RESUMO

The objective of this research was to study complications of orbital floor reconstruction by making analysis of early and late follow up treatment results of patients with orbital floor fractures. We summerized the experience in surgery of 215 victims with zygomatico-orbital complex fractures, who underwent treatment in teaching hospitals of the oral and maxillo-facial surgery department of Privolzhsky Research Medical University from 2011 to 2017. The orbital wall fractures which required reconstruction and restorative treatment were diagnosed in 78 (36.3%) patients. The amount of surgical intervention included transconjuctive approach and orbital floor reconstruction with polymeric and metal implants. In most cases we obtained good and satisfactory results during the period of early and late follow up. We made the analysis of developed early and late complications from orbital floor reconstruction. Post-operative enophthalmos and implant displacement are noted to be the most adverse events in the late follow up period and they require re-surgery. Our experience suggests that it is possible to avoid complications and receive good esthetic and functional results in case a surgeon is highly qualified and carefully plans an operation, namely calculation of anatomical and topometrical orbit parameters by CT and a right choice of operation methods and implants.

RESUMO

The localization of signaling molecules close to their targets is the central principle of cell signaling. The colocalization of multicomponent signaling complexes is realized through protein scaffolds that provide better specificity than undirected diffusion ofthe same components. ROS-generating complexes have been suggested to follow this principle by specific intracellular localization of ROS production and the limitation of ROS diffusion distances. However, the lack of adequate methods did not allow direct detection of local ROS production to confirm the model ofredox signaling compartmentalization. Nevertheless, evidences of local ROS production and restriction of diffusion were provided by kinetic modeling and data on the subcellular localization of NADPH-oxidase isoforms, their adapter proteins and local restriction of ROS diffusion. Here we shall discuss the properties of antioxidant system which prevents uncontrolled ROS diffusion from the sites of generation to the adjacent subcellular compartments; the current data of the specific localization NADPH-oxidases activity and its influence on intracellular processes; the recent evidences of the ROS diffusion restriction.

RESUMO

The study presents the results of surgical management of 65 patients with zygomatico-orbital complex and orbital floor fractures according to a self-designed technique using polymer implants Reperen. Being a method of choice the technique reduces treatment time, promotes good esthetic results, reduces complication rate, and contributes to a patient's quality life improvement.

Based upon clinical immunological checkup of 76 patients with common phlegmons of maxillofacial region, their complications and analysis of general phlogogenic activity the variants of hyperergic inflammation process flow were determined, that allowed to diagnose its activity and forecast possibility of its further development and its outcome.

Until recently, the production of reactive oxygen species by NADPH oxidase has been considered only in the context of the oxidative damage to pathogens inside the phagosome. However, homologues of phagocytic NADPH oxidase have been found in almost all cell types, where they produce hydrogen peroxide and thereby regulate the initial intracellular stages of MAP kinase cascades. In the present work, the activation of two MAP kinase cascades, p38 and Erk1/2, during phagocytosis has been studied. It was found that phagocytosis activates both cascades. The activation of Erkl/2 is dependent, and the activation of p38 is not dependent, on the activity of NADPH oxidase. Thus, it can be stated that the activation of MAP kinases in phagocytes during phagocytosis occurs by a mechanism similar to that operating in nonphagocytizing cells, indicating the universality of the function of NADPH oxidases in different cell types.

Using HyPer, a ratiometric GFP-based biosensor, the dynamics of H2O2 in living cells has been studied. It was found that activation of the receptor of the epidermal growth factor (EGF) in epithelial cells leads to sustained generation of intracellular H2O2, which is blocked by apocynin, an inhibitor of the assembly of plasma membrane NADPH oxidase. Apocynin also blocked HeLa cell proliferation induced by EGF, indicating that NADPH oxidase should be involved in the process. However, apocynin failed to alter the kinetics of the EGF-stimulated ERK1/2 activation. It was concluded that NADPH oxidase and intracellular H2O2 are the important downstream targets of the EGF receptor, which mediate the proliferation response by mechanisms distinct from the activation of the classical ERK1/2 MAP-kinase pathway.

RESUMO

There are some differences between hypertension in men and women. For example, female sex hormones protect against the development of essential hypertension in premenopausal women. On the other hand, in pregnant women, hypertension must be identified as severe disease in every cases. Essential hypertension in postmenopausal women may be categorized for common disease in many cases. In this article, I show some datas and my comments about female hypertension for mainly daily therapeutic observation.

Although adrenal gland is one of the major target organs of angiotensin II (Ang II), the pathophysiological significance of the its receptor subtype has not been elucidated. We demonstrated by reverse transcription-polymerase chain reaction with Southern blot analysis mRNA expression of both AT1 and AT2 in human adrenal tissues of normal adrenocortical tissues, aldosterone-producing adenoma, Cushing's syndrome, and pheochromocytoma. Ang II-induced aldosterone secretion in vitro was suppressed only by 50% in the presence of selective AT1 antagonist CV-11974, while AT2 agonist CGP-42112 increased aldosterone secretion by 55% over the control. Ang II or CGP-42112 did not affect cortisol secretion. In addition, Ang II could stimulate aldosterone secretion in AT1a knockout mice both in the presence and absence of CV-11974. These results suggest that non-AT1 receptor subtype(s) including AT2, as well as AT1, is involved in the stimulation of aldosterone secretion from human adrenals.

Besides the circulating renin-angiotensin-aldosterone (R-A-A) system, the tissue R-A-A system has been elucidated to play important roles as autocrines and/or paracrines. The components of R-A-A system are expressed in the ovary, uterus and placenta, indicating the existence of the tissue R-A-A system in these organs. The data indicating the involvement of R-A-A system of these tissues into reproduction have been accumulated. AT2 receptors might modulate the initiation and progression of follicle atresia involving granulosa cell apoptosis. AT2 receptors are expressed abundantly in the uterus and decreased during pregnancy. The placental renin are shown to be secreted into the maternal circulation and elevate blood pressure. It is expected to elucidate the significance of the R-A-A system in the reproductive system.

OBJECTIVE: The aim of this study was to investigate the effects of calcium channel blockers on messenger RNA expression of endothelin-1 and endothelial-type nitric oxide synthase in the cardiovascular tissue of stroke-prone spontaneously hypertensive rats (SHRSP). MATERIALS AND METHODS: The calcium channel blocker nilvadipine (1.0 or 3.2 mg/kg per day) was subcutaneously administered to two groups of SHRSP, from 4 or 8 weeks of age, for 8 weeks and 4 weeks, respectively. For comparison, nifedipine (3.2 mg/kg per day) was similarly administered to SHRSP from 4 weeks of age for 8 weeks. Kidney, heart, aorta and brain tissue samples were obtained when the rats were 12 weeks old. Messenger RNA expression of endothelin-1 and endothelial-type nitric oxide synthase was determined by reverse transcription-polymerase chain reaction followed by Southern blotting and a ribonuclease protection assay, respectively. Results were compared with those in untreated SHRSP and Wistar-Kyoto rats at 12 weeks of age. RESULTS: Both nilvadipine and nifedipine significantly decreased blood pressure in SHRSP. Although there were no changes in the weights of the kidney and brain, there was a significant decrease in the weight of the left ventricle of the groups treated with nilvadipine (1.0 mg/kg per day: mean +/- SEM 0.282 +/- 0.003 g; 3.2 mg/kg per day: 0.269 +/- 0.005 g) and nifedipine (1 mg/kg/day: 0.281 +/- 0.012 g) for 8 weeks compared with untreated SHRSP (0.301 +/- 0.004 g). Endothelin-1 messenger RNA expression, which was significantly increased by about twofold in the kidney, heart and brain of SHRSP compared with Wistar-Kyoto rats, was normalized by both calcium blockers. Endothelin-1 messenger RNA expression, which was decreased in the aorta of SHRSP, was further decreased by both calcium blockers. While there was no significant difference in endothelial-type nitric oxide synthase messenger RNA expression in the kidney, heart and aorta between the untreated SHRSP and Wistar-Kyoto rats, expression in the aorta was significantly increased in the group treated with these calcium blockers for 8 weeks from 4 weeks of age. CONCLUSIONS: These results suggest that, in addition to their potent antihypertensive effects, calcium channel blockers may exhibit cardiovasculoprotective and renoprotective effects by modifying mRNA expression of endothelin-1 and endothelial-type nitric oxide synthase in tissue.

RESUMO

A total of 814 newborns were examined using a highly sensitive method of indication of viral antigens. A high incidence of congenital viral infection was revealed: 450 per 1000 live-born children. Enteroviral infections (mainly Coxsackie) ranked first in prevalence. These infections in the newborns are mainly caused by the vertical transmission of enteroviruses from mothers with persistent forms of infections. Virtually all the other viruses (influenza viruses, cytomegalovirus, herpes simplex virus, parainfluenza, measles, and rubella viruses) are detected in the newborns included in the group at a high risk of vertical transmissions of enteroviral infections. In the majority of cases the said viruses were detected in association with enteroviruses.

Pneumonia is a major cause of death in the elderly. To investigate the role of silent aspiration in community-acquired pneumonia, we examined the occurrence of silent aspiration during sleep in 14 elderly patients with acute episode of pneumonia and 10 age-matched control subjects by a new technique using indium111 chloride. Scanning of the thorax demonstrated that 71% of patients aspirated, whereas aspiration was observed in only 10% of control subjects. The percentage of positive scans was significantly higher in patients with acute episode of pneumonia than in control subjects (p < 0.02). The results may indicate an important role of silent aspiration in the development of community-acquired pneumonia in the elderly.
