RESUMO

OBJECTIVE: To determine history, clinical and electrocardiographic abnormalities, treatment, and outcome of dogs exposed to toxins produced by the Bufo marinus toad. DESIGN: Retrospective study. ANIMALS: 94 dogs. PROCEDURE: Medical records of dogs examined between July 1997 and July 1998 for which a diagnosis of toad intoxication had been made on the basis of history and physical examination findings were reviewed. RESULTS: Most (66) dogs were treated during the spring and summer. For 54 dogs, exposure to toads had been witnessed. For the remaining 40, toad intoxication was diagnosed on the basis of history and clinical signs. The most common clinical signs were neurologic abnormalities, hyperemic mucous membranes, ptyalism, recumbency or collapse, tachypnea, and vomiting. The oral cavity was lavaged with tap water in all dogs. Fifty-two dogs were hospitalized for treatment. Body weight of dogs hospitalized > 2 hours was significantly less than that of dogs treated as outpatients. The most common electrocardiographic findings were sinus arrhythmia, sinus tachycardia, and normal sinus rhythm. Eighty-nine dogs recovered fully, 4 died, and 1 was euthanatized. CONCLUSIONS AND CLINICAL RELEVANCE: In areas in which B marinus toads are endemic, toad intoxication should be considered in the differential diagnosis for dogs with an acute onset of neurologic abnormalities, hyperemic mucous membranes, and ptyalism, especially during the spring and summer months. The prognosis is good for dogs with toad intoxication that receive appropriate treatment.

Pharyngocutaneous fistula is a significant cause of postoperative morbidity following total laryngectomy. The records of 132 patients at the Johns Hopkins Hospital were reviewed retrospectively to determine the role of radiographic contrast studies in the early postoperative period after total laryngectomy. Radiographic studies were performed in 41 cases, of which 38 were cinepharyngoesophagograms. Fistulae occurred postoperatively in 28 patients (21%). In patients with no clinical signs or symptoms suggestive of an impending fistula (fever, wound erythema, wound swelling, or persistent elevated neck drain output), there is no need to perform a cinepharyngoesophagogram before starting oral alimentation. The presence of soft-tissue air in the neck seen on preliminary scout spot films, suggestive of an impending fistula seen in patients who also underwent a neck dissection, did not correlate with a clinical fistula if no extravasation of contrast was observed.

Recommendations are presented for standardized imaging planes and display conventions for two-dimensional echocardiography in the dog and cat. Three transducer locations ("windows") provide access to consistent imaging planes: the right parasternal location, the left caudal (apical) parasternal location, and the left cranial parasternal location. Recommendations for image display orientations are very similar to those for comparable human cardiac images, with the heart base or cranial aspect of the heart displayed to the examiner's right on the video display. From the right parasternal location, standard views include a long-axis four-chamber view and a long-axis left ventricular outflow view, and short-axis views at the levels of the left ventricular apex, papillary muscles, chordae tendineae, mitral valve, aortic valve, and pulmonary arteries. From the left caudal (apical) location, standard views include long-axis two-chamber and four-chamber views. From the left cranial parasternal location, standard views include a long-axis view of the left ventricular outflow tract and ascending aorta (with variations to image the right atrium and tricuspid valve, and the pulmonary valve and pulmonary artery), and a short-axis view of the aortic root encircled by the right heart. These images are presented by means of idealized line drawings. Adoption of these standards should facilitate consistent performance, recording, teaching, and communicating results of studies obtained by two-dimensional echocardiography.

To study intra-arterial infusion of chemotherapeutic agents, human squamous carcinoma cells were obtained from the FaDu cell line and were implanted in athymic rats (rnu/rnu). The xenografts were grown and then were reimplanted on a vascular pedicle and were completely isolated from the surrounding tissue. The vascular isolation of these pedicles was confirmed radiographically and histologically. After 26 days of growth, an osmotic pump was implanted to deliver either saline, intra-arterial cisplatin, or intravenous cisplatin to the vascular pedicle. Platinum levels were identified in the tumors following intra-arterial and intravenous infusions, demonstrating the usefulness of this model for delivery of antineoplastic agents. We were unable to detect differences between tumor groups for final tumor volume, tumor platinum levels, and tumor histology with the one-dose schedule used. We were able to show a remarkable uniformity of the tumor platinum levels with varying serum platinum levels with intra-arterial and intravenous infusion of cisplatin. The toxicity of each drug administration method was assessed by levels of renal platinum and rat serum creatinine and blood urea nitrogen. No differences in toxicity with the dose administrated. We believe the immunodeficient rat with a xenografted isolated vascular pedicle is an excellent model to study the effect of intra-arterial therapeutic modalities on head and neck squamous cell carcinoma.

M-mode echocardiograms were recorded from 21 adult sheep (20 ewes, 1 whether). Fifteen echocardiographic measurements were taken and compared with body weights or heart rates, using linear regression equations. Significant correlations (P less than 0.05) were found when body weight was compared with left ventricular internal dimensions in systole (Yo = 0.187 Xo + 18.60, P less than 0.05) and diastole (Yo = 0.346 Xo + 26.69, P less than 0.02), septal thickness in systole (Yo = 0.0876 Xo + 7.64, P less than 0.05) and diastole (Yo = 0.0673 Xo + 4.45, P less than 0.05), aortic root dimension (Yo = 0.129 Xo + 23.40, P less than 0.05), and left atrial dimension (Yo = 0.194 Xo + 15.95, P less than 0.005). Heart rate was significantly correlated (P less than 0.05) with body weight (Yo = -0.245 X 96.71, P less than 0.05), ejection time (Yo = -0.0013 Xo + 0.376, P less than 0.001), velocity of circumferential fiber shortening (Yo = 0.0061 Xo + 0.928, P less than 0.05), mean velocity of mitral valve middiastolic closure (Yo = -0.184 Xo + 1.65, P less than 0.02), and left atrial dimension (Yo = -0.109 + 40.55, P less than 0.005). Fractional shortening of the left ventricle was (mean) 37.2 +/- 5.7% and the left atrial to aortic root ratio was (mean) 0.92 +/- 0.10.
