Aim: Statins are associated with lower risk of gallstones due to anti-inflammatory effects. We assessed whether statins impact circulating inflammation among Chilean women with gallstones. Materials & methods: 200 Mapuche women were matched on statin use and age to 200 non-Mapuche women in the Chile Biliary Longitudinal Study. We analyzed 92 inflammatory biomarkers using multivariable-adjusted regression models, random forests and pathway analyses. Results: Statins were not significantly associated with any inflammation marker when women were analyzed jointly or stratified by ancestry. No significant associations were found through random forest methods and pathway analyses. Discussion: We did not find significant associations between statin use and inflammation markers in women with gallstones, suggesting that statins do not reduce inflammation once gallstones have formed.


Statins are prescribed to lower cholesterol and can also decrease the risk of gallstone formation by reducing inflammation. We assessed whether statin use reduces inflammation among women who have already developed gallstones. We analyzed 92 inflammation markers among 400 women in Chile, including 200 women with Mapuche Amerindian ancestry and 200 women of Latina/European ancestry. We found that statin use was not correlated with inflammation in this group of women overall nor by ancestry. This may mean that statin use does not reduce inflammation in women who already were diagnosed with gallstones.

Traumatic abdominal wall hernias are a rare complication of high energy blunt trauma. There exist several studies evaluating and outlining potential management options but still no generalized consensus on management. This series was meant to evaluate the diagnosis and management of traumatic abdominal wall hernias. A prospectively maintained database was used to identify patients with TAWH from 2021 to 2022. The primary outcome was operative management. Secondary outcomes included: time to diagnosis and post-operative outcomes. Of the 19 patients in this case series, 100% (n = 19/19) were secondary to blunt trauma with a mean ISS of 21. Exploratory laparotomy was performed in 17 cases. 14 cases had concomitant traumatic injuries to visceral structures. Complications were found in nearly half of the patients with 3 experiencing wound dehiscence. Future studies should be aimed at standardizing management approach taking into account nature of the mechanism and concomitant injuries.

BACKGROUND: Obligate blood-feeding insects obtain the nutrients and water necessary to ensure survival from the vertebrate blood. The internal taste sensilla, situated in the pharynx, evaluate the suitability of the ingested food. Here, through multiple approaches, we characterized the pharyngeal organ (PO) of the hematophagous kissing bug Rhodnius prolixus to determine its role in food assessment. The PO, located antero-dorsally in the pharynx, comprises eight taste sensilla that become bathed with the incoming blood. RESULTS: We showed that these taste sensilla house gustatory receptor neurons projecting their axons through the labral nerves to reach the subesophageal zone in the brain. We found that these neurons are electrically activated by relevant appetitive and aversive gustatory stimuli such as NaCl, ATP, and caffeine. Using RNA-Seq, we examined the expression of sensory-related gene families in the PO. We identified gustatory receptors, ionotropic receptors, transient receptor potential channels, pickpocket channels, opsins, takeouts, neuropeptide precursors, neuropeptide receptors, and biogenic amine receptors. RNA interference assays demonstrated that the salt-related pickpocket channel Rproppk014276 is required during feeding of an appetitive solution of NaCl and ATP. CONCLUSIONS: We provide evidence of the role of the pharyngeal organ in food evaluation. This work shows a comprehensive characterization of a pharyngeal taste organ in a hematophagous insect.

Stevia rebaudiana Bertoni water extracts have been used as a natural sweetener and customary medicine by the indigenous inhabitants of South America for several hundred years. This plant was sent to Europe in the 16th century and was described by Peter Jacob Esteve in Spain. Recently the food industry has started to employ S. rebaudiana as sweetener using its glycosides after purification. Advertisement claims that Stevia glycosides is good for controling body mass and reducing glycemia. This study's objective was to evaluate the effect of S. rebaudiana leaf extract on Wistar rats as animal model to prove its effectiveness on body mass control, glycemia reduction, and other biochemical parameters. Three groups were randomly formed with 24 males and 24 females: A blank group without any sweetener, a control group drinking water with 10% glucose, and the test group ingesting a 0.94% water extract of S. rebaudiana. Body mass measurements as well as food and drink consumption were daily performed. The experiment lasted 120 days after the specimens were weaned and got used to eating solid food. Euthanasia was done and blood serum was collected to evaluate the following biochemical parameters: Glucose, triglycerides, cholesterol, insulin, glucagon, leptin, ghrelin, and glucose-dependent insulinotropic peptide, GIP. Results indicated that only female rats had statistical differences in body mass gain. No relevant effects either positive or negative were found in the biochemical parameters measured. The crude extracts of S. rebaudiana did not show any relevant changes in biochemical and hormonal profiles, changes nor body mass with respect to the blank and control groups of young and healthy rats in the age range of infancy to youth. According to the results obtained, the therapeutic properties that have been associated to S. rebaudiana consumption especially for body mass control and glycemia reduction, did not occur in young and healthy male and female rats in equivalent age to infants, young children, and youths.

OBJECTIVE: Investigation of fundamental movement skills (FMS) and physical fitness differences among Hispanic and Non-Hispanic White elementary school children. MATERIALS AND METHODS: This study included 194 children aged between 4 and 11 in kindergarten, 2nd, and 5th grades. For evaluable participants, t-tests were used to test potential differences in fitness or fundamental movement skills. RESULTS: There were significant group differences across the participants in terms of age, height, body mass index (BMI), BMI percentile, arm circumference, and waist circumference. In addition, there were significant differences between Hispanic and Non-Hispanic White groups in terms of catching, running, dodging, punting, and two-hand striking (p's < 0.05). Lastly, there were significant differences between Hispanic and Non-Hispanic White in pull-ups, push-ups, and sit-ups (p's < 0.05). CONCLUSIONS: Our study shows Hispanic children have lower fundamental movement skills and physical fitness compared to their Non-Hispanic White peers. As poor fundament movement skills and physical fitness at such a young age may lead to adverse implications for their physical fitness and health in adulthood. Future studies should focus on increasing fitness, and FMS in Hispanic elementary school children.

Japanese plum fruits are rich in phenolic compounds, such as anthocyanins and flavan-3-ols, whose contents vary significantly among cultivars. Catechin (C) and epicatechin (EC) are flavan-3-ol monomers described in the fruits of this species and are associated with bitterness, astringency, antioxidant capacity, and susceptibility to enzymatic mesocarp browning. In this study, we aimed to identify quantitative trait loci (QTL) associated with the content of flavan-3-ol in Japanese plum fruits. We evaluated the content of C and EC in the mesocarp and exocarp of samples from 79 and 64 seedlings of an F1 progeny (<'98-99' × 'Angeleno'>) in the first and second seasons, respectively. We also constructed improved versions of linkage maps from '98-99' and 'Angeleno,' presently called single-nucleotide polymorphisms (SNPs) after mapping the already available GBS reads to Prunus salicina Lindl. cv. 'Sanyueli' v2.0 reference genome. These data allowed for describing a cluster of QTLs in the cultivar, 'Angeleno,' associated with the flavan-3-ol composition of mesocarp and exocarp, which explain up to 100% of the C/EC ratio. Additionally, we developed a C/EC metabolic marker, which was mapped between the markers with the highest log of odds (LOD) scores detected by the QTL analysis. The C/EC locus was located in the LG1, at an interval spanning 0.70 cM at 108.30-108.90 cM. Our results suggest the presence of a novel major gene controlling the preferential synthesis of C or EC in the Japanese plum fruits. This study is a significant advance in understanding the regulation of synthesizing compounds associated with fruit quality, postharvest, and human health promotion.

RESUMEN Introducción: Las tecnologías de la información y las comunicaciones han proporcionado nuevas formas de publicar los resultados científicos a través de plataformas digitales de fácil manejo. Objetivo: Valorar la influencia de los mecanismos de la sociedad de consumo en el uso de las revistas en acceso abierto en las condiciones actuales de los países subdesarrollados. Método: Se asumió como método general de investigación el dialéctico-materialista y, de manera particular, los métodos de revisión bibliográfica para desarrollar un análisis crítico-reflexivo sobre el contenido relacionado con la temática; se tomaron en cuenta tesis de doctorado, artículos originales, de revisión y editoriales publicados entre 2010 y 2021. La búsqueda fue realizada en las bases de datos SciELO y Google Académico en junio 2021; las palabras clave utilizadas fueron: "revistas", "acceso abierto" y "tecnologías de la información y las comunicaciones". Resultados: Se analizaron aspectos relacionados con el tema, entre ellos: el concepto de acceso abierto, la geopolítica del conocimiento y su influencia para los investigadores del Sur; se alerta sobre el riesgo de colocar artículos en una revista depredadora. Conclusiones: Publicar en revistas en acceso abierto puede ser una ruta espinosa por carencias en el manejo de las plataformas en acceso abierto y habilidades de redacción del artículo científico; por la geopolítica del conocimiento y las limitaciones que suele imponer el mundo de la mercantilización editorial; y porque se corre el riesgo de ser engañado por revistas depredadoras, megadepredadoras y secuestradoras.


ABSTRACT Introduction: Information and communication technologies have provided new ways of publishing scientific achievement through user-friendly digital platforms. Objective: To assess the influence of the consumer society mechanisms on the use of the Open Access journals in the current conditions of underdeveloped countries. Method: It was assumed the dialectical materialist method as general method, particularly, it was used the literature review to develop a critical-reflexive analysis of the content related to the subject matter. Various doctorate thesis, original articles, review articles and editorials published between 2010 and 2021 were taken into account. The search was conducted in the SciELO and Google Scholar databases in June 2021; the keywords used were: "journals", "open access" and "information and communication technologies". Results: Aspects related to the topic were analyzed, among them: the concept of Open Access, knowledge geopolitics and its influence on researchers from the South; the risk of placing articles in a predatory journal was warned. Conclusions: Publishing in Open Access journals can be a thorny path due to the lack of Open Access platform management and scientific article writing skills; due to the geopolitics on the knowledge and the limitations usually imposed by the world of editorial commoditization; including the risk of being deceived by predatory, mega-predatory and hijacking journals.

Método: O método dialético-materialista foi assumido como método geral de pesquisa e, de forma particular, os métodos de revisão bibliográfica para desenvolver uma análise crítico-reflexiva sobre o conteúdo relacionado à temática; Foram consideradas teses de doutorado, artigos originais, de revisão e editoriais publicados entre 2010 e 2021. A busca foi realizada nas bases de dados SciELO e Google Academic em junho de 2021; as palavras-chave utilizadas foram: "periódicos", "acesso aberto" e "tecnologias da informação e comunicação". Resultados: Foram analisados aspectos relacionados ao tema, entre eles: o conceito de Acesso Aberto, a geopolítica do conhecimento e sua influência nos pesquisadores do Sul; ser alertado para o risco de veicular artigos em revista predatória. Conclusões: Publicar em periódicos em Acesso Aberto pode ser um caminho espinhoso devido às deficiências na gestão de plataformas de Acesso Aberto e na habilidade de redação de artigos científicos; pela geopolítica do conhecimento e as limitações que o mundo da comercialização editorial costuma impor; e porque você corre o risco de ser enganado por revistas predatórias, megadedatórias e sequestradoras.

OBJECTIVE: To compare approaches to myomectomy (laparotomic, laparoscopic, and robotic). To show the relationship between the number of fibroids and the reproduction diagnosis. METHODS: Observational, analytical, retrospective, and cross-sectional study; where the surgical approach used, was evaluated in terms of surgical bleeding, time, number and weight of fibroids and reproductive results. RESULTS: 69 patients were treated through different approaches and divided into 3 groups. The differences found among groups were in favor of laparotomic myomectomy in terms of the number (p=0.000) and weight of fibroids (p=0.004). Robotic surgery was also longer (p=0.000). In the analysis of the influence of the number of fibroids to achieve pregnancy, the result was in favor of the minimally invasive routes, after surgery, both in the group of < 6 fibroids (p=0.017), and that of > 6 fibroids (p=0.001), without differences in the time from surgery to pregnancy (p=0.979). CONCLUSIONS: The surgical approach decision should consider the number and size of resected fibroids, surgical time, and reproductive diagnosis. The minimally invasive route should be offered whenever possible due to its better outcome on achieving pregnancy, without forgetting the benefits of laparotomy, while also accrediting the recently introduced robotic-assisted approach.

RESUMO

This article reports the case of a 28-year-old female 31.6 weeks pregnant with twins diagnosed with SARS-CoV-2 infection, who delivered a boy and a girl. The newborns underwent RT-PCR testing for SARS-CoV-2; the male tested negative and the female newborn tested positive, in that the female placenta was SARS-CoV-2 positive and the male placenta negative. Clinical and laboratory findings evincing vertical transmission of SARS-CoV-2 were identified. Strict, multidisciplinary prenatal care is recommended for this group of patients. This case report alone does not provide statistical evidence of vertical transmission, but it is an account of a relevant matter.

BACKGROUND: Despite the liver being one of the most frequently injured abdominal organs in trauma patients, clinical management strategies differ between trauma surgeons. Few studies have critically evaluated current practice patterns in the operative management of liver trauma. Historical studies recommended against the use of drains but there has not been a modern investigation of this issue. The objective of this study was to analyze outcomes associated with intra-operative drain use for liver trauma. METHODS: A retrospective chart review of all adult trauma patients presenting to a Level I trauma center from 2012 to 2018 was performed. Patients who underwent operative management of liver trauma were divided into groups based on whether an intra-abdominal drain was utilized and differences in outcomes between the groups were analyzed. The primary endpoint evaluated was post-operative intra-abdominal abscesses. Univariate and multivariate analyses were performed. RESULTS: 184 patients with operative management of liver trauma were included in the study. Closed suction drains were utilized in 26.1% of post-operative patients. Rate of intra-abdominal abscesses was significantly higher in the drain group (35.4% versus 8.8%, P < 0.001). Drains were more commonly used in patients receiving more units of PRBCs (median, 9 units [IQR 4-20] versus median 5.5 units, [IQR 2-14], P = 0.03). Drain use was found to be an independent risk factor for post-operative intra-abdominal abscess on multivariate analysis (OR 4.9, 95% CI 1.7-14, P = 0.003). CONCLUSIONS: The results of this study support previous conclusions that drain placement for operative liver trauma is associated with increased risks of infectious complications. Drains were used in patients with more severe liver injury, intra-operative bile leaks, penetrating trauma, and increased blood transfusion requirements. Future studies should focus on the development of specific guidelines for the use of drains in liver trauma.

OBJECTIVE: Serum anti-Müllerian hormone (AMH) presents a strong positive correlation with quantitative aspects of the ovarian reserve, while its correlation with embryo quality is unclear. This study assessed the association between serum AMH as a marker of ovarian reserve and embryo quality, in women undergoing in vitro fertilization. METHODS: This observational analytical retrospective study included patients seen between 2010 and 2018. In vitro fertilization patients with measured AMH levels were analyzed based on the following parameters: number of retrieved oocytes; number of metaphase II oocytes; embryo quality; and treatment outcome. Statistical analysis was performed using ANOVA, Mann-Whitney U test, linear regression, and Pearson and Spearman correlations. RESULTS: We found a positive correlation between AMH levels, number of retrieved oocytes and number of metaphase II oocytes (r 0.649, p=0.000). The numbers of retrieved and metaphase II oocytes were predicted in 42% (R2: 429) of the cases based on AMH levels (p=0.000). Serum AMH levels were not associated with embryo quality on Day 3 (p=0.151); an association was seen between AMH levels and embryo quality on Day 5 (p=0.006). The distribution of AMH levels was the same across patients, regardless of whether they were able to achieve pregnancy (p=0.767). CONCLUSIONS: AMH levels correlated with embryo quality on Day 5; no association was found between AMH levels and embryo quality on Day 3 or pregnancy rate. The use of AMH levels to predict embryo quality still requires further studies; therefore, AMH should be used to assess the ovarian reserve only.

Introduction: Despite vast research, premature birth's electrophysiological mechanisms are not fully understood. Prediction of preterm birth contributes to child survival by providing timely and skilled care to both mother and child. Electrohysterography is an affordable, noninvasive technique that has been highly sensitive in diagnosing preterm labor. This study aimed to choose the more appropriate combination of characteristics, such as electrode channel and bandwidth, as well as those linear, time-frequency, and nonlinear features of the electrohysterogram (EHG) for predicting preterm birth using classifiers. Methods: We analyzed two open-access datasets of 30 minutes of EHG obtained in regular checkups of women around 31 weeks of pregnancy who experienced premature labor (P) and term labor (T). The current approach filtered the raw EHGs in three relevant frequency subbands (0.3-1 Hz, 1-2 Hz, and 2-3Hz). The EHG time series were then segmented to create 120-second windows, from which individual characteristics were calculated. The linear, time-frequency, and nonlinear indices of EHG of each combination (channel-filter) were fed to different classifiers using feature selection techniques. Results: The best performance, i.e., 88.52% accuracy, 83.83% sensitivity, and 93.22% specificity, was obtained in the 2-3 Hz bands using Medium Frequency, Continuous Wavelet Transform (CWT), and entropy-based indices. Interestingly, CWT features were significantly different in all filter-channel combinations. The proposed study uses small samples of EHG signals to diagnose preterm birth accurately, showing their potential application in the clinical environment. Discussion: Our results suggest that CWT and novel entropy-based features of EHG could be suitable descriptors for analyzing and understanding the complex nature of preterm labor mechanisms.

RESUMEN Fundamento: la educación cívica juega un papel preponderante en la formación y desarrollo de valores, sentimientos, convicciones y principios de las nuevas generaciones de estudiantes de las ciencias médicas en Cuba. Objetivo: diseñar acciones educativas para el perfeccionamiento de la educación cívica en estudiantes de las carreras de tecnología de la salud en Villa Clara. Métodos: se realizó una investigación de desarrollo en la Facultad de Tecnología y Enfermería en Villa Clara, durante el curso 2019-2020. Se utilizaron métodos teóricos: análisis-síntesis, inducción-deducción e histórico-lógico; y empíricos: revisión documental, entrevista, cuestionario y observación, y el criterio de especialistas para la valoración de las acciones educativas. Resultados: se constató que existen dificultades en el cumplimiento de las normas de la educación cívica, lo cual expresa insuficiencias en algunos docentes al aplicarlas; y en los estudiantes en su comportamiento social; por lo que se diseñaron acciones educativas para contribuir al reforzamiento de valores éticos, morales y cívicos, acordes con el proyecto educativo de la sociedad cubana. Conclusiones: las acciones fueron valoradas por los especialistas como pertinentes, útiles y novedosas. Su aplicación permitió el mejoramiento, no solo de conocimientos teóricos en docentes y estudiantes, sino el reconocimiento de mejoras en sus comportamientos ciudadanos y la confrontación de puntos de vistas, criterios y concepciones frente a patrones incorrectos de conductas.


ABSTRACT Background: civic education plays a preponderant role in the formation and development of values, feelings, convictions and principles of the new generations of students of medical sciences in Cuba. Objective: to design educational actions for the improvement of civic education in students of health technology degrees in Villa Clara. Methods: a development investigation was carried out at the Faculty of Technology and Nursing in Villa Clara, during the 2019-2020 academic year. Theoretical methods were used: analysis-synthesis, induction-deduction and historical-logical; and empirical ones: documentary review, interview, questionnaire and observation, and the criteria of specialists for the assessment of educational actions. Results: it was found that there are difficulties in complying with the civic education standards, which expresses inadequacies in some teachers when applying them; and in students in their social behavior; therefore, educational actions were designed to contribute to the reinforcement of ethical, moral and civic values, in accordance with the educational project of the Cuban society. Conclusions: the actions were valued by the specialists as pertinent, useful and novel. Its implementation allowed the improvement, not only of theoretical knowledge in teachers and students, but also the recognition of improvements in their citizen behaviors and the confrontation of points of view, criteria and conceptions against incorrect patterns of behavior.

Resumen Objetivo: Evaluar el agotamiento emocional o cansancio emocional, realización personal y despersonalización en el quehacer de los médicos del Servicio de Traumatología y Pabellón Central del Hospital Hernán Henríquez Aravena (HHHA) y Departamento de Cirugía de la Universidad de La Frontera, Temuco, Chile. Materiales y Método: Corte transversal. 48 sujetos: 10 residentes de traumatología y 8 de anestesiología, 12 anestesiólogos y 18 traumatólogos académicos asistenciales. Se aplicó bajo consentimiento informado: Inventario de Maslash (MBI). Análisis: tendencia central, porcentaje, así como t de student y coeficientes de alfa de Cronbach. Resultados: La prevalencia global del síndrome de burnout es 97%, por dimensiones, cansancio emocional mostró una prevalencia del 100%; baja realización personal en el trabajo 100% y despersonalización 91,6%. Para traumatología: cansancio emocional mostró prevalencia 100%; baja realización personal en el trabajo 100%, y despersonalización 82%. Para anestesiología: cansancio emocional mostró prevalencia 75%; baja realización personal en el trabajo 30% y despersonalización 30%. Existen diferencias significativas en cansancio emocional siendo mayor en género femenino y en quienes duermen menos de 7 h. El coeficiente de alpha de Cronbach del MBI fue de 0,7. Conclusión: Tanto residentes como académicos asistenciales de traumatología y anestesiología mostraron un elevado cansancio emocional que coexiste con una baja realización personal en el trabajo y alta despersonalización. En Chile, las políticas de protección están principalmente enfocadas en la Seguridad y Salud Ocupacional con la gestión de los riesgos, pero no se observan políticas de protección al estrés y salud mental que involucren contención y apoyo a la labor terapéutica de los profesionales de la medicina.


Aim: To assess levels of burnout, including emotional exhaustion, personal accomplishment and depersonalization, in the daily work of academic doctors in the Orthopedic Surgery Service and Central Surgery Service of the Hospital Hernán Henríquez Aravena (HHHA) and Surgery Department in the Universidad de La Frontera, Temuco, Chile. Materials and Method: Cross-sectional study. 48 subjects participated: 10 residents in orthopedic surgery, 8 residents in anesthesiology, 12 academic anesthesiologists and 18 academic orthopedic surgeons. Instruments applied under informed consent: Maslach Burnout Inventory (MBI). Analysis: Measures of central tendency and percentage, independent sample t-tests. Cronbach's alpha coefficients of the MBI. Results: The overall prevalence of burnout syndrome is 97%, whereas by dimensions, emotional exhaustion showed a prevalence of 100%, low personal accomplishment at work 100% and depersonalization 91.6%. For orthopedic surgery by dimensions: emotional exhaustion showed a prevalence of 100%, low personal accomplishment at work 100% and depersonalization 82%. For anesthesiology: emotional exhaustion showed a prevalence of 75%, low personal accomplishment at work 30% and depersonalization 30%. Only gender and hours of sleep showed significant differences in emotional exhaustion, with higher scores for women and those who slept less than 7 hours. 0.7 Cronbach's alpha of the MBI. Conclusión: There is excessive emotional fatigue, low job felt accomplishment and depersonalization in orthopedic surgery residents and academic. In anesthesiology, emotional high exhaustion coexists with low personal accomplishment values and depersonalization high. In Chile, protection policies are mainly focused on Occupational Safety and Health with risk management, but there are no stress protection and mental health policies involving containment and support for the therapeutic work of medical professionals.

Consumption of fresh fruit is known to protect against non-communicable diseases due to the fruit's content in compounds with an antioxidant capacity, among them is polyphenols. Asian plums (Prunus salicina L.) accumulate more than 40 phenolic compounds, with a remarkable diversity in their profiles, depending on the variety and environmental conditions. Although candidate genes have been indicated to control this trait, the loci controlling its phenotypic variation have not yet been defined in this species. The aim of this work was to identify the quantitative trait Loci (QTL) controlling the phenolic compounds content in the Asian plum skin and flesh. Using UHPLC-DAD-Orbitrap-MS, we determined that cyanidin-3-glucoside and cyanidin-3-rutinoside are the main anthocyanins in Asian plums. Other anthocyanins found to a lesser extent were tentatively identified as cyanidin bound to different sugar and procyanidin moieties. Then we phenotyped fruits of 92 and 80 F1 seedlings from the cross < "98.99" × "Angeleno"> (98 Ang) for two harvest seasons. We used HPLC-DAD to quantify single anthocyanin and spectrophotometric techniques to determine the total content of phenols, flavonoids, procyanidins, and antioxidant activity (DPPH and FRAP). To determine the phenotype-genotype association of phenolic compounds content, phenotypic values (adjusted by linear mixed-effects models), genotypic data and linkage maps were analyzed with the multiple QTL model (MQM) approach. We found a total of 21 significant trait-marker associations: 13 QTLs segregating from "98.99" and 8 QTLs from "Angeleno." From these associations, 8 corresponded to phenolic compound content in the flesh and 13 in the skin. Phenotype variance was explained by the detected loci, ranging from 12.4 to 27.1%. The identified loci are related to the content of cyanidin-3-glucoside (LG4), cyanidin-3-rutinoside (LG4), total flavonoids and procyanidins (LG5 and LG8), and minor anthocyanin compounds (LG3 and LG4). These results will help improve the efficiency of breeding programs for the generation of Asian plum varieties with high phenolic compound content.

OBJECTIVE: This study aimed to examine the association between serum estradiol levels and number of metaphase II oocytes harvested after in vitro fertilization cycles used in embryo transfers and the subsequent impact on pregnancy rates. METHODS: This observational analytical retrospective study was carried out in 2010-2018 at the Angeles del Pedregal Hospital. It included 181 cases and looked into the number of metaphase II oocytes to predict pregnancy rates. Statistical analysis was based on the calculation of correlations between variables and logistic regressions. RESULTS: Estradiol levels increased with the number of oocytes by a median correlation (r=0.482, p=0.000). On the day of trigger, estradiol levels predicted the number of retrieved oocytes with 23% reliability (R2=0.232, p=0.000); a linear trend correlation of r=0.489, p=0.000 was found between estradiol levels on the day of trigger and number of metaphase II oocytes. CONCLUSIONS: Serum estradiol on the day of trigger as a predictor of metaphase II oocytes in antagonist cycles encourages greater oocyte maturity and fertilization, whereas, in isolation, it does not determine the pregnancy achievement.

Resumen Antecedentes: La hormona antimüllleriana (HAM) ha sido utilizada como marcador de reserva ovárica, lo cual ayuda a predecir el éxito. Se desconoce si valores de otros países son apicables a México. Objetivo: Determinar los valores de HAM en nuestra población para graficarlas acorde a grupos de edad y evaluar su comportamiento. Metodología: Estudio observacional, descriptivo, retrospectivo y transversal de pacientes del 2008 al 2018, con reporte de nivel de HAM excluyendo a pacientes con enfermedades que alteran su nivel, analizando por grupos de edad en un laboratorio. Resultados: 450 pacientes de 36.93 años, el rango del valor de la HAM fue desde 0.003 ng/ml hasta 9.4 ng/ml, con una media de 1.5, con una disminución anual a partir de los 40 años (0.1-0.3). Correlación de Spearman de -436 (p < 0.000) con una asociación leve entre la edad y el nivel de HAM, pero sí lineal (p < 0.000). Conclusiones: La HAM es útil y sirve para dar recomendaciones personalizadas, así como base de estudios epidemiológicos en pro de una medicina individualizada.


Abstract Background: Antimüllerian hormone (AMH) has been used as a marker of ovarian reserve, which helps predict success. It is unknown if values from other countries are applicable to Mexico. Objective: To determine the AMH values in our population to graph them according to age groups and evaluate their behavior. Methodology: Observational, descriptive, retrospective, and cross-sectional study of patients from 2008 to 2018, with a AMH level report excluding patients with diseases that alter their level, analyzing by age groups in a laboratory. Results: 450 patients of 36.93 years old, the range of the AMH value was from 0.003 ng/ml to 9.4 ng/ml, with a mean of 1.5, with an annual decrease from 40 years of age (0.1-0.3). Spearman's correlation of -436 (p < 0.000) with a slight association between age and AMH level, but it was linear (p < 0.000). Conclusions: AMH is useful and serves to give personalized recommendations, as well as basis of epidemiological studies in favor of an individualized medicine.

Purpose: Safety and toxicity evaluation of a novel, liposome-encapsulated rapamycin formulation, intended for autoimmune ocular disorders. Methods: The formulation was assessed by micronucleus polychromatic erythrocyte production, irritability by Hen's Egg Test-Chorioallantoic Membrane (HET CAM), sterility, and pyrogenicity testing. Subconjunctival (SCJ) and intravitreal (IVT) administration of the formulation were performed to evaluate subacute and acute toxicity, respectively. Differences between groups in biochemical and hematological parameters were evaluated by analysis of variance and t-tests. Numeric score was assigned to histopathological classification. Electroretinography (ERG) testing was also performed. Data were analyzed by a 1 way no parametric Kruskal-Wallis and the Mann-Whitney tests. Significance was considered when P < 0.05. Results: No significant toxicity directly related to the preparation was detected. Micronucleus count, mucous irritation score, and pyrogenicity results were negative. Pathology demonstrated no damage related to the formulation after SCJ injection. After IVT injection, only lens injury associated with technique was observed. Retinal function was also conserved in ERG. Conclusions: The preparation evaluated offers a good toxicity and safety profile when injected in a SCJ or IVT manner in an animal model. A clinical trial conducted in humans is highly warranted, as it could reveal an alternative immunosuppressive treatment for ophthalmological immune-mediated pathologies.

Resumen Objetivo: Describir resultados en términos de morbilidad y mortalidad de la colecistectomía extendida laparoscópica (CELap) en pacientes con cáncer de vesícula biliar (CVB) incidental. Materiales y Método Serie de casos de pacientes con CVB incidental sometidos a CELap en el Hospital Regional de Temuco entre diciembre de 2017 y marzo de 2019. Resultados: Incluimos 10 pacientes, con edad promedio de 59,2 ± 11 años, 90% de género femenino. Respecto a la invasión de pared de la vesícula biliar (TNM), 1 presentó invasión hasta mucosa (T1a) con invasión de senos de Rokitansky Aschoff y 9 hasta subserosa (T2). Dos tuvieron ganglio cístico positivo en biopsia inicial. Respecto a la CELap, el tiempo operatorio promedio fue 333 ± 40 minutos. El promedio de ganglios resecados fue 4 ± 2,78, presentando lecho hepático positivo en 1 paciente. La clasificación TNM obtenida: un paciente T1aN0M0, siete T2N0M0 y dos T2N1M0. La estancia hospitalaria promedio fue 5 ± 2,3 días. Siete pacientes recibieron, posteriormente, quimioterapia con gemcitabina + cisplatino. Hubo morbilidad en 2 pacientes, tipo I de Dindo-Clavien. No reportamos mortalidad. El seguimiento promedio fue 7,1 ±5,1 meses, no reportamos recurrencia. Discusión: Esta serie presenta menor número de ganglios resecados que otros estudios (posiblemente por ser nuestra serie inicial) y mayor morbilidad, pero sólo tipo I de Dindo-Clavien. Presentamos una estancia hospitalaria similar a series internacionales y menor presencia de metástasis según reportan análisis retrospectivos. Conclusión: La CELap es una opción terapéutica aceptable y presenta cifras de morbilidad y mortalidad comparables con series nacionales e internacionales.


Aim: Describe results in terms of morbidity and mortality of minimally invasive treatment in patients with gallbladder cancer until subserosal layer. Materials and Method: Case series of patients with gallbladder cancer undergoing CELap at Hospital Regional of Temuco between December 2017 and March 2019. Results: Ten patients were included, the average age was 59,2 ±11 years. Ninety percent female. According to the invasion in gallbladder layers (TNM Classification), 1 patient was T1a (mucosa) with invasion of Rokytansky-Aschoff sinus and 9 patients T2 (subserosa). Two patients had a positive cystic node. The average operating time of CELap was 333 ± 40 minutes. The average number of resected nodes was 4 ± 2,78 and a positive liver bed was found in 1 patient. The TNM classification was 1 patient T1aN0M0, 7 patients T2N0M0 and 2 patients T2N1M0. Mean hospitalization was 5 ± 2,3 days. Seven patients subsequently received chemotherapy with gemcitabine + cisplatin. There was 2 patients with morbidity, type I of Dindo-Clavien scale. No mortality is reported. The average follow-up was 7,1 ±5,11 months and no recurrence was reported. Discussion: This series has a lower number of resected nodes than other studies (possibly because it is our initial series) and higer morbidity, but only Dindo-Clavien type I. Furthermore, we present a hospital stay similar to international series and a lower presence of metastases as reported in retrospective analysis. Conclusion: CELap is an acceptable therapeutic option and presents morbidity and mortality comparable with the national and international series.
