OBJECTIVE: Individuals diagnosed with anorexia nervosa (AN) often report seeing themselves as overweight. While body size estimation tasks suggest that such individuals overestimate their body size, these tasks have failed to establish whether this misestimation stems from visual misperception. Misestimation might, instead, be due to response bias. We designed a paradigm to distinguish between visual and response bias contributions to body size misestimation: the symmetrical body size estimation (s-BSE) paradigm. METHOD: The s-BSE paradigm involves two tasks. In the conventional task, participants estimate the width of their photographed body by adjusting the size of a rectangle to match. In the transposed task, participants adjust the size of a photograph of their body to match the rectangle. If overestimation stems exclusively from visual misperception, then errors in each task would be equal and opposite. Using this paradigm, we compared the performance of women diagnosed with AN (n = 14) against women without any eating disorder (n = 40). RESULTS: In the conventional task, we replicated previous findings indicating that both women with AN and women without any eating disorder overestimate their body size. In the transposed task, neither group adjusted the bodies to be narrower than the rectangle. Participants with AN set their photographs to be significantly wider. DISCUSSION: While we replicated previous findings of body size overestimation amongst women with AN and those without any eating disorder, our results are inconsistent with the hypothesis that such overestimation stems exclusively from visual misperception and instead suggest a substantial response bias effect. PUBLIC SIGNIFICANCE: Women with anorexia nervosa overestimate their own body size. Research has not yet determined whether this overestimation stems from them seeing themselves as larger or other, non-visual factors. We employ a new method for distinguishing these possibilities and find that non-visual factors influence size estimates for women with and without anorexia nervosa. This method can help future research control for non-perceptual influences on participant responses.

Due to a close functional relation between brain areas processing emotion and those processing olfaction, major depression is often accompanied by reduced olfactory function. Such hyposmia can be improved by regular olfactory training (OT) over several months. As this training furthermore improves subjective well-being, we explored whether OT is a useful complementary strategy for depression treatment. A total of 102 depressive outpatients were randomly assigned to OT or a control training condition, which were performed twice a day for 16 weeks. Compliance was continuously monitored. Before and after training we measured depression severity and olfactory function. About half of the patients of both groups did not complete the training. Among the remaining patients, depression severity decreased significantly in both groups. The absence of an interaction effect indicated no selective impact of OT and the variance of depression improvement explained by OT was as little as 0.1%. The low compliance suggests that OT is not feasible for large parts of our sample of depressive outpatients, most likely due to a disease-immanent lack of motivation. In those patients who completed the training, lack of specific effects suggest that OT is not more useful then unspecific activation or attention training. BACKGROUND: Olfactory dysfunction is common in older individuals. The importance of such dysfunction to individuals is highly variable: many people do not seem to care about their olfactory dysfunction, others suffer and complain about problems in daily life, a reduced quality of life, or symptoms of depression. OBJECTIVE: Understanding the importance of olfaction for different age groups in normosmic as well as in smell-disordered subjects may help to find reasons for insufficient coping with olfactory dysfunction. METHODS: We used a questionnaire to capture the individual importance of olfaction in a sample of 433 normosmic and 172 dysosmic people from 15-82 years of age. Furthermore, all participants underwent standardized assessment of olfactory function. RESULTS: The importance of olfaction was highest in the group of young (≤25-year-old) normosmic women. Dysosmia was associated with a decreased importance of olfaction, irrespective of age. However, 18 % of the dysosmic patients showed a tendency to aggravate their symptoms. This high degree of aggravation could not be explained by sex, age, or severity of olfactory dysfunction. CONCLUSIONS: The high importance of olfaction observed in young women may reflect the specific needs of this group, such as mate selection and child care. The strongly reduced importance of olfaction in dysosmic subjects seems to serve as an adaptive coping mechanism. A tendency to aggravate symptoms may indicate insufficient coping. Hence, detecting high aggravation could be a first step to recognizing high psychological strain and the need for psychotherapeutic treatment.

BACKGROUND: In their relentless pursuit of thinness, individuals with anorexia nervosa (AN) engage in maladaptive behaviors (restrictive food choices and overexercising) that may originate in altered decision making and learning. METHODS: In this functional magnetic resonance imaging study, we employed computational modeling to elucidate the neural correlates of feedback learning and value-based decision making in 36 female patients with AN and 36 age-matched healthy volunteers (12-24 years). Participants performed a decision task that required adaptation to changing reward contingencies. Data were analyzed within a hierarchical Gaussian filter model that captures interindividual variability in learning under uncertainty. RESULTS: Behaviorally, patients displayed an increased learning rate specifically after punishments. At the neural level, hemodynamic correlates for the learning rate, expected value, and prediction error did not differ between the groups. However, activity in the posterior medial frontal cortex was elevated in AN following punishment. CONCLUSIONS: Our findings suggest that the neural underpinning of feedback learning is selectively altered for punishment in AN.

BACKGROUND: We have previously shown increased resting-state functional connectivity (rsFC) in the frontoparietal network (FPN) and the default mode network (DMN) in patients with acute anorexia nervosa. Based on these findings we investigated within-network rsFC in patients recovered from anorexia nervosa to examine whether these abnormalities are a state or trait marker of the disease. To extend the understanding of functional connectivity in patients with anorexia nervosa, we also estimated rsFC between large-scale networks. METHODS: Girls and women recovered from anorexia nervosa and pair-wise, age- and sex-matched healthy controls underwent a resting-state fMRI scan. Using independent component analyses (ICA), we isolated the FPN, DMN and salience network. We used standard comparisons as well as a hypothesis-based approach to test the findings of our previous rsFC study in this recovered cohort. Temporal correlations between network time-course pairs were computed to investigate functional network connectivity (FNC). RESULTS: Thirty-one patients recovered from anorexia nervosa and 31 controls participated in our study. Standard group comparisons revealed reduced rsFC between the dorsolateral prefrontal cortex (dlPFC) and the FPN in the recovered group. Using a hypothesis-based approach we extended the previous finding of increased rsFC between the angular gyrus and the FPN in patients recovered from anorexia nervosa. No group differences in FNC were revealed. LIMITATIONS: The study design did not allow us to conclude that the difference found in rsFC constitutes a scar effect of the disease. CONCLUSION: This study suggests that some abnormal rsFC patterns found in patients recovered from anorexia nervosa normalize after long-term weight restoration, while distorted rsFC in the FPN, a network that has been associated with cognitive control, may constitute a trait marker of the disorder.

Network-based analyses of deviant brain function have become extremely popular in psychiatric neuroimaging. Underpinning brain network analyses is the selection of appropriate regions of interest (ROIs). Although ROI selection is fundamental in network analysis, its impact on detecting disease effects remains unclear. We investigated the impact of parcellation choice when comparing results from different studies. We investigated the effects of anatomical (AAL) and literature-based (Dosenbach) parcellation schemes on comparability of group differences in 35 female patients with anorexia nervosa and 35 age- and sex-matched healthy controls. Global and local network properties, including network-based statistics (NBS), were assessed on resting state functional magnetic resonance imaging data obtained at 3T. Parcellation schemes were comparably consistent on global network properties, while NBS and local metrics differed in location, but not metric type. Location of local metric alterations varied for AAL (parietal and cingulate cortices) versus Dosenbach (insula, thalamus) parcellation approaches. However, consistency was observed for the occipital cortex. Patient-specific global network properties can be robustly observed using different parcellation schemes, while graph metrics characterizing impairments of individual nodes vary considerably. Therefore, the impact of parcellation choice on specific group differences varies depending on the level of network organization.
