B2 (12/28) and D (1/28). No direct relationship between the severity or recurrence of UTI and the distribution of phylogroups was observed. All studied virulence factors were found in group B2 strains with the highest frequency. The prevalent virulence profile included the combination of three main genes: fimH, kpsMTII and fyuA and, to a lesser extent, the presence of other determinants such as usp, PAI and/or papAH. These results indicate that virulent UPEC incorporated three important properties: adhesion, iron uptake and evasion of phagocytosis, which favored the production of recurrent UTI. This is the first report describing the association of phylogenetic groups with the potential virulence of CTX-M-15 ß-lactamase producing UPEC strains in Venezuela.

En este estudio se determinó el perfil de distribución de grupos filogéneticos y la detección genética de factores de virulencia en cepas de Escherichia coli uropatógena (ECUP) productoras de ß-lactamasa CTX-M-15. Veintiocho cepas fueron aisladas de pacientes con infección del tracto urinario (ITU) que asistieron al Laboratorio de Salud Pública del estado Mérida, Venezuela, durante el lapso comprendido entre enero 2009 y julio 2011. La determinación de los grupos filogenéticos y la detección de seis genes de virulencia, fimH, fyuA, kpsMTII, usp, PAI y papAH, se realizó mediante amplificación por PCR. Quince cepas de 28 se ubicaron principalmente en el filogrupo A, seguidos por el B2 (12/28) y D (1/28). No se observó una relación directa entre la recurrencia o gravedad de la ITU y la distribución de los filogrupos. Todos los factores de virulencia estudiados se encontraron con la frecuencia más alta en el grupo B2. El perfil de virulencia prevalente estuvo conformado por la asociación de tres genes principales: fimH, fyuA y kpsMTII y en menor frecuencia, por la presencia de otros determinantes como usp, PAI y/o papAH. Estos resultados indican que la mayoría de ECUP estuvieron dotadas de tres propiedades virulentas importantes: adhesión, captación de hierro y evasión de la fagocitosis, las cuales favorecieron la producción de ITU recurrentes. Este es el primer trabajo que describe la asociación de grupos filogenéticos con el potencial de virulencia de cepas de ECUP productoras de ß-lactamasa CTX-M-15 en Venezuela.

Objetivo: Determinar los grupos filogenéticos y la diversidad clonal de cepas de Escherichia coli aisladas de productos lácteos artesanales elaborados y comercializados en Mérida, Venezuela. Materiales y métodos: Se analizaron 45 cepas de E. coli provenientes de productos lácteos artesanales (crema de leche, cuajada y requesón). Estas cepas se aislaron según la norma de la Comisión Venezolana de Normas Industriales (COVENIN) y la identificación se llevó a cabo por metodologías convencionales. La susceptibilidad antimicrobiana se determinó mediante la técnica de difusión del disco, y los fenotipos con susceptibilidad intermedia o resistente fueron confirmados por concentración inhibitoria mínima (CIM). El grupo filogenético se determinó mediante amplificación por PCR de los genes chuA, yjaA y del fragmento TspE4.C2 y la tipificación de las cepas por Rep-PCR. Resultados: El 73,3% (33/45) de las cepas fueron susceptibles a todos los antibióticos probados y el 24,4% (11/45) presentaron resistencia a la ampicilina. La mayoría de las cepas resistentes fueron aisladas de requesón. El 82,2% de las cepas fueron ubicadas en el filogrupo A y el 8,9% en los grupos B1 y D, respectivamente. El filogrupo B2 no fue detectado. La tipificación por Rep-PCR de las cepas demostró una estructura poblacional heterogénea. Conclusión: La caracterización molecular de estas cepas demostró que no están relacionadas clonalmente y en su mayoría se distribuyeron entre los filogrupos correspondientes a cepas comensales de baja patogenicidad. La detección de E. coli en alimentos lácteos indica una contaminación de origen fecal, que eventualmente podría estar asociada con la presencia de otros enteropatógenos.


Objective: To determine phylogenetic groups and clonal diversity of Escherichia coli strains isolated from several homemade dairy foods produced and marketed in Mérida, Venezuela. Materials and methods: Forty-five E. coli strains isolated from homemade dairy products (cream cheese, curd and cottage cheese) were analyzed. These strains were isolated according to procedures established by the Venezuelan Commission of Industrial Norms (COVENIN) and identification was carried out using conventional methods. Antimicrobial susceptibility was determined by the disk diffusion method and phenotypes with intermediate or resistant susceptibility were confirmed by minimum inhibitory concentration (MIC). Phylogenetic groups were identified by PCR amplification of chuA , yjaA genes and the TspE4.c2 DNA fragment, while molecular typing was carried out by Rep-PCR. Results: Of the 45 isolates, 33 (73.3%) were susceptible to all antibiotics tested while 11 (24.4%) were ampicillin-resistant. The phylogenetic group A was the most common (82.2%), followed by B1 and D (8.9%, respectively). The phylogroup B2 was not detected and Rep-PCR typing of E. coli strains showed a heterogeneous population structure. Conclusion: The molecular characterization of E. coli strains showed that they were clonally nonrelated and mostly distributed among phylogenetic groups that belong to low pathogenicity commensal strains. Detection of E. coli strains in dairy foods indicates a contamination of fecal origin, which could be associated with the presence of other enteric pathogens.

In this study we determined the prevalence of extended-spectrum beta-lactamases (ESBLs) in phylogenetic groups of uropathogenic E. coli (UPEC) isolated from patients in the community. Twenty one UPEC strains with reduced susceptibility to broad-spectrum cephalosporins were collected between January 2009 and July 2010, from patients with urinary tract infection who attended the Public Health Laboratory in Mérida, Venezuela. Genotypic characterization determined that all UPEC strains harbored blaBLEEs genes: 76.2% of the strains showed the presence of a single ESBL-producer gene, represented by blaCTX-M-15, whereas 23.8% of UPEC showed various combinations of bla genes (blacCTX-M-15 + blaTEM-1, blaCTX-M-15 + blaSHV and blaSHV + blaTEM-1). In this study, 61.9% of the isolates were placed in phylogroup A and the remaining strains were assigned to group B2 (38.1%). There was no evidence of spread of a particular UPEC clone; only seven strains belonged to a clonal group with an index of similarity greater than 85%. To our knowledge, this is the first description of blxCTX-M-15 in UPEC from patients with community-acquired urinary tract infections, which shows that Venezuela is also part of the so-called CTX-M-15 pandemic. The findings in this study, as well as its clinical and epidemiological implications, lead to the need for monitoring and controlling the spread of CTX-M-15 producing UPECs, not only regionally, but also nationwide.

En este estudio se determinó la prevalencia de b-lactamasas de espectro extenso (BLEEs) en grupos filogenéticos de E. coli uropatógena (ECUP) aislados en pacientes de la comunidad. Durante Enero 2009 a Julio 2010, se coleccionaron 21 cepas de ECUP, con susceptibilidad disminuida a las cefalosporinas de amplio espectro, provenientes de pacientes que asistieron al Laboratorio de Salud Pública del estado Mérida, Venezuela con diagnóstico de infección del tracto urinario (ITU). La caracterización genotípica determinó que todas las cepas ECUP albergaban genes blaBLEEs. En el 76,2% de las cepas se observó la presencia de un único gen productor de BLEE, representado por blaCTX-M-15, mientras que el 23,8% estuvo conformado por ECUP con diversas combinaciones de genes bla (blaCTX-M-15 + blaTEM-1, blaCTX-M-15 + blaSHV y blaSHV + blaTEM-1). El 61,9% de los aislados se ubicó en el filogrupo A y el resto de las cepas en el grupo B2 (38,1%). No se evidenció la diseminación de una clona de ECUP particular, solo 7 cepas demostraron pertenecer a un grupo clonal con un índice de similitud de más de 85%. De acuerdo a nuestro conocimiento, esta es la primera descripción de blaCTX-M-15 en ECUP causantes de ITU en pacientes de la comunidad, lo que evidencia que Venezuela también forma parte de la llamada pandemia CTX-M-15. Los hallazgos obtenidos en este estudio y las implicaciones clínicas y epidemiológicas que de ello derivan, conllevan a la necesidad de controlar y vigilar la diseminación de ECUP productora de CTX-M-15 no sólo en el ámbito regional sino también nacional.

In this study, the distribution of phylogenetic groups and the genetic detection of virulence factors in CTX-M-15 ß-lactamase-producing uropathogenic Escherichia coli (UPEC) strains were analyzed. Twenty eight strains were isolated between January 2009 and July 2011 from patients with urinary tract infection (UTI) who attended the Public Health Laboratory at Mérida, Venezuela. Determination of phylogenetic groups and detection of six virulence genes, fimH, fyuA, kpsMTII, usp, PAI and papAH, were performed by PCR amplification. Fifteen of the 28 isolates were mainly located in the phylogenetic group A, followed by B2 (12/28) and D (1/28). No direct relationship between the severity or recurrence of UTI and the distribution of phylogroups was observed. All studied virulence factors were found in group B2 strains with the highest frequency. The prevalent virulence profile included the combination of three main genes: fimH, kpsMTII and fyuA and, to a lesser extent, the presence of other determinants such as usp, PAI and/or papAH. These results indicate that virulent UPEC incorporated three important properties: adhesion, iron uptake and evasion of phagocytosis, which favored the production of recurrent UTI. This is the first report describing the association of phylogenetic groups with the potential virulence of CTX-M-15 ß-lactamase producing UPEC strains in Venezuela.

Introducción. Las secuencias de inserción tales como IS CR1 promueven la captura, transposición y expresión de los genes bla CTX-M, facilitando, de esta manera, su diseminación rápida en la población bacteriana. Objetivo. Se determinó la presencia del elemento IS CR1 y su asociación con genes bla CTX-M-1 y bla CTX-M-2 en plásmidos de diferentes grupos de incompatibilidad en Klebsiella pneumoniae de origen hospitalario. Materiales y métodos. Se aislaron tres cepas de K. pneumoniae con sensibilidad disminuida a cefalosporinas de amplio espectro, de neonatos con septicemia hospitalaria. La presencia de β -lactamasas de espectro expandido (BLEE) fue determinada fenotípicamente. Los plásmidos se aislaron y clasificaron según grupos de incompatibilidad por tipificación del replicón por reacción en cadena de la polimerasa (PCR). Los genes bla BLEE y su asociación a IS CR1 se determinaron por PCR y secuenciación directa, usando varios juegos de iniciadores. Resultados. Todas las cepas demostraron un perfil fenotípico indicativo de producción de BLEE, transferibles por conjugación. Los ensayos de PCR para para cefotaximasas (CTX-M) y el análisis de la secuenciación, revelaron que las cepas portaban genes bla CTX-M-1 y bla CTX-M-2. Estos genes se encontraron en plásmidos conjugados de 150 kb, aproximadamente, relacionados con los grupos IncN e IncFIIA, respectivamente. IS CR1 se encontró ´aguas arriba´ ( upstream ) y asociado con los genes bla CTX-M-1 y bla CTX-M-2. Conclusión. Este es el primer reporte realizado en Venezuela donde la presencia de IS CR1 está estrechamente asociada con la movilización de los genes bla CTX-M-1 y bla CTX-M-2 en plásmidos conjugativos IncN y IncFIIA en cepas de K. pneumoniae que circulan en una Unidad de Alto Riesgo Neonatal.

A clinical isolate of C. freundii with reduced susceptibility to extended-spectrum ß-lactams from a woman with cystocele associated with recurrent urinary tract infection was analyzed. Susceptibility tests, double disk synergy tests (DDST) and enzymatic activity by the agar iodometric method suggested the presence of ESBLs. Conjugation experiments revealed the presence of a large conjugative plasmid (pLM07/20) with an exclusive FrepB replicon type (IncF/FIB). PCR analysis and sequencing confirmed the presence of the blaCTX-M-14 gene in the pLM07/20 from C. freundii.LM07/10. Although this is the first report of CTX-M-14 in Venezuela, we alert the medical community that future increase of these ß-lactamases in our city could be due to dissemination of plasmids into bacterial populations.

Objetivo: Evaluar la calidad sanitaria del pollo crudo que se expende en supermercados del área urbana del estado Mérida, Venezuela, y caracterizar fenotípicamente las cepas aisladas de Samonella enterica. Materiales y métodos: Se estudiaron 45 muestras de pollo crudo (15 correspondieron a pollo sin aliños, 15 con aliños y 15 de origen industrial). Los indicadores de calidad sanitaria evaluados fueron: bacterias aerobias mesófilas, coliformes totales, Escherichia coli y Staphylococcus aureus, utilizando la metodología de la Comisión Venezolana de Normas Industriales. La identificación microbiológica y serológica de Salmonella spp. se realizó mediante técnicas convencionales. La sensibilidad antimicrobiana se determinó por concentración inhibitoria mínima (CIM) y, para la detección de β-lactamasas de espectro expandido (BLEE), se utilizó la prueba del sinergismo del doble disco. La sensibilidad a desinfectantes se determinó por el método de dilución-neutralización. Resultados: Independientemente de las características de la muestra de pollo crudo (no industrial) y del establecimiento comercial, los indicadores: bacterias aerobias mesófilas, coliforme totales, E. coli y S. aureus, superaron significativamente los recuentos límites de aceptabilidad. Salmonella enterica se aisló en 20% de las muestras, y las serovariedades Heidelberg (55,6%) y Enteritidis (22,2%) fueron las más frecuentes. Estas variedades serológicas presentaron multirresistencia y producción de BLEE, pero fueron sensibles a los desinfectantes probados. Conclusión: Los pollos crudos con aliños y sin ellos, manufacturados en los establecimientos estudiados, no cumplen con la calidad microbiológica adecuada, lo que hace de ellos un producto perecedero a corto plazo y un nicho favorecedor de patógenos importantes que amenazan la salud de los consumidores.


Objectives: Evaluate the sanitary quality of raw chicken sold in the urban area of the State of Merida, Venezuela and characterize the phenotype of isolated Samonella enterica strains. Materials and methods: A total of 45 raw chicken samples were studied; 15 of them were without seasoning, 15 were with seasoning, and 15 were industrially processed. The following sanitary quality indicators were assessed: Mesophiles Aerobic Bacteria (MAB), total coliforms, E. coli and S. aureus, and the Venezuelan Commission of Industrial Norms methodology was used for that purpose. Microbiological and serological identification of Salmonella was carried out using conventional techniques. Antimicrobial susceptibility was determined by minimal inhibitory concentration (MIC) and the detection of extended-spectrum β- lactamases (ESBLs) was carried out by using the double disc synergy test. Susceptibility to disinfectants was determined by the dilution-neutralization method. Results. Regardless of the (not industrial) samples’ characteristics and/or the commercial establishment, it was determined that the ranges for MAB, total coliform, E. coli and S. aureus counts significantly exceeded limits of acceptability. Strains of Salmonella enterica were isolated in 20% of the samples, whose distribution showed that serovars Heidelberg (55.6%) and Enteritidis (22.2%) were the most frequent. These serovars showed multi-resistance to antibiotics and ESBLs production but were susceptible to the tested disinfectants. Conclusion. This study showed that raw chickens, with and without seasoning, processed in the surveyed commercial establishments do not meet the required microbiological quality standards suitable for human consumption. Therefore, these are short-term perishable products and a haven for important pathogens that threaten consumer health.
