RESUMO

Modification of Ferguson haemorrhoidectomy had been started because it was easier to ligate the haemorrhoidal pedicle with a rubber band instead of using the stitch. There is no need to use a retractor for such a procedure as it would cause discomfort to the patient when only infiltrative anaesthesia for one or two haemorrhoidal complexes was given. In the period from 1994 to 1999, we performed 398 haemorrhoidectomies as a one-day surgical procedure under local infiltrative anaesthesia. The examination follow-ups of the patients were performed and medical charts were reviewed. Early postoperative complications were rare: haemorrhage occurred in 1.8%, urine retention in 0.5%, high temperature in 1.3% and temporary incontinence in 0.3%. Overall, 28 patients (7%) had additional treatment for residual haemorrhoid problems 5-10 years after the primary haemorrhoidectomy was performed. We believe that our modified technique is a welcome alternative to the one-day surgical practice.

BACKGROUND: The aim of this report is to describe our experience with stapled haemorrhoidopexy as a day surgery procedure. METHODS: From January 2000 to January 2003, a total of 214 patients with third- and fourth-degree haemorrhoids underwent stapled haemorrhoidopexy under spinal anaesthesia. We analysed early postoperative complications and long-term results. Patients were followed for 4-36 months (mean, 22 months). Only 3 patients (1%) were hospitalised. The long-term complications were analysed by means of a mailed questionnaire. RESULTS: Minor bleeding at wiping after defecation was observed by 9% of patients and minor haemorrhoidal prolapse by 8% of patients. Pain after defecation was reported by 6% of patients and anal stenosis occurred in 2% of them. Faecal urgency was reported by 3% of patients with previously unknown incontinence problems. CONCLUSION: According to our experience, stapled haemorrhoidopexy can be safely performed as a day surgery procedure.

BACKGROUND/AIMS: The modern segmental concept of the liver with a segmental bile duct distribution is of practical importance for biliary specialists and should include information on variations of intrahepatic and extrahepatic bile ducts. The aim of this study was to find biliary variations, especially on a segmental and sectorial level, and to try to arrange them into types. METHODOLOGY: We analyzed 51 corrosion casts of the human liver, which enabled the three-dimensional study of extrahepatic, sectorial, segmental and subsegmental bile ducts, their variations in course and confluencing, and the relationship between the structures of the portal pedicle. RESULTS: The types of confluence and their frequency were determined separately for the left, right, right anterior sectorial, right posterior sectorial and common hepatic ducts. "Normal" left confluence was formed in 82% and three variations in 18%. "Normal" right confluence was formed in 75% and four variations in 25%. A complete ("normal") anterior sectorial duct was present in 35% and four variations in 65%. A complete ("normal") posterior sectorial duct was present in 61% and four variations in 39%. CONCLUSIONS: The study showed that the biliary tree variations are quite frequent, and therefore clinically important, and that they could be arranged into several types.
