RESUMO

THE AIM OF THE STUDY: Cisplatin's effect on the inner ear in children and compare results of the tonal audiometry and the oto-acousitic emission audiometry. MATERIAL AND METHODS: Audiological examinations were conducted on 33 children who had been treated with Cisplatin. Normal hearing prior to the cytostatic therapy would qualify a child for inclusion in the examinations. Cisplatin was administered via intravenous infusion in dosages of 15 mg/m2 to 20 mg/m2 on five successive days of each month or 50-75 mg/m2 every 3 weeks. The patients were divided into groups according to ratio of the cumulative dosage per 1 m2 of the body area. Hearing was assessed with tonal and impendence audiometry as well as oto-acousitic emission audiometry--DPOAE. RESULTS: The tonal audiometry helped detect hearing damage in 12 out of the 33 children participating in the examination, with unilateral loss found in 2 and bilateral loss encountered in 10 children. Hearing threshold shifts ranged between 10 and 40 dB in frequencies above 2000 Hz. The largest hearing threshold shift was registered for the region of 6000-8000 Hz. Tonal audiometry and oto-acoustic emission audiometry result consistency was encountered in 11 children from the study group. CONCLUSIONS: No effect of the cytostatic dose level on the degree of hearing loss could be demonstrated in the examined group of children. The sensitivity of the oto-acoustic emission and of the tonal audiometry to detect and monitor hypacusis were found to be comparable.

INTRODUCTION: Papillomatosis of the larynx appears as the result of an infection by the human papilloma virus (HPV). In children, the disease produces benign lesions, which grow rapidly and show a marked tendency to recur once removed. The course of the disease is unpredictable. Some patients can achieve spontaneous remission; however, in other cases the disease progresses violently and requires multiple operations. There is currently no effective therapy for recurrent respiratory papillomatosis (RRP). The basic goal of treatment is to preserve the patency of air passages while simultaneously preserving the phonatory and anatomical functioning of the larynx. Over the last 10 years, the possibility of a pharmacological treatment has been continually explored: in particular, the administration of locally passed cidofovir (first reported in a 1999 study by Pransky) to the larynx with papillomas is under current scrutiny. GOAL OF STUDY: The goal of the following study is to estimate the effectiveness of locally administered cidofovir for treatment of papillomas of the larynx in children. PATIENTS AND METHODS: From 2005 to 2007, 10 patients (seven boys and three girls) between the ages of 1-18 years were treated. Operations were executed using microlaryngoscopy (Karl-Storz endoscopy) with general anesthesia. Cidofovir was passed intralesionally with concentration 5-10mg/ml in quantity from 1 to 5mg/kg in a 4-week interval. RESULTS: Seven of the 10 patients achieved total remission 6 months after the completion of treatment, scoring 0 points on Derkay's scale. These patients underwent therapy with cidofovir from 6 to 13 months. Three months after the completion of treatment, three children suffered a recurrence of papillomas. Two of them are estimated at 3 points on Derkay's scale, while the third is estimated at 5 points. CONCLUSIONS: The results of the study indicate that intralesional injections of cidofovir can be an effective method of treatment for recurrent respiratory papillomatosis in children.

RESUMO

Authors report two cases of benign, osteogenic neoplasms of the maxilla in children which defined as cemento-ossifying fibroma and juvenile cemento-ossifying fibroma. Difficulty in diagnosis of the tumor is indicated. The importance of clinical data as well as radiology and histopathology examination for a proper diagnosis is indicated. Review of literature is presented.

RESUMO

Neurofibroma is a tumor rarely found arising in the head and neck region. A rare case of solitary neurofibroma of the nose in a 2 years old child is presented. Tumor after delivery was observed. Big size and localization of the tumor with accompany destruction bone and cartilage of the nose was indicated.

RESUMO

A newborn infant with rare teratoma of infratemporal fossa is presented. This neoplasm develops in childhood or early adulthood, generally in gonads. 10% of all teratomas are tumors of head and neck. The diagnosis needs to be made promptly, usually with the aid of computerized tomography, followed by excisional biopsy. The treatment of choice is operation and complete excision of tumor. The review of literature is presented.
