RESUMO

This study explored the detectability of category 3 or higher microcalcifications using 5-MP color and monochrome monitors. Contrast detail mammography phantom with polymethyl methacrylate (PMMA) images were observed in color and monochrome by five radiographers, and the image quality figures (IQF) were calculated based on the gold disc locations identified. Five radiographers and two radiologists observed 200 mammograms from 100 patients (including 36 with microcalcifications) and rated the microcalcifications. The results were analyzed using area under the curve (AUC) and jackknife resampling. A paired t test was used for statistical analysis (p < 0.05). The mean IQF of color and monochrome monitors were 10.73 and 10.49 (30 mm PMMA, p = 0.653) and 8.47 and 8.74 (50 mm PMMA, p = 0.774), respectively. The mean AUC of color and monochrome monitors were 0.917 and 0.936 (p = 0.335), respectively, with windowing and magnification. The detectability of microcalcifications was not significantly different between the monitors.

CONTEXT: Pheochromocytoma (PHEO) and paraganglioma (PGL) (PHEO and PGL: PPGLs), catecholamine-producing tumors, represent an emerging cause of secondary osteoporosis. However, despite decreased bone mineral density (BMD), vertebral fracture (VF) is not associated with BMD in PPGLs. OBJECTIVE: To evaluate whether deteriorated bone quality is involved in the development of osteoporosis in PPGLs. PARTICIPANTS: Trabecular bone score (TBS), used to assess trabecular bone quality, was examined in 56 patients with PPGLs and 52 with non-functional adrenal tumors (AT). Radiograph of the spine was carried out in 35 patients with PPGLs, and TBS was analyzed in 18 patients with PPGLs at follow-up. MAIN OUTCOME MEASURE: TBS and BMD at the lumbar spine in patients with PPGLs with and without VF. RESULTS: PPGLs had a lower TBS (n = 56, 1.338 [1.294-1.420]) than non-functional AT (n = 52, 1.394 [1.342-1.444]; p = 0.033). Among those with PPGLs, patients with VF (n = 14, 1.314 [1.289-1.346]) had a lower TBS than those without VF (n = 21, 1.383 [1.324-1.426]; p = 0.046), despite no significant difference in BMD at the lumbar spine between the two groups (p = 0.501). An optimal cut-off level of TBS for diagnosing VF in PPGLs was 1.323, and its area under the curve was 0.702. The severity of catecholamine excess and maximal size of tumor were associated with decreased TBS in PPGLs patients (p = 0.016 and p = 0.020, respectively). Surgical resection of PPGLs improved TBS at follow-up, with 2.5% increase (p = 0.007). CONCLUSIONS: This study provides evidence for the importance of deteriorated bone quality rather than decreased bone mass in the development of VF in PPGLs.

We aimed to examine the substance in a precipitate of heat-treated onion concentrate (HOC) that contributes to a sensation of lingering of aroma, a koku attribute induced by the sensing of richness and persistence in terms of taste, aroma and texture. Adding precipitate, separated from HOC, to consommé enhanced the lingering sensation of aroma in the consommé more than adding the supernatant from HOC. After the precipitate was washed with hot water and ethanol its enhancing effect disappeared. Analysis of the HOC precipitate showed that it contained phytosterols, such as beta-sitosterol and stigmasterol. Tests of binding to aroma compounds showed that both sterols, as well as the washed precipitate, were able to bind methyl propyl disulfide and N-hexanal. Thus phytosterols in the HOC precipitate seemed to bind and hold the aroma compounds and gradually release them, inducing a lingering sensation of aroma under the koku concept during consumption.

Workloads of radiological technologists under different conditions of heights of radiographic table and/or X-ray tube assembly were calculated using a software for preventing musculoskeletal complaint to investigate optimal working environment for general X-ray examinations. In the patient positioning, compressive force of lumbar disc decreased at higher radiographic table within the range of 45-90 cm. On the other hand, workload of the shoulder joint increased with increase in the height of radiographic table. Load of the shoulder joint similarly increased as the height of the X-ray tube assembly increased. Compressive force of lumbar disc reduced by approximately 10-30% as the height ratio of the radiographic table to body height increased by approximately 40%, compared to the lowest table of 45 cm. Muscle load of a 50-years-old woman was approximately double compared to a 30-year-old man, even in the same workload. It is important to keep suitable height of radiographic table for reduction of the workloads of lumbar rather than shoulder joint, because floating-type radiographic table is generally used.

PURPOSE: The purpose of this study was to evaluate the clinical usefulness of partial volume effect (PVE)-corrected F-18 FDG PET/CT for predicting I-131 accumulation in metastatic lymph nodes (mLNs) during I-131 therapy for papillary thyroid carcinoma (PTC). METHODS: Sixty-five mLNs in 31 PTC patients who underwent F-18 FDG PET/CT in an initial radioiodine therapy (RIT) were retrospectively evaluated. Of these, 25 mLNs were I-131-positive and 40 were I-131-negative. SUVmax and SUVmax with PVE correction (cSUVmax) were measured for each mLN, where PVE correction was performed utilizing a simple table lookup correction method. Then, SUVmax/cSUVmax was compared between I-131-positive and I-131-negative mLNs, including the analyses for the mLNs with small-sized (<1 cm) and weak FDG accumulation (SUVmax <3.5). The predictability for I-131 accumulation with SUVmax/cSUVmax was also compared. RESULTS: For all 65 mLNs, SUVmax/cSUVmax was significantly higher in I-131-negative than I-131-positive mLNs (p < 0.0001). Only in cSUVmax, I-131-negative mLNs were significantly higher than I-131-positive, in terms of the 30 small-sized mLNs (p = 0.0001) and 14 mLNs with weak FDG uptake (p = 0.007). The highest accuracy in predictability for I-131 accumulation was significantly better with cSUVmax (92 %) than SUVmax (62 %) (p < 0.0001). CONCLUSION: PVE-corrected F-18 FDG PET/CT is a valuable predictor of I-131 accumulation in mLNs during RIT.

Trigonella foenum-graecum (fenugreek) can ameliorate dyslipidemia, but the detailed mechanism is unclear. In this study, we examined the effects of fenugreek on hepatic lipid metabolism, particularly lipogenesis, which is enhanced in obesity and diabetes, in diabetic obese KK-Ay mice. KK-Ay mice were fed a control high-fat diet (HFD; 60% of energy as fat) (C group) or an HFD containing 0.5% or 2% fenugreek (0.5F and 2.0F groups, respectively) for 4 wk. Hepatic and plasma TG and mRNA expression levels of lipogenic genes were lower in the 2.0F group at 4 wk (P < 0.05), but not in the 0.5F group, than in the C group. The hydrolyzed saponin fraction, but not the saponin fraction per se, in fenugreek inhibited the accumulation of TG in HepG2 cells. We fractionated the hydrolyzed saponin into 15 fractions by HPLC and examined the effect of these fractions on TG accumulation in HepG2 cells. Fraction 11 inhibited TG accumulation in HepG2 cells and we determined by liquid chromatography tandem MS that the active substance contained in fraction 11 is diosgenin. Diosgenin (5 and 10 µmol/L) inhibited the accumulation of TG and the expression of lipogenic genes in HepG2 cells. Moreover, diosgenin inhibited the transactivation of liver-X-receptor-α, as measured using a luciferase assay system and by gel mobility shift assay. These findings suggest that fenugreek ameliorates dyslipidemia by decreasing the hepatic lipid content in diabetic mice and that its effect is mediated by diosgenin. Fenugreek, which contains diosgenin, may be useful for the management of diabetes-related hepatic dyslipidemias.

OBJECTIVE: We evaluated the partial volume effect in PET/CT images and developed a simple correction method to address this problem. METHODS: Six spheres and the background in the phantom were filled with F-18 and we thus obtained 4 different sphere-to-background (SB) ratios. Thirty-nine cervical lymph nodes in 7 patients with papillary thyroid carcinoma (15 malignant and 24 benign) were also examined as a preliminary clinical study. First, we developed recovery coefficient (RC) curves normalized to the maximum counts of the 37-mm sphere. Next, we developed a correction table to determine the true SB ratio using three parameters, including the maximum counts of both the sphere and background and the lesion diameter, by modifying the approximation formula of the RC curves including the point-spread function correction. The full width at half maximum in this formula is estimated with the function of the SB ratio. RESULTS: In the phantom study, a size-dependent underestimation of the radioactivity was observed. The degree of decline of RC was influenced by the SB ratio. In preliminary clinical examination, the difference in the SUV(max) between malignant and benign LNs thus became more prominent after the correction. The PV correction slightly improved the diagnostic accuracy from 95 to 100%. CONCLUSIONS: We developed a simple table lookup correction method for the partial volume effect of PET/CT. This new method is considered to be clinically useful for the diagnosis of cervical LN metastasis. Further examination with a greater number of subjects is required to corroborate its clinical usefulness.

In obesity, adipocyte hypertrophy and chronic inflammation in adipose tissues cause insulin resistance and type-2 diabetes. Trigonella foenum-graecum (fenugreek) can ameliorate hyperglycemia and diabetes. However, the effects of fenugreek on adipocyte size and inflammation in adipose tissues have not been demonstrated. In this study, we determined the effects of fenugreek on adipocyte size and inflammation in adipose tissues in diabetic obese KK-Ay mice, and identified the active substance in fenugreek. Treatment of KK-Ay mice with a high fat diet supplemented with 2% fenugreek ameliorated diabetes. Moreover, fenugreek miniaturized the adipocytes and increased the mRNA expression levels of differentiation-related genes in adipose tissues. Fenugreek also inhibited macrophage infiltration into adipose tissues and decreased the mRNA expression levels of inflammatory genes. In addition, we identified diosgenin, a major aglycone of saponins in fenugreek to promote adipocyte differentiation and to inhibit expressions of several molecular candidates associated with inflammation in 3T3-L1 cells. These results suggest that fenugreek ameliorated diabetes by promoting adipocyte differentiation and inhibiting inflammation in adipose tissues, and its effects are mediated by diosgenin. Fenugreek containing diosgenin may be useful for ameliorating the glucose metabolic disorder associated with obesity.

Obese adipose tissues are characterized by the enhanced infiltration of macrophages. It is considered that the paracrine loop involving monocyte chemoattractant protein-1, tumor necrosis factor-alpha, and the free fatty acid between adipocytes and macrophages establishes a vicious cycle that aggravates inflammatory changes and insulin resistance in obese adipose tissues. Diosgenin, a saponin aglycon found in a variety of plants, has anti-inflammatory properties. In the present study, we examined the effect of diosgenin on the inflammatory changes in the interaction between adipocytes and macrophages. A coculture of 3T3-L1 adipocytes and RAW 264 macrophages markedly enhanced the production of tumor necrosis factor-alpha, monocyte chemoattractant protein-1, and nitric oxide compared with the sum of their single cultures; however, treatment with diosgenin inhibited the production of these proinflammatory mediators. Diosgenin also suppressed the inflammation in RAW 264 macrophages that was induced by the conditioned medium derived from 3T3-L1 adipocytes. Furthermore, diosgenin inhibited the conditioned medium-induced degradation of inhibitor kappaB and the phosphorylation of c-jun N-terminal kinase in macrophages. These results indicate that diosgenin exhibits anti-inflammatory properties in the interaction of adipocytes and macrophages by inhibiting the inflammatory signals in macrophages. Diosgenin may be useful for ameliorating the inflammatory changes in obese adipose tissues.
