RESUMO

The metastatic involvement of the breast from non-mammary neoplasms is a relatively rare condition: 0.5 to 6% of the breast cancers. All cancers can give breast metastases, several months after the discovery of the primitive tumor or in 25% of the cases in being the first sign. Their clinical and radiological presentation polymorphic installation a problem of differential diagnosis between benign tumor, like the fibroadenomas on the one hand, and malignant tumors of other share. The presence of multiple, bilateral round tumors, superficial without the traditional signs of malignity which one meets with the primitive tumors: irregular margins, spiculations, microcalcifications, posterior cone of shadow is evocative diagnosis. The IRM finds the criteria of malignity specific to the primitive tumor. Pathologic examination completed with immunohistochemical tests is a key point for diagnosis. The search for a primitive tumor is essential, in particular for neuroendocrine carcinoma, stromal sarcoma, and the angiosarcoma which can be primitive tumors of the breast. Treatment is therefore modified, taking into consideration the treatment and prognosis of the primary disease.

RESUMO

The Lynch syndrome or HNPCC, is due to a mutation of the genes pertaining to the system MMR. This frequency varies from 1/1000 to 1/400. It's responsible for 6% of endométrial cancers, 2% of ovarian cancers and 1 to 3% of colorectal cancers. A good knowledge of this syndrome is necessary for doing the diagnosis and adjustment of gynaecological and colorectal follow-up. The authors report four cases in Lynch syndrome family.

RESUMO

The authors report a new cases of cystic lymphangioma of the adrenal diagnosed at the time of investigation of systemic hypertension which became normal after the lesion was treated surgically. The place of these lymphangiectasis cysts amongst adrenal cystic lesions, their clinical manifestations, methods of diagnosis and treatment are envisaged.

RESUMO

The authors present a homogeneous series of stress incontinence of urine in the woman treated by a method of indirect suspension of the cervico-urethral region. Results were assessed after a follow up of between three months and twelve years, with more than 5 years in 23 patients. After studying the underlying condition in which the disorder developed, they analyse the long term results which are presented using an actuarial technique. The deterioration in results seen during the first year (20% failures) was often related to associated instability of the detrusor. After one year the results remained stable with follow up periods of more than 10 years.

RESUMO

Nephroblastoma is a rare tumour in the adult and the diagnosis is only rarely made preoperatively. In the light of a new case, the authors consider the problems of histopathological differential diagnosis with other borderline forms. They study the clinical aspects of these tumours. The prognosis would seem to be extremely gloomy, worse than that of renal adenocarcinoma.

A rare form of true non-parasitic cysts of the spleen, cystic lymphangiomas are seen as round calcified images in the left hypochondrium. Bidimensional ultrasonography has considerably simplified the diagnosis of splenic cysts, but modern investigations, which can usually eliminate the presence of hydatid cysts in the spleen, cannot differentiate the various types of non-parasitic cysts. Diagnosis of cystic lymphangiomas, currently made after pathological examinations, can be assisted by needle-puncture aspiration guided by ultrasonography, and the role of this investigatory method in diagnosis and therapy is discussed. The only radical treatment for splenic cysts is splenectomy, which should be conducted routinely because of the difficulty of pre-operative diagnosis and the high risk of severe complications.
