RESUMO

OBJECTIVE: To assess the methylation patterns of four pluripotency gene promoters in mouse oocytes after in vivo maturation, in vitro maturation (IVM), and vitrification followed by IVM. DESIGN: Experimental study. SETTING: Research laboratory. ANIMAL(S): Three populations of metaphase II mouse oocytes were analyzed after in vivo maturation, IVM, and vitrification followed by IVM (V-IVM). Cumulus cells and blastocyst embryos were controls. INTERVENTION(S): The CpG methylation patterns (overall and CpG specific) in the promoters of four pluripotency genes (Oct4, Nanog, Foxd3, and Sox2) were analyzed for each cell type by traditional DNA bisulfite sequencing. MAIN OUTCOME MEASURE(S): Differences for overall methylation were evaluated using the Student's t-test and for individual CpG sites by χ2 analysis. RESULT(S): Significantly lower levels of overall methylation in promoters of Oct4 (25%) and Sox2 (4.5%) were noted in V-IVM oocytes compared with in vivo-matured oocytes (62.5% and 8.5%, respectively). Cumulus cell promoters were generally hypomethylated at Nanog, Foxd3. and Sox2, but hypermethylated at Oct4. CONCLUSION(S): The methylation status of Oct4 and Sox2 promoters of V-IVM mouse oocytes are altered when compared with in vivo-matured oocytes. The biological risk and significance of these changes are unknown and this study indicates caution and that further analyses are warranted.

RESUMO

Asymmetric obstructed uterus didelphys (Herlyn-Werner-Wunderlich syndrome) is a rare congenital müllerian anomaly consisting of uterus didelphys, hemivaginal septum, and ipsilateral renal agenesis. Herein is reported a case of incomplete Herlyn-Werner-Wunderlich syndrome diagnosed using 3-dimensional transvaginal ultrasound in a 14-year-old patient with absence of the hemivaginal septum. The most contributive diagnostic factors and appropriate therapeutic management in such cases are discussed.

The purpose of this study was to assess follicular viability and competence through in vitro maturation (IVM) of cryoloop vitrified mouse preantral follicles. Early mouse preantral follicles were isolated and vitrified using the cyroloop vitrification technique. After thawing, the preantral follicles and oocytes were cultured and in vitro matured for 10 d to the metaphase two stage (M2). Oocytes were assessed for viability at 2 and 10 d of IVM and compared to a control group of freshly isolated preantral follicles undergoing IVM. Of vitrified follicles, 94.0% (345/367) were recovered after thawing. The survival rate after the first two-days of IVM culture was 82.3% (284/345) for the cryoloop vitrified follicles and 100% for the control follicles (437/437). The percentage of oocytes in the cryoloop group that developed to M2 was 70.2% (174/248), comparable to that of the control group at 68.7% (241/351) (p value 0.12). Our results indicate that cyroloop vitrification is a viable and practical technique for cryopreservation of mouse preantral follicles. Human oocyte cryopreservation by means of cryoloop vitrification may prove to be useful as a possible treatment modality for human fertility preservation.

RESUMO

UNLABELLED: Over 4500 hematopoietic stem cell transplants (HSCT) are performed on patients in the United States each year. As HSCT patients shift their survivorship care from large transplant centers to community health care providers, many gynecologists are assuming their pre- and post-HSCT gynecologic care. This article reviews recommendations, current research, and expert opinions on the gynecologic care of HSCT patients. TARGET AUDIENCE: Obstetricians & Gynecologists, Family Physicians. LEARNING OBJECTIVES: After completing this educational activity, the physician should be better able to implement strategies for the prevention and management of menstrual bleeding during hematopoietic stem cell transplants; educate female patients regarding Fertility Preservation options before hematopoietic stem cell transplantation; and apply posthematopoietic stem cell transplant reproductive care screening and treatment recommendations for bone health, sexual health, and secondary cancer development.
