PURPOSE: Patients with brain metastases are often referred for brain radiotherapy (BrRT) when exclusive palliative management would be more appropriate. To assess the indication of BrRT during end-of-life (EOL) care and evaluate the characteristics of the patients who underwent the treatment. METHODS: This retrospective study comprised patients from four independent oncology centers who had undergone BrRT for metastases. The variables included were Karnofsky performance status (KPS), primary tumor site, metastatic status, neurologic symptomatic status, the number and size of metastases, posterior fossa or meningeal involvement, type of BrRT, having undergone brain metastasectomy, and the availability of systemic therapies after BrRT. Patients were allocated into three subgroups with ≤30, 31-60, and 61-90 days of survival, and a control group of patients who survived >90 days. RESULTS: A total of 546 patients were included in the study. A KPS of <70 (P = .021), the number of brain metastases (P = .001), the lack of brain metastasectomy (P = .006), and the lack of systemic therapies after BrRT (P = .047) were significantly associated with the EOL subgroups. Multivariate analysis showed that a KPS of <70 (P < .001), the lack of brain metastasectomy (P = .015), and the lack of systemic therapies after BrRT (P = .027) were significantly associated with worse survival. In all, 241 (44.1%) patients died within 90 days-120 (22.0%) within 30 days, 75 (13.7%) within 31-60 days, and 46 (8.4%) within 61-90 days of BrRT. Patients with colorectal cancer were significantly more likely to die within 90 days of BrRT than >90 days. CONCLUSION: Considering patients' performance status and whether they are candidates for brain metastasectomy or systemic therapies after BrRT is critical to improving BrRT benefits in scenarios of EOL.

This paper presents a computational and experimental analysis of a glaucoma flat drainage device (FDD). The FDD consists of a metallic microplate placed into the eye sclerocorneal limbus, which creates a virtual path between the anterior chamber and its exterior, allowing the intraocular pressure (IOP) to be kept in a normal range. It also uses the surrounding tissue as a flow regulator in order to provide close values of IOP for a wide range of aqueous humor (AH) flow rates. The Neo Hookean hyperelastic model is used for the solid part, while the Reynolds thin film fluid model is used for the fluid part. On the other hand, a gravitational-driven flow test is implemented in order to validate the simulation process. An in vitro experiment evaluated the flow characteristics of the device implanted in fourteen extirpated pig eyes, giving as a result the best-fit for the Young modulus of the tissue surrounding the device. Finally, according to the resulting computational model, for a range of 1.4-3.1 µL/min, the device presents a pressure variation range of 6-7.5 mmHg.

RESUMO

Hydroa vacciniforme (HV)-like lymphoma is a rare, usually fatal Epstein-Barr virus-driven lymphoproliferative disease affecting children from Asia, Mexico, and South America. Cutaneous manifestations imitate HV, a benign photodermatosis in which systemic symptoms are not observed, and spontaneous regression occurs later in adolescence or young adulthood. We report a case of HV-like lymphoma in a 12-year-old girl, descendent from an ancient Amazon indigenous tribe that, as far as we know, represents the second Brazilian case ever reported in the medical literature.

BACKGROUND: Control of the Asian citrus psyllid Diaphorina citri Kuwayama, the most important pest of citrus worldwide, is based on the use of insecticides, though unsatisfactory results have recently been reported. In this study, insecticide resistance of D. citri to three insecticides (bifenthrin, malathion, and chlorpyrifos) was examined. RESULTS: Three populations (designated Dci-CParácuaro, Dci-El Junco, and Dci-Antúnez) of both adults and fourth-instar D. citri individuals were collected in 2014 at two different times and on one occasion, respectively, from three locations (Crucero de Parácuaro, El Junco, and Antúnez). These locations represent the major commercial Mexican lemon production areas in the Apatzingán Valley in the state of Michoacán, Mexico. The three populations of D. citri adults and fourth-instar nymphs at the different collection times showed low levels of resistance (≤7-fold) to bifenthrin, but were very resistant to malathion (≤345- and ≤432-fold for adults and fourth instars, respectively) and chlorpyrifos (≤2435- and ≤1424-fold for adults and fourth instars, respectively). CONCLUSION: Resistance levels to the tested insecticides were highly variable but homogeneous among seasons and localities. Resistance management programmes that include crop sanitation, use of biological and cultural control practices, and rotation of insecticide classes should be established, particularly in areas where D. citri has developed resistance to malathion and chlorpyrifos. © 2017 Society of Chemical Industry.

Schwannomas arise from the Schwann cells of the peripheral and cranial nerves. They represent 8% of the primary cerebral neoplasms. Although schwannomas usually develop in sensory nerves, most often on the vestibular and trigeminal nerves, in very rare cases they can develop in motor nerves. We reported an unusual case of a 29-year-old woman with headache, nausea, vomiting, and blurred vision ongoing for 3 years. Magnetic resonance of the brain showed a solid-cystic expanded injury, heterogeneous, with limits partially defined and epicenter on the pineal gland. The lesion presented hyposignal in T1 and isosignal in T2. An intense enhancement of the solid part was observed after contrast injection. Foci of calcification and absence of diffusion restriction were also observed. The patient underwent microneurosurgery with supracerebellar infratentorial approach in a seated position. Subtotal resection was performed with maintenance of calcified tumor tissue adhered to the right Rosenthal basal vein. In the postoperative phase, the patient remained with diplopia when looking down; however, she reported improvement of headache and nausea.


Os schwannomas surgem das células de Schwann, dos nervos periféricos e cranianos e representam 8% das neoplasias cerebrais primárias. Apesar de os schwannomas se desenvolverem comumente em nervos sensoriais, mais frequentemente no nervo vestibular e trigêmeo, em casos muito raros ele pode se desenvolver em nervos motores. Relatamos um caso raro, de uma paciente do sexo feminino, 29 anos, com quadro de cefaleia, náuseas, vômitos e turvação visual há três anos. Ressonância magnética de encéfalo demonstrou lesão expansiva sólido-cística, heterogênea, com limites parcialmente definidos e epicentro na glândula pineal. Apresentou hipossinal em T1 e isossinal em T2. Houve intenso realce da parte sólida após injeção de contraste. Foram observados focos de calcificação e ausência de restrição à difusão. A paciente foi submetida a tratamento microneurocirúrgico com acesso infratentorial supracerebelar em posição sentada. Houve ressecção subtotal com manutenção de tecido tumoral calcificado aderido a veia basal de Rosenthal direita. No pós-operatório, a paciente permaneceu com diplopia na mirada ocular para baixo, entretanto apresentou melhora de cefaleia e náuseas.

Antimony is a metalloid that affects biological functions in humans due to a mechanism still not understood. There is no doubt that the toxicity and physicochemical properties of Sb are strongly related with its chemical state. In this paper, the interaction between Sb(III) and Sb(V) with bovine serum albumin (BSA) was investigated in vitro by fluorescence spectroscopy, and circular dichroism (CD) under simulated physiological conditions. Moreover, the coupling of the separation technique, asymmetric flow field-flow fractionation, with elemental mass spectrometry to understand the interaction of Sb(V) and Sb(III) with the BSA was also used. Our results showed a different behaviour of Sb(III) vs. Sb(V) regarding their effects on the interaction with the BSA. The effects in terms of protein aggregates and conformational changes were higher in the presence of Sb(III) compared to Sb(V) which may explain the differences in toxicity between both Sb species in vivo. Obtained results demonstrated the protective effect of GSH that modifies the degree of interaction between the Sb species with BSA. Interestingly, in our experiments it was possible to detect an interaction between BSA and Sb species, which may be related with the presence of labile complex between the Sb and a protein for the first time.

Clear cell papulosis is a rare, self-limited, benign disease of early childhood, characterized by white macules and flat papules over the milk line. Histopathologically, it is characterized by scattered clear cells throughout the basal and/or suprabasal epidermal layers, which-as clear cells of Toker of the nipple do-typically express cytokeratin 7. They also exhibit other markers expected for adenoid differentiation, such as low-molecular weight cytokeratins, carcinoembryonic antigen, epithelial membrane antigen, and mucin. The age of onset, distribution of lesions, histopathology, and its benign behavior nature help to exclude clinically similar conditions, either benign or malignant. The authors report a case of clear cell papulosis in a 7-year-old Brazilian girl in whom lesions were observed on the legs and histologically formed by solid and adenoid aggregates of clear cells, in a similar fashion than clear fetal cells of Toker.

Objetivo: Verificar a acurácia da ultrassonografia no seguimento das lesões mamárias diagnosticadas previamente como doenças benignas pela citopatologia. Métodos: Foram selecionados casos de pacientes submetidas à punção aspirativa com agulha fina de lesões mamárias e que a citopatologia descreveu achados benignos. Um grupo foi seguido com ultrassonografia e outro submetido à biópsia excisional ou incisional das lesões. A acurácia foi calculada a partir da sensibilidade, especificidade, valor preditivo positivo e negativo; e análise através da curva ROC. Resultados: Foram analisados 507 resultados citopatológicos, no período de agosto de 2002 a dezembro de 2010. Desses, 23,23% foram excluídos do estudo por terem sido classificados como material inadequado. A acurácia da punção aspirativa com agulha fina foi de 91,76%; quando comparada com os resultados anatomopatológicos. Já a acurácia da ultrassonografia foi de 99,38%. Quando comparados simultaneamente a biópsia e a ultrassonografia, utilizando a curva ROC, a acurácia do teste índice foi de 88 e 84%, respectivamente. Conclusão: O seguimento ultrassonográfico das lesões mamárias pode ser uma alternativa no seguimento de lesões mamárias diagnosticadas previamente como benignas à citopatologia.


Objective: The objective is to verify the accuracy of ultrasonography in the follow-up of breast lesions previously diagnosed as benign diseases by cytopathology. Methods: A retrospective and observational study aimed to compare the accuracy of fine needle aspiration in relation to the pathological exam and to the ultrasonography, in follow-up of breast lesions through the calculation of the sensitivity, specificity, positive predictive value, negative predictive value, and using the ROC curve. Results: 507 fine needle aspirations were analyzed in the period of August 2002 to December 2010. A total of 23.23% were excluded from the study because they were considered as inappropriate material. When compared to the pathological exam, the accuracy of fine needle aspiration was 91.76%. When related to the ultrasonography, the accuracy was 99.38%. When compared to both pathological exam and ultrasonography, using the ROC curve, the accuracy of the index test was 88 and 84%, respectively. Conclusion: The radiological follow-up of the breast lesions can be an alternative in the follow-up of lesions previously diagnosed as benign to fine needle aspiration.

BACKGROUND: Leprosy and HIV are diseases that have a major impact on public health in Brazil. Patients coinfected with both diseases, appear to be at higher risk to develop leprosy reactions. OBJECTIVE: The aim of this study is to describe the histopathological aspects of cutaneous lesions during reactional states in a group of patients with HIV-leprosy coinfection, compared to patients with leprosy, without coinfection. METHODS: Two groups were established: group 1 comprised of 40 patients coinfected with HIV-leprosy; group 2, comprised of 107 patients with leprosy only. Patients presenting reactional states of leprosy had their lesions biopsied and comparatively evaluated. RESULTS: Reversal reaction was the most frequent feature in both groups, with dermis edema as the most common histopathological finding. Giant cells were seen in all group 1 histopathological examinations. Dermis edema was the most common finding in patients with erythema nodosum leprosum. CONCLUSION: Few histopathological differences were found in both groups, with reversal reaction as the most significant one, although this fact should be analyzed considering the predominant BT clinical form in the coinfected group and BB form in the group without HIV. Larger prospective studies in patients with HIV-leprosy coinfection are needed to confirm and broaden these results.

The morphogenesis of giant missionary grass (Axonopus jesuticus x A. scoparius) was evaluated in this study in response to the application of 0, 40, 80, 120, 160, and 200m³ ha-1year-1 of pig slurry, calculated to provide 0, 100, 200, 300, 400, and 500kg N ha-1year-1, respectively. The experiment was carried out in the field, at Chapecó, Santa Catarina, Brazil, in 2010-2011. The doses were fractioned in four applications, performed after the pasture cuttings, during the growth season of the grass. Morphogenetic evaluations were performed weekly between 10/26/2010 and 12/07/2010 (spring), 12/14/2010 and 01/11/2011 (late spring/early summer), 01/18/2011 and 02/07/2011 (summer), and 02/15/2011 and 03/21/2011 (late summer). The leaf senescence, leaf elongation, and pseudoculm elongation rates, canopy and pseudoculm heights, leaf blade length, and tillering increased because of fertilization. The application of pig slurry as a source of nitrogen alters the tissue flow of giant missionary grass, which requires attention to pasture management in order to maximize the efficiency of forage use and to prevent losses of herbage by leaf senescence.(AU)


A morfogênese da grama-missioneira-gigante (Axonopus jesuticus x A. scoparius) tirar itálico dos parentes foi avaliada neste estudo em resposta à aplicação de doses de 0, 40, 80, 120, 160 e 200m³ ha-1ano-1 de dejeto líquido de suínos, calculadas para suprir 0, 100, 200, 300, 400 e 500kg de N ha-1ano-1, respectivamente. O experimento foi realizado no campo, em Chapecó, Santa Catarina, Brasil, em 2010-2011. As doses foram fracionadas em quatro aplicações, após os cortes da pastagem, durante a estação de crescimento da gramínea. As avaliações morfogenéticas ocorreram semanalmente entre 26/10/2010 e 07/12/2010 (primavera), 14/12/2010 e 11/01/2011 (final da primavera/início do verão), 18/01/2011 e 07/02/2011 (verão), e 15/02/2011 e 21/03/2011 (final do verão). As taxas de senescência foliar, alongamento foliar e alongamento do pseudocolmo, a altura do dossel e do pseudocolmo, o comprimento de lâmina foliar e o afilhamento foram incrementados como resultado da fertilização. A aplicação de dejetos de suínos como fonte de nitrogênio altera o fluxo de tecido de grama-missioneira-gigante, o que requer atenção ao manejo da pastagem para maximizar a eficiência de uso da forragem e prevenir perdas por senescência.(AU)

The etiopathogenesis remains unknown and the disease is characterized by a solitary, scaly or keratotic papule, or plaque mainly located on the extremities. Histopathological features closely resemble those of psoriasis. We report the case of a 70-year-old woman presenting a solitary and asymptomatic keratotic plaque, located on the back of the left leg, unresponsive to topical corticosteroids. We performed an excisional biopsy and histopathology was consistent with psoriasiform keratosis.

Histopathological features closely resemble those of psoriasis. We report the case of a 70-year-old woman presenting a solitary and asymptomatic keratotic plaque, located on the back of the left leg, unresponsive to topical corticosteroids. We performed an excisional biopsy and histopathology was consistent with psoriasiform keratosis.

Este trabalho teve como objetivo avaliar a morfogênese da grama-tapete (Axonopus affinis) adubada durante dois anos com dejeto líquido de suínos (DLS). Na média de dois anos, foram aplicados 0, 51, 102, 153, 204 e 255m³ de DLS ha-1, calculados para prover 0, 100, 200, 300, 400 e 500kg de N/ha/ano, respetivamente. A morfogênese foi avaliada na primavera e verão de 2008-2009 e 2009-2010, considerando-se como temperatura basal 10°C para cálculo da soma térmica no intervalo entre as amostragens. This study aimed to evaluate the morphogenesis of carpet grass fertilized for two years with pig slurry (PS). On the average of two years were applied 0, 51, 102, 153, 204 and 255m³ of PS ha-1, calculated to provide 0, 100, 200, 300, 400 and 500kg of N ha-1 year-1, respectively. The morphogenesis was evaluated in the spring and summer of 2008-2009 and 2009-2010, considering 10°C as threshold temperature to evaluate the thermal sum in degree-days (DD) between the samplings. The fertilization linearly increased the tiller number, tiller elongation rate, leaf elongation rate and leaf area index. There was no effect of the fertilization on pseudoculm elongation rate (0.004cm DD-1), leaf appearance rate (0.004 leaf tiller-1 DD-1) and phyllochron (274DD).

RESUMO

Este trabalho teve como objetivo avaliar a morfogênese da grama-tapete (Axonopus affinis) adubada durante dois anos com dejeto líquido de suínos (DLS). Na média de dois anos, foram aplicados 0, 51, 102, 153, 204 e 255m³ de DLS ha-1, calculados para prover 0, 100, 200, 300, 400 e 500kg de N/ha/ano, respetivamente. A morfogênese foi avaliada na primavera e verão de 2008-2009 e 2009-2010, considerando-se como temperatura basal 10°C para cálculo da soma térmica no intervalo entre as amostragens. Houve aumento linear no número de perfilhos, taxa de alongamento de perfilhos, taxa de expansão foliar e índice de área foliar em função das doses de DLS. This study aimed to evaluate the chemical and anatomical attributes of leaves of giant missionary grass to application of 0, 62, 124, 186, 248 and 310m³ ha-1 of pig slurry. At 83 days after the last application of fertilizer, the leaf blades were collected, fixed in FAA 70%, sectioned, stained, photographed and digitalized. The transversal section of leaf blades were evaluated for proportion of epidermis, lignified vascular tissue + sclerenchyma, non-lignified vascular tissue and parenchyma with an image-processing system calibrated to 1mm pixel-1. Leaf samples were analyzed for crude protein, acid detergent fiber, neutral detergent fiber and hemicellulose content by near infrared reflectance spectroscopy. The pig slurry application up to 310m³ ha-1 significantly increased the percentage of crude protein, parenchyma, epidermis, non-lignified vascular tissue and hemicellulose, while decreasing the percentage of acid detergent fiber and lignified vascular tissue + sclerenchyma. The Pearson's correlation was positive between crude protein and non-lignified vascular tissue, and between acid detergent fiber and lignified vascular tissue + sclerenchyma. The percentage of hemicellulose was positively correlated with epidermis, parenchyma and non-lignified vascular tissue. A negative correlation between acid detergent fiber and epidermis, parenchyma and non-lignified vascular tissue was observed.


Este trabalho teve como objetivo avaliar os atributos anatômicos e químicos da grama-missioneira-gigante à aplicação de 0, 62, 124, 186, 248 and 310m³ ha-1. Decorridos 83 dias do último corte e da aplicação do dejeto, as lâminas foliares foram coletadas, fixadas em FAA 70%, seccionadas, coradas, fotografadas e digitalizadas. Secções transversais das lâminas foliares foram avaliadas quanto à proporção das áreas de epiderme, tecido vascular lignificado + esclerênquima, tecido vascular não lignificado e parênquima, na região entre a quilha e o segundo maior feixe vascular, com sistema de processamento de imagem calibrado para 1mm pixel-1. Amostras de folhas foram coletadas e analisadas quanto ao teor de proteína bruta, fibra insolúvel em detergente ácido e fibra insolúvel em detergente neutro, pelo método de espectrometria de reflectância no infravermelho proximal. A aplicação de até 310m³ ha-1 do dejeto líquido de suíno aumentou os percentuais de proteína bruta, hemicelulose, parênquima e tecido vascular não lignificado, reduzindo os percentuais de fibra insolúvel em detergente ácido e tecido vascular lignificado + esclerênquima. A correlação de Pearson foi positiva entre o percentual de proteína bruta e tecido vascular não lignificado, e entre o percentual de fibra insolúvel em detergente ácido e tecido vascular lignificado + esclerênquima. O percentual de hemicelulose mostrou correlação positiva com epiderme, parênquima e tecido vascular não lignificado. Houve correlação negativa entre o percentual de fibra insolúvel em detergente ácido e epiderme, parênquima e tecido vascular não lignificado. O dejeto líquido de suínos, utilizado como fonte de nitrogênio, promove melhoria em atributos químicos e anatômicos, que afetam o valor nutritivo da grama-missioneira-gigante.

RESUMO

Tufted angioma is an acquired vascular proliferation with specific histological characteristics. The most common clinical features are erythematous macules. It occurs predominantly in children and young adults, especially in the chest and neck. We report the case of a male patient, 12 years old, with an increased right ear auricle associated with erythematous macules and increased local temperature, referred with a diagnosis of lepromatous leprosy; however, this diagnosis was questioned. Histopathology was consistent with tufted angioma.
