INTRODUCCIÓN: El tratamiento de la parálisis facial periférica con toxina botulínica es seguro y eficaz. Si bien es conocido su impacto beneficioso sobre la calidad de vida de los pacientes, hasta ahora no se ha estudiado específicamente su percepción subjetiva. Nuestro objetivo ha sido conocer los resultados del tratamiento con toxina botulínica centrándonos en la opinión del paciente. PACIENTES Y MÉTODOS: Estudio prospectivo realizado en una muestra aleatoria de pacientes con secuelas de parálisis facial periférica, que estaban siendo tratados con toxina botulínica en el Servicio de Medicina Física y Rehabilitación de nuestro hospital. Creamos un cuestionario sencillo para evaluar tanto la satisfacción como la percepción subjetiva de mejoría después del tratamiento con toxina botulínica. RESULTADOS: Después de la infiltración, el 95% de los pacientes se sentía bien o muy bien. Más del 80% de ellos notó mejoría respecto a la sensación de tirantez en la mejilla y el cuello. Alrededor de 3/4 partes de los pacientes percibieron mejoría en el rango de movilidad voluntaria y en aproximadamente el 80% mejoraron las sincinesias. Casi todos ellos volverían a tratarse si se les propusiera de nuevo y el 100% recomendaría la infiltración con toxina botulínica a otros pacientes con parálisis facial. CONCLUSIONES: El tratamiento con toxina botulínica proporciona una mejora subjetiva importante en los pacientes con secuelas de parálisis facial periférica, tanto en reposo como respecto al control de sincinesias. El grado de satisfacción es superior a 8/10 en el 99% de los pacientes estudiados

AIM: To parse out the impact of advanced ageing and disuse on skeletal muscle function, we utilized both in vivo and in vitro techniques to comprehensively assess upper- and lower-limb muscle contractile properties in 8 young (YG; 25 ± 6 years) and 8 oldest-old mobile (OM; 87 ± 5 years) and 8 immobile (OI; 88 ± 4 years) women. METHODS: In vivo, maximal voluntary contraction (MVC), electrically evoked resting twitch force (RT), and physiological cross-sectional area (PCSA) of the quadriceps and elbow flexors were assessed. Muscle biopsies of the vastus lateralis and biceps brachii facilitated the in vitro assessment of single fibre-specific tension (Po). RESULTS: In vivo, compared to the young, both the OM and OI exhibited a more pronounced loss of MVC in the lower limb [OM (-60%) and OI (-75%)] than the upper limb (OM = -51%; OI = -47%). Taking into account the reduction in muscle PCSA (OM = -10%; OI = -18%), only evident in the lower limb, by calculating voluntary muscle-specific force, the lower limb of the OI (-40%) was more compromised than the OM (-13%). However, in vivo, RT in both upper and lower limbs (approx. 9.8 N m cm(-2) ) and Po (approx. 123 mN mm(-2) ), assessed in vitro, implies preserved intrinsic contractile function in all muscles of the oldest-old and were well correlated (r = 0.81). CONCLUSION: These findings suggest that in the oldest-old, neither advanced ageing nor disuse, per se, impacts intrinsic skeletal muscle function, as assessed in vitro. However, in vivo, muscle function is attenuated by age and exacerbated by disuse, implicating factors other than skeletal muscle, such as neuromuscular control, in this diminution of function.

Ageing is an inevitable biological process with gradual and spontaneous biochemical and physiological changes and increased susceptibility to diseases. Some nutritional factors (zinc, niacin, selenium) may remodel these changes leading to a possible escaping of diseases, with the consequence of healthy ageing, because they are involved in improving immune functions, metabolic homeostasis and antioxidant defence. Experiments performed "in vitro" (human lymphocytes exposed to endotoxins) and "in vivo" (old mice or young mice with low zinc dietary intake) show that zinc is important for immune efficiency (both innate and adaptive), metabolic homeostasis (energy utilization and hormone turnover) and antioxidant activity (SOD enzyme). Niacin is a precursor of NAD+, the substrate for the activity of DNA repair enzyme PARP-1 and, consequently, may contribute to maintaining genomic stability. Selenium provokes zinc release by Metallothioneins (MT), via reduction of glutathione peroxidase. This fact is crucial in ageing because high MT may be unable to release zinc with subsequent low intracellular free zinc ion availability for immune efficiency, metabolic harmony and antioxidant activity. Taking into account the existence of zinc transporters (ZnT and ZIP family) for cellular zinc efflux and influx, respectively, the association between zinc transporters and MT is crucial in maintaining satisfactory intracellular zinc homeostasis in ageing. Improved immune performance, metabolic homeostasis, antioxidant defence occur in elderly after physiological zinc supplementation, which also induces prolonged survival in old, nude and neonatal thymectomized mice. The association "zinc plus selenium" improves humoral immunity in old subjects after influenza vaccination. The association "zinc plus niacin" in elderly is actually in progress.

Decreased zinc ion availability in ageing is associated with altered immune response. One of the main regulators of zinc availability is metallothionein. Metallothionein induction is under the control of interleukin-6, a pro-inflammatory cytokine whose production is associated with poor ageing. The production of interleukin-6 is controlled, in part, by variability in the -174 nucleotide position. Under conditions of chronic inflammation, such as in ageing, zinc release by metallothionein is limited and may reduce zinc availability. Understanding the precise nature of the interactions between interleukin-6 and metallothioneins will aid in identifying individuals who are at risk of zinc deficiency. In the current study, we used gene arrays to investigate the effects of in vitro zinc supplementation on gene expression in elderly donors with described interleukin-6 and metallothionein 1a polymorphisms. Ingenuity Pathway Analysis identified several zinc-responsive genetic networks uniquely regulated only in elderly individuals with the pro-inflammatory interleukin-6 polymorphism. These include zinc-dependent decreased transcription of pro-inflammatory cytokines and alterations in metabolic regulatory pathways. The genomic effects of zinc increased in significance in the presence of the metallothionein 1a +647 C/A transition, suggesting that the interleukin-6 and metallothionein 1a genes act in a concerted manner to control zinc-regulated gene expression.

Diabetes mellitus is a chronic disease characterized by an overproduction of reactive oxygen species, which perturbs zinc metabolism and promotes the onset of cardiovascular disease (CVD) in diabetic patients. Metallothioneins (MT) are cysteine-rich metal-binding proteins which, by means of their antioxidant and zinc-buffering properties, might prevent the development of diabetic cardiovascular complications. A recent investigation shows that a polymorphism (+647 A/C) in the human MT-1A gene, affects the intracellular zinc ion release (iZnR) from the proteins and is associated with longevity in Italian population. The aim of the present study is to assess the involvement of +647 A/C and +1245 A/G MT1A polymorphisms with the susceptibility to type 2 diabetes (DM2) and cardiovascular complications. The study included 694 old individuals: 242 old healthy controls, 217 DM2 patients without clinical evidence of CVD (DNC) and 235 diabetic patients with diagnosis of CVD (DCVD). +647 A/C MT1A polymorphism, but not the second SNP, was associated with DM2. C allele carriers were more prevalent in DNC and DCVD patients than in control group (OR=1.37, p=0.034; OR=1.54, p=0.002, respectively). C+ carriers was associated with higher glycemia and glycosylated hemoglobin in DCVD patients, but not in DNC or control subjects. No differences in plasma zinc, but a modulation of MT levels and iZnR in PBMCs were observed in DCVD cohort when related to +647 A/C MT1A polymorphism. In summary, this work provides novel evidence on the association of the +647 A/C MT1A polymorphism with DM2. Moreover, C+ carriers in DCVD patients presented a worse glycemic control, a reduced iZnR and a higher MT levels, suggesting a possible role of MT in diabetic cardiovascular complications.

Mild zinc deficiency, which is prevalent in vegetarians, diseased individuals, and the general aging population, depresses immunity and increases risk of disease in later life. However, human zinc intervention trials have produced conflicting results, perhaps because many of these trials included young or zinc-sufficient subjects. Since heterogeneity of the adult population may impact on response to dietary zinc, nutrigenomic approaches aimed at understanding the impact of zinc on modulation of gene and protein activities may aid in identifying subsets of the population-in particular the aging population-with increased risk of zinc deficiency who might receive benefit from a dietary zinc intervention and in this way may influence the success of the intervention. In the current study we used nutrigenomic approaches to investigate the impact of age on zinc-regulated gene expression in peripheral blood mononuclear cells. Ingenuity Pathway Analysis (Ingenuity Systems, Redwood City, CA) identified several genetic networks and functional canonical pathways which appeared responsive to zinc that were differentially regulated in young and elderly individuals. These include tryptophan metabolism, eicosanoid signaling, p38 mitogen-activated protein kinase (MAPK) signaling, integrin signaling, purine metabolism, G-protein-coupled receptor signaling, and most significantly, peroxisome proliferator-activated receptor (PPAR) signaling. These data suggest that age impacts strongly on the transcriptional effects of zinc and provides evidence to support the hypothesis that young and elderly individuals may respond differentially to zinc intervention.

Abnormal increments of pro-inflammatory cytokines (IL-6 and TNF-alpha) characterize the outbreak of infectious diseases, which are the major cause of death in the elderly. A counterbalance to the inflammation is exerted by IL-10 with an inhibitory role on TNF-alpha production. As is well known, some cytokine gene polymorphisms influence the cytokine production, playing a role as susceptibility or resistance factors against immune-mediated and infectious disease. Genetic variations in the -308A/G locus for TNF-alpha seems to affect the clinical outcome of some infectious diseases. In fact, the -308A allele is associated with severe septic shock and death. On this basis, we have screened healthy old subjects, nonagenarians and old patients affected by the acute phase of chronic obstructive bronchitis and bronchopneumonia of bacteria origin for the -308G/A locus (PCR-RFLP). Subjects are grouped in A+ (AG, AA genotypes) and A- (GG genotype) and data on IL-6, TNF-alpha, IL-10, NK cell cytotoxicity, zinc and metallothioneins (MTs) gene expression (RT-PCR) were stratified according to different TNF-alpha genotypes. The frequency of the A allele was increased in infected patients in comparison with healthy old controls. No differences existed between A+ and A- young adult, old and nonagenarian controls in tested parameters. Conversely, A+-infected patients displayed elevated IL-6, TNF-alpha and MTmRNA, low IL-10 coupled with impaired NK cell cytotoxicity and lower zinc ion than A- patients. However, the data reported are gender independent. Therefore, the -308A polymorphism at the locus of TNF-alpha may be one of the susceptibility factor for infectious diseases in old persons, particularly considering its association to the increased release of pro-inflammatory cytokines and to the reduction of zinc release and MTs synthesis involved in the control of the inflammatory response. These data strongly suggest that the genetic screening of the -308G/A polymorphism may be a valid tool for identification of subjects needing a more appropriate therapy when affected by acute and/or recurrent infectious diseases.

An examination of the naso-frontal angle shows that a cranial or caudal shift in position affects the dorsum length. The fundamental aspect characterizing the naso-frontal angle is its width: it may be normal, too wide, or too narrow. It's easier to modify a too-narrow angle than a too-wide one. We show surgical techniques, emphasizing their method applied to the over-wide angle. The second "idèe fixe" is the morphology of the alar lateral crus. Every variety of this structure has a peculiar property of shape and relationship with other elements that predetermines the therapeutic action. Diagrams and photographs are shown to describe the surgical techniques and the clinical results. There is also a demonstration of the method for alar lateral crus resection, which we call "the long keel shape," since it resembles the shape of a sailboat keel.

Displacement of the lateral crus of the alar cartilage of the nose in cleft lip is demonstrable in diagrams showing the pathological anatomy of this deformity. An account is given of our method used for several years, whereby the lateral limb is returned to a more natural position, alar convexity is restored, and the angle between the limbs is corrected. Two flaps taken from the inside surface of the nostril are alternated. The caudal one is a full-thickness flap composed of vestibular skin, while the cranial flap comprises the cartilage and the vestibular mucosa; the lateral limb is left attached to the mucosa. Alternation does not require any interruption of cartilage continuity, so there is no risk of surgical damage to the perichondrium and cartilage in young subjects. The two limbs (particularly the lateral one) are positioned in such a way that their normal growth can be expected, as shown by the aesthetic results on follow-up after a period of many years. This method is a primary reconstruction technique of the ala nasi, applied at the same time as the lip repair and compatible with current methods.

RESUMO

A surgical method for treatment of the extremely hypoplastic tuberous breast is described. It is based on turning differently shaped glandular flaps to correct the deformity, followed by insertion of a prosthesis, and, where necessary, correction of areola size and position and adjustment of the skin of the inferior pole. The results so far have afforded total and recurrence-free aesthetic correction of the usual deformities. The underlying aim of the operation is to transform a hypoplastic tuberous breast into a simple hypoplasia without discarding gland tissue which can be used to thicken the most deficient mammary area.

The naso-frontal angle is one of the features that most markedly determines the morphology of the human face. Two main morphological elements must be taken into consideration: the slope of the forehead and the line of nasal dorsum. For a balanced physiognomy, nevertheless, it is necessary to achieve a correct interaction of the length and width of nasal pyramid, the naso-labial angle, and the chin. Closer examination of the naso-frontal angle shows that a cranial or caudal shift in its position affects the length of the dorsum; however, the fundamental aspect characterizing the naso-frontal angle is its width: it may be normal, too wide, or too narrow. Turning to surgical correction, the authors state that it is easier to modify an over-narrow angle, than one that is too wide. The authors show the single surgical techniques adopted for the correction of the angle, emphasizing the personal method applied to the overwide angle.

The author emphasizes that conservative rhinoplasty techniques are frequently satisfactory, but in certain techniques there is a limit to which nasal tips can be reshaped. In certain circumstances, therefore, interruption of the "domes" of the alar cartilages is suggested to achieve the most satisfactory aesthetic results.

The authors report a psychological equiry carried out by interviews and tests on mastectomised patients to find out the psycho-effective and behavioural implications related to this type of mutilation, to the type of preoperative information and to the relation with the doctor in charge, also to an eventual morphological reconstruction of the breast. They also analyse the relation between the objective result evaluated by the surgeon and the subjective evaluation of the patient. Some results of breast reconstruction are presented.

A brief account of the clinical and histological features of keloids, their differentiation from hypertrophic scars, their etiopathogenesis, and the various methods for their treatment is followed by an assessment of the possibilities offered by cryotherapy and its advantages and drawbacks. Reference is made by way of illustration to 4 patients chosen from a series of more than 100 on the basis of the importance of their clinical pictures. The conclusion is drawn that cryotherapy with nitrogen protoxide is both useful and practical in the management of carefully selected cases and deserves fuller understanding and more widespread employment than it enjoys at present.

RESUMO

Current anthropometric measurements establish the nasofrontal angle. However, it is unwise to establish the lines and equilibrium of volumes of the face within fixed geometrical parameters. Even the data given by various forms of rhinometers do no seem to be suitable since they do not take into account the inclination and curvature of the forehead. The authors thus prefer to study the individual to be treated within a program of harmonization of the three areas of the face, the upper, middle, and lower thirds. The results of treatment utilizing this concept are depicted.
