RESUMO

Effectiveness of infrared-terahertz radiation with acupuncture in the initial period of ischemic stroke. This article presents the data of observational retrospective clinical investigation aiming at generalization of the experience of usage of infrared terahertz radiation combined with acupuncture for treatment of the patients in the initial period of ischemic stroke. The experimental group contained 30 patients which had various clinical problems at the initial period of hospitalization. The attempts to dissolve these problems within the acceptable period of time employing domestic and international standard treatment approaches failed. The control group consisted of 31 patients with uncomplicated course of stroke. The patients of the experimental group in addition to the standard treatment received infrared terahertz treatment (device ) combined with acupuncture. Despite the fact that the condition of the patients in the experimental group at admission was much serious than in the control group, at the time of discharge the level of functioning of the first group was definitely higher than the second. In 6 months that superiority increased and has been retained during two years of following up. Despite the fact that the employed in the current investigation method has been used for dissolving different clinical problems, it has been resulted in increase of the general level of functioning proved by the dynamics of such an integral criterion as index Barthel. This means that employment of infrared terahertz radiation in the stroke patients possesses the systemic effect. To recommend the discussed method for treatment of all patients in the initial period of stroke as well as its repeated employment during rehabilitation the separate prospective randomized clinical trials should be undertaken.

RESUMO

Increase of cardiovascular tension is a common thing for professional athletic training. Cardiovascular pathologies can be prevented by permanent physiological monitoring using, among others, the methods of cardiologic monitoring so far available in stationary diagnostic centers. On-line remote diagnostics during training is potent to enhance effectiveness and efficiency of sporting people's health management. In addition, RD will also enable extensive investigations of the bodily responses of individually determined training loads. The paper gives an overview of the current RM technologies.

We examined 41 patients with ischemic heart disease (n=26) and dilated cardiomyopathy (DCM) (n=15). Control group comprised 15 practically healthy subjects. We found no substantial differences in the presence of CD34+CD133- and CD34+CD133-VEGFR-2+ cells in peripheral blood of patients with chronic heart failure (CHF) but noted lowering of level of CD34-CD133+VEGFR-2+ cells mainly in DCM group. Most significant factors determining lowering of number of circulating endothelial precursor cells (EPC) were age, level of low density lipoprotein cholesterol, and intima media thickness of common carotid arteries. We found lowering of colony forming ability of EPC in patients with CHF first of all in the group of patients with DCM. Abnormality of qualitative and quantitative composition of EPC can be considered as one of risk factors of endothelial dysfunction, pathological remodeling of cardiovascular system and CHF progression.

We assessed effect of therapy with HMG--CoA reductase inhibitor rosuvastatin on the number of circulating hemopoietic stem CD34+, CD133+ cells in peripheral blood of patients with systolic left ventricular dysfunction of ischemic genesis. Number of circulating hemopoietic stem CD34+, CD133+ cells in patients with chronic heart failure did not differ from their number in control group, however we revealed tendency to increase of number of endothelial CD34+, CD133+, VEGFR-2+ progenitor cells. We confirmed presence of relationship between quantity of circulating stem and progenitor cells and traditional risk factors of cardiovascular events: age, excessive body mass, arterial hypertension, and intima media thickness of common carotid arteries. Besides lipid lowering effect therapy with rosuvastatin was associated with lowering of C-reactive protein level and increase of number of circulating CD34+, CD133+ cells.

RESUMO

High resolution signal-averaged ECG (SAECG), echocardiography and 24-hour Holter ECG monitoring were used in the study of 30 patients (mean age 56-/+2 years) with coronary heart disease and stable class II-IV angina (group 1) and 66 patients (mean age 45-/+1 years) with dilated cardiomyopathy of ischemic origin (group II). SAECG was used for detection of late ventricular potentials and calculation of time-voltage and velocity parameters of P-waves. Disturbances of myocardial de- and repolarization indicative of pronounced hemodynamic left atrial overload associated with elevated left ventricular end diastolic pressure were revealed. These disturbances were more pronounced in patients of group II. Late ventricular potentials and arrhythmias were more frequent in patients with more severe cardiac failure. Sensitivity of SAECG for detection of arrhythmogenic substrate in the myocardium was 70 and 90% in groups I and II, respectively. Positive predictive power of ventricular late potentials for occurrence of ventricular arrhythmias in these groups was 86 and 91%, respectively.

AIM: To estimate the incidence of late ventricular potentials (LVP) and their implication in cardiac arrhythmia. MATERIALS AND METHODS: 53 patients with blood hypertension stage II (mean age 50 +/- 1 years) having symmetric (n = 23) and asymmetric (n = 15) left ventricular hypertrophy as well as 15 patients free of hypertrophy underwent conventional 12-lead ECG, Holter monitoring, echocardiography and late potentials (LP) recording using signal-averaged high resolution ECG. RESULTS: LP occurred more often in asymmetric myocardial hypertrophy than in symmetric one. There was no strong correlation between LP and ventricular arrhythmias except patients with symmetric myocardial hypertrophy. CONCLUSION: In patients with hypertension stage II late potentials seem to arise due to increased hypertrophic myocardium depolarization time, myocardial fibrosis and local intramural blockades.

A single administration of nitroglycerin, corinfar, anapriline was studied for effects on exercise tolerance in 33 patients with exercise-induced angina with varying adrenergic vasoconstriction during exercise estimated by responses to pratsiol. The effect was found to be related to the degree of functional vasoconstriction. It is concluded that there is a pathogenetic heterogeneity of angina on effort in which in 57% of patients having high coronary reserve, adrenergic vasoconstriction along with organic changes in of great value in the genesis of myocardial ischemia during exercise. In 33% patients with low coronary reserve, the major role is played by organic changes in the coronary bed and vasoconstrictive responses are unassociated with adrenergic exposure. Only in 12% of patients the functional component of coronary vasoconstriction is lacking.

The aim of the present investigation was to evaluate sympathetic vasoconstrictor influence and to define its role in the formation of coronary occlusion in patients with effort angina. The investigation covered 22 patients with Functional Class II-III stable effort angina. To detect vasoconstrictor responses on exercise and to identify their adrenergic component, pair bicycle tests were performed before and after administration of nitroglycerin and pratsiol. Repeated bicycle tests were conducted twice a day: at 10-11 a.m. and at 3-4 p.m. to study spontaneous exercise tolerance variations confirming the presence of dynamic coronary occlusion. The examinations indicated that in 18 patients the exercise resulted in functional major artery narrowing, aggravating the organic stenosis. Activation of smooth muscle alpha-adrenoreceptors is of a definite significance in the genesis of dynamic coronary occlusion in 50% of all the examinees.

RESUMO

Available indirect methods were employed to examine 52 patients with a history of myocardial infarction and 18 normal subjects by the end of the early rehabilitation period. Hemodynamic overload of the left atrium was revealed. The main hemodynamic parameters and processes marking contractile and electrical myocardial functions did not return to normal. The symptoms of left ventricle failure persisted thus requiring treatment continuation.
