RESUMO

OBJECTIVES: To investigate the concentrations of osteocalcin and collagen type I C-terminal telopeptides in pregnant women with pre-eclampsia. STUDY DESIGN: 26 patients with severe pre-eclampsia and 24 healthy pregnant women were included in the study. Serum concentrations of osteocalcin and C-telopeptides--degradation products of type I collagen were determined using the ELISA method. Statistical analysis was performed using Mann-Whitney U-test. RESULTS: In pre-eclamptic patients, the concentrations of osteocalcin and degradation products of collagen type I were significantly higher (P<0.005) when compared to healthy pregnant women. CONCLUSION: These results could suggest that there are alterations in bone metabolism in pregnant women with pre-eclampsia.

RESUMO

Rush pins are used for stabilization of forearm shaft fractures after surgical reduction. The pins are bent intraoperatively to conform to the natural curvature of the radius. Introduction of the pin into the radius takes place between the extensors of the index, the extensor digitus communis and the extensor pollicis longus. Safe and equally effective introduction of the pins is also possible through the olecranon when treating fractures of the proximal part of the ulna or from distal end in fractures of the distal half of the ulna. The clinical material in this paper consisted of 21 patients treated surgically with Rush pins because of fractures of the forearm. In two cases primary consolidation was not achieved. Union of the fracture did occur after periostage and iliac bone grafting. Long term follow-up of 19 patients demonstrated a full range of motion in the involved extremity in 14 cases. Limited rotation of the forearm was noted in 5 patients.

RESUMO

Symptoms and treatment of 16 years old patient who had osteochondritis dissecans of the patella is discussed. The separated cartilaginous fragment has been surgically removed, subchondral bone multiply drilled. After 3 months of exercises patient returned to the normal level of physical activity.

RESUMO

Principles of proximal row carpectomy and results of treatment in 8 patients with pseudoarthrosis of the scaphoid and in 7 patients with inveterate carpal dislocation are presented. Mean follow-up was 5 years. Mean range of movement was 68 degrees, the pain decreased or ceased in 14 patients, grip strength increased 14%.

RESUMO

Etio-pathomorphogenesis of compression syndromes of the ulnar nerve strained over the medial side of the valgus elbow is presented on the basis of follow-up of 11 patients. Valgus deformity of the elbow resulted from neglecting of surgical reduction of the displaced capitulum and part of the trochlea in the childhood. Cartilage of the articular surface of the displaced fragment facing injury site of the humeral condyle impedes healing of the fracture and disturbs the growth of the lateral epiphysis. Thus valgus deformity is accompanied by pseudoarthroses and various deformities of the articular surface. Function of the distorted elbow joint was good, however, and the range of motion pretty wide. An irritation or late paresis of the ulnar nerve appeared only 17 or even 45 years after the fracture in childhood. Permanent ulnar paresis can be prevented by early nerve transposition, that is during first year of ulnar nerve irritation symptoms.

RESUMO

Pathomorphogenesis of radioulnar synostosis which occurred in 4 out of 174 patients treated because of recent and inveterate fractures as well as pseudoarthroses within shafts of forearm bones has been presented. Fractures of both radius and ulna, located at the same level within proximal half, inveterate or neglected, after severe or multiple trauma are at the highest risk of developing radioulnar synostosis.

RESUMO

The method and the results of surgical treatment of Smith and Barton fracture of the distal radial epiphysis has been presented in 10 patients. Dislocation of fragments is reduced after exposure of the palmar side of the distal radial bone. Stabilization is secured by a metal plate, which supports the epiphysis and is fixed to the proximal fragment by screws. It prevents from redislocation of fragments and helps to recover full activity of the hand, which was obtained in 8 patients. Delayed reduction or stabilization of fragments by Kirschner wires in 2 persons has failed.

RESUMO

The late results of partial patellectomy in treatment of fracture of the apex or the 1/3 of the peripheral patella have been presented. Fourteen patients were evaluated. Attention was paid to the dependence of the result on the correct surgical technique. Full painless range of movements in the operated on knee was achieved in 7 persons. In the other 7 patients, extension of the knee was full but flexion was moderately limited. Periodical pain occurs in 3 persons.

RESUMO

The surgical technique of patellectomy with preservation of continuity of the extension apparatus of the knee by the de la Caffinière method and the early results of treatment in 8 operated on patients have been presented. The indications for surgery were: posttraumatic stiffness of the knee in extension in 4 patients, patello-femoral arthrosis with preserved wide range of movement in 3 patients, fresh multi-fragmented fracture of the patella without rupture of the extension apparatus in one patient. The good treatment result was obtained in 6 patients (3 treated for patello-femoral arthrosis, 1 for fresh fracture of the patella, and 2 for stiffness of the knee in extension). The poor results in the other 2 patients operated on or stiffness of the knee in extension were due to large damage of the cartilaginous surface of the condyles and extensive intra- and periarticular adhesions.
