RESUMO

The aim of the study is to assess the effectiveness of quality control of transport environments in accordance with the requirements of domestic and international standards using aerobic and optional anaerobic microorganisms. The results of a comparative assessment of the survival of 10 strains of aerobic and optional anaerobic microorganisms after storage in the semi-liquid transport environment of Ames for 24 and 48 hours at temperatures (4-8)0 C and (20-25)0 C in accordance with the requirements of domestic and international standards are presented. The methodology described in ICC 4.2.2316 causes a number of technical difficulties leading to non-reproducible results. The survival rate of microorganisms in semi-liquid transport environments under ISO 11133-2016 is of high quality. The quantitative assessment is based on two Swab Elution and Roll Plate methods recommended by the CLSI M40-A2 standard.

The results of the comparative testing of the susceptibility of M. tuberculosis clinical strains to isoniazid, streptomycin, rifampicin and ethambutol using the TB test kit, developed in SCRAMB, (Obolensk) and the absolute concentrations method; the TB test kit and the BACTEC MGIT 960 automated system are presented in the study. A total of 629 and 220 strains, respectively, were tested. A high degree of agreement of the results was shown: 89.1-98.6% for isoniazid, 96.2-98.0% for rifampicin, 91.5-98.2% for streptomycin and 89.1-95.9% for ethambutol. The smallest number of discrepancies in the results was obtained when comparing the TB test kit and BACTEC MGIT 960. The discrepant results analysis was performed by the proportion method, PCR sequencing, or re-testing on new lots of the TB test kit and Lowenstein-Jensen medium with anti-tuberculosis drugs, after which the sensitivity, the specificity and the efficiency of the TB test kit have exceeded 95 % for all anti-tuberculosis drugs. The turnaround time with the TB test kit (median 9.25-9.9 days, ranged from 8 to13 days) was significantly shorter than that with the absolute concentration method (median 21-23 days, ranged from 20 to 28 days) and is commensurate with the turnaround time with BACTEC MGIT 960 (average 7.2 days, ranged from 5 to 12 days). The TB test kit is easy to use, does not require expensive equipment and special staff training.

RESUMO

The national growth mediums were developed for isolating and cultivating of main agents of purulent bacterial meningitis--haemophilus agar, chocolate agar, PBM-agar. The growing and selective characteristics of developed growth mediums are examined. The haemophilus agar ensures growth of Haemophilus influenzae. The chocolate agar, PBM-agar ensure growth of Neisseria meningitidis, Streptococcus pneumoniae and Haemophilus influenzae. By growing characteristics, the national growth mediums match foreign analogues. Under application of growth mediums with selective additions it is possible to achieve selective isolation of main agents of purulent bacterial meningitis with inhibition of growth of microbes-associates.

RESUMO

Studies results about characteristics of growth medium for Legionella cultivation--legionelbacagar--are presented in article. It was shown that the medium has good growth characteristics and prolonged shelf life. Successful use of this medium for cultivation of Legionella pneumophila strains isolated during outbreak in Sverdlovsk region in 2007 was demonstrated. Feasibility of its use for isolation of Legionella from environment and clinical samples was discussed.

Description of the pyrazinamide-resistant clinical strains of M. tuberculosis derived from sputum of patients treated in TB clinics in Tula was made (June, 2001 - July, 2002). It was demonstrated that 30.3% (n = 91) strains were resistant to pyrazinamide. It was found out that these strains were resistant to other antituberculosis drugs in most cases. The method of PCR-sequencing was used to find the mutations in the gene pncA determining resistance to pyrazinamide. 44 different types of mutations localized in 28 codons were detected. The predominance of the mutations in 57 (13.2%), 63 (7.7%), 97 (7.7%), 12 (6.6%), 103 (6.6%) codons and in -11 (6.6%) promoter ofp ncA was observed in the pyrazinamide-resistant strains. Several new mutations determining resistance of the clinical strains of M. tuberculosis to pyrazinamide were described. A high correlation between resistance of mycobacteria to pyrazinamide and activity of pyrazinamidase was observed.

A study was made of the interrelationship of myocardial perfusion with physical working capacity in different types of DM (with the duration of disease from 4 to 8 years without clinical signs of circulatory insufficiency). Rated physical exercise testing in 12 patients with insulin dependent diabetes and 27 patients with noninsulin dependent diabetes as well as in 40 healthy subjects has shown a GTT decrease in diabetes mellitus irrespective of patient's age, sex and body mass to be more marked in noninsulin dependent type and to be closely related to disturbed oxygen supply of the heart and a lowered myocardial reserve. Bicycle ergometric testing combined with 201Tl scintigraphy in 20 patients has shown disorder of perfusion in all the patients irrespective of a diabetes type and duration of disease. Insulin dependent diabetes mellitus was characterized by stable perfusion defects resulting from metabolic derangements, and noninsulin dependent diabetes mellitus was characterized by a decrease in the level of a maximum Tl uptake by the myocardium and transient perfusion defects of ischemic type.

The insulin test was carried out in 68 healthy subjects and 20 patients with moderate thyrotoxicosis at the stage of compensation. Blood levels of pyruvate and lactate, plasma and erythrocytic glucose, urinary catecholamines and gas exchange were measured before, 30 and 120 min after intravenous administration of insulin (0.15 U/kg). All the measurements evidenced hypermetabolism: oxygen consumption, carbonic acid production, pulmonary ventilation were enhanced; lactate-pyruvate index and urine catecholamines were lower versus those of healthy subjects. Correlation analysis demonstrated disturbed regulation of hyperventilation by the sympathetic-adrenal system in thyrotoxicosis.

RESUMO

Ninety-six children aged 3 to 14 years with cerebral paralysis in the form of spastic diplegia of medium and grave intensity were examined. The majority of the children showed disorders of zinc metabolism and of other types of metabolism. To correct metabolic abnormalities, 20 children received zinc sulfate in biological doses per os in addition to the main complex of treatment measures. 38 children suffering from cerebral paralysis made up the control group and were given a complex of routine rehabilitation treatment measures. It has been established that introduction of the biotic doses of zinc sulfate into the complex of therapeutic measures for children with cerebral paralysis in the form of spastic diplegia favoured the improvement of metabolic processes, the clinical health status of the children, and enhancement of the body defence properties.

The results of bicycle ergometry testing were studied in 20 patients with diabetes mellitus of type II without clinical signs of circulatory insufficiency, 21 patients with postinfarction cardiosclerosis with undisturbed carbohydrate tolerance, and in 18 healthy persons. The blood level of glucose in patients with diabetes mellitus was investigated on an empty stomach and 30, 60 and 120 min. after physical exercising. Similar changes of bicycle ergometry results were found in diabetic and CHD patients. In compensation of diabetes in response to graded physical exercise glycemic changes were absent, in decompensation glycemia increased in 30 and 60 min. and returned to the basal level 120 min. after testing. Since the results of bicycle exercises in diabetic (compensated) and CHD patients were the same, the principles of their physical rehabilitation must be identical. In patients with decompensated diabetes mellitus a hyperglycemic reaction to graded physical exercise serves the basis for reduction of motor activity in patients with decompensated diabetes mellitus.

RESUMO

Insulinotropic action of glucose after a glucose tolerance test and fructose after a fructose tolerance test as well as insulinotropic and hypoglycemic effects of diamicron were examined in 21 patients with type II diabetes mellitus. The data obtained indicate that insulinotropic and hyperglycemic activity of fructose yields to that of glucose. Apart from increasing insulin concentration in the blood, diamicron enhances glucose utilization at the periphery.

Glycemia level and diurnal glucosuria were studied in the morning, afternoon and evening in a group of 27 patients with insulin-dependent diabetes mellitus before and after diamicron therapy for 1.5 to 3 months. A similar investigation was performed in a group of 20 patients, suffering from diabetes mellitus of the second type before and following maninil treatment during 1.5 to 2 months. It was shown that diamicron exerts a more pronounced sugar-lowering effect in comparison with that of maninil. It promotes stable glycemia development and should take an appropriate place among hypoglycemics used now in diabetology.

Changes in insulin and growth hormone secretion were studied in 112 children and adolescents (50 healthy ones, 40 with excessive weight, and 22 with heredity aggravated by diabetes mellitus). Three types of these changes were distinguished in healthy adolescents: normoreactive, hyperreactive, and inert. The same type of growth hormone secretion changes corresponded to each type of insulin secretion changes. There was revealed a negative correlation between the insulin and growth hormone levels in the group of healthy adolescents. Dynamic relation was noted between the indices of the mentioned hormones in the course of the test: it was stronger during the ascending than during the descending phase of the test. Glucose load proved to change the correlation between the hormones level. In the group of healthy adolescents correlation between the insulin and the growth hormone levels was greater before carbohydrate load than after it. In case of excessive weight or heredity aggravated by diabetes mellitus glucose load disturbed the correlation between the hormones: when a negative correlation existed between the insulin and growth hormone levels before the load no correlation was revealed after in. This fact confirmed the role played by excessive carbohydrate consumption in the pathogenesis of diabetes mellitus.
