RESUMO

As a result of examining 211 persons with severe somatic pathology (the crush syndrome, multiple fractures, spinal cord injuries, and so forth) received during the earthquake, 179 (84.8%) manifested psychopathological disorders. Of these, 70 persons (33.2%) had asthenia, consciousness disturbances, insomnia which were viewed as somatogenic, 17 persons (8.1%) with craniocerebral injury had the signs of the establishment of the psycho-organic syndrome, and in 109 persons (51%), the clinical picture was determined by psychogenically conditioned depressive, anxious-phobic and psychopathic reactions. The data obtained allow one to judge the characteristics of the mental status during the subacute period (from the first to the second week) after the earthquake.

RESUMO

Seventy-six patients with endogenous subdepression were observed and treated by a regional out-patient gerontopsychiatric unit. Roughly a half of those seeing the doctor had subdepression. General principles of therapy are designed for endogenous subdepression in senile patients. The data suggest their efficient treatment with antidepressants and tranquilizers depending on syndromal or nosologic origin of the depression.

RESUMO

Cyclothymia was studied in 129 patients aged over 60 in the gerontology unit of an outpatient clinic in Moscow. Premanifest period of the disease was analyzed clinically to single out the functional and somatic disorders. The typology of the first cyclothymic phases and conditions of their development are described as related to the ages of the disease manifestation under 60 vs over 60. The data evidencing the polar nature of the affective disorders are presented to characterize the natural history of the ailment in senile cyclothymic patients.

RESUMO

A total of 44 patients suffering from slow-progressive schizophrenia with affective disorders prevalence were examined. A long latent stage was defined as Bonn's "masked mania", two variants of the development were singled out in the active period of the disease. The first variant was characterized by depressive disorders and "mixed states" type of a clinical picture. The patients were optimistic, demonstrating high self-estimation in spite of depressive complaints, flaccidity, suppression, weakness, apathy and pseudoneurotic disorders. Accelerated development of associations was retained and motor retardation was absent Personality changes were limited by emotional and psychopathic ones. The second variant represented affective paranoia, delusional disorders formed on the basis of chronic hypomania. Its expressivity correlated with the intensity of hypomanic effect. Personality changes included thought disturbances, autization, mild decrease of the energy potential.

RESUMO

The authors examined 28 patients with chronic hypomanic states that form within the bounds of acquired cyclothymia after a schizophrenic psychosis. Two types of the chronic hypomanias were differentiated. The first was hyperthymia, a residual pseudopsychotic state characterized with a combination of cycloid and schizoid features with an invariably elated mood. The patients were energetic, purposeful, indefatigable, and industrious at work. No signs of disease progress were observed for a long time. The second type was subpsychotic hypomania associated with a slow, but still continuing progress of the disease. The elated mood was combined with marked signs of the defect (emotional and intellectual impoverishment, psychopathy-like changes) and still persisting positive symptoms. The activity and ability to adapt themselves enabled these patients to remain sufficiently compensated socially at the new, lower level of the adaptation. Later the psychopathological disorders got deeper, and their scope increased.
