RESUMO

OBJECTIVE: To estimate the influence the dysplastic phenotype on the stomatological status in the persons living in the different climatic and geographical conditions (the south of Tyumen region, Khanty-Mansiysk and Yamalo-Nenets autonomous districts). MATERIAL AND METHODS: The cross-sectional and observational study of 578 male and female adolescents aged 13.65±1.7 was carried out. The level of oral hygiene, caries intensity and spreading and parodontium inflammatory diseases were estimated. All examined persons were divided into 2 groups depending on the presence of signs of connective tissue dysplasia (CTD). RESULTS: The significant spreading of undifferentiated forms of CTD was determined. It was 53.05% on the territory of the south of Tyumen region, 63.7% was in Khanty-Mansiysk district, 64.4% was in Yamalo-Nenets district (p=0.010). The involvement of the dento-maxillary system in the process was observed in 83.1% of adolescents with CTD. Caries spreading and intensity is significantly higher in the group of adolescents with CTD. The differences are statistically significant in all studied climatic and geographical zones. The higher spreading of signs of parodontium inflammatory diseases in the presence of CTD is registered. Spreading of parodontium inflammatory diseases among adolescents with CTD is statistically higher in Khanty-Mansiysk and Yamalo-Nenets autonomous districts than in the south of Tyumen region (p=0.004 and p<0.0001 respectively). CONCLUSION: In the circumpolar region the share of persons with CTD and dysplastic depended changes of dento-maxillary system is statistically higher than on the territory of moderate latitudes. In the presence of CTD caries spreading and parodontium inflammatory diseases increases considerably but in the circumpolar zone the changes are significantly marked. The further study of the role of some factors including confounding in the dysplastic phenotype forming and stomatological pathology in the different climatic and geographical zone conditions is required.

OBJECTIVE: To determine the level of adherence to treatment in patients applying to dental organizations of different forms of ownership. MATERIAL AND METHODS: 1.067 patients of both sexes and various ages who applied to public and private dental organizations of Moscow, Omsk, and Tyumen have been surveyed using the Questionnaire for Quantitative Assessment of Adherence to Treatment (KOP-25), which allows to obtain a quantitative evaluation of the level of adherence to drug treatment, medical support, lifestyle modification, and characterize the level of integral adherence to treatment in patients. Basing on a point rating system of assessment of the responses of the interviewed patients, integrative and structural indicators have been calculated using special formulas. The level of up to 50% has been interpreted as «low¼, from 51 to 75% as «medium¼, over 75% as «high¼. RESULTS: Patients applying to state and privately owned dental organizations have an insufficient level of adherence to treatment (50% medium and 37.4% low), 12.6% of the respondents show high adherence to treatment; adherence to treatment is higher in women, with the highest shown in the group of over 60 years old, independently of the form of ownership of the medical organizations; the interviewed men of all age groups are more inclined to apply for treatment to private medical organizations; all interviewed patients are the least ready for lifestyle modification, women are the most prone to using medication, and the least inclined to treatment are young patients interviewed when applying to public medical organizations. CONCLUSION: Individual treatment adherence assessment allows creating optimal treatment plans with treatment volume most acceptable for patients.

RESUMO

The study was aimed to conduct a morphological assessment of the influence exerted by a certain method of activation of endodontic rinsing liquid on the root dentin structure and canal cleaning effectiveness in an ex vivo experiment. The study was performed on 20 single-rooted teeth removed due to complicated caries in people aged 25-44 years. A devised method of vacuum-jet irrigation was used to carry out the final antiseptic treatment and to activate a rinsing liquid in the group No. 1 (n=10), and an endodontic syringe with ultrasonic activation was used in the group No. 2 (n=10). Optical and scanning electron microscopy data were used to assess dentin histostructure, presence or absence of organic inclusions in the root canal lumens, quality of preparation and contour lines of their sides. An analysis of scans demonstrates that the vacuum-jet irrigation method effectively cleans the root canal lumen in the apical area in 90% of the cases, while irrigation with an endodontic syringe and ultrasound activation has been effective in 67% of the cases. The smear layer throughout the root canals in the samples of the group No. 1 was removed in 100% of the cases. At the same time, the smear layer in the canal apical area was not completely removed in either of the samples of the group No. 2. An analysis of photomicrographs of histotopographic cross sections of the dental roots in group No. 2 under consideration revealed numerous cavities in the circumpulpal dentin, which form nidi of destruction up to 10-15 mcr in diameter merging in the cervical and middle third of the root. At the same time, a typical structure of the circumpulpal dentin with a well-developed network of dentinal tubules and intact peritubular and intertubular matrix was observed throughout the dental roots in the samples of the group No. 1. These investigational studies allow us to conclude that vacuum-jet irrigation of root canals provides better cleaning, completely removes the smear layer and does not damage the root dentin structure.
