RESUMO

The antioxidative system (AOS) ceruloplasmin-transferrin (Cp-Tr) was studied by means of electron paramagnetic resonance in 14 rabbits with experimental atherosclerosis and in 33 patients with ischemic heart disease (IHD). A correlation was found between the AOS Cp-Tr activity and the pathological process severity: mild disease was associated with high AOS activity, while in severe disease course, this activity was threefold lower. This regularity was detectable both in experimental animals and in human IHD patients. It was found that hemosorption (HS) exerted a positive effect only in the presence of low AOS Cp-Tr activity which increased after HS in these cases. In high AOS activity HS caused deterioration of the patients' condition and accumulation of lipid peroxidation products in the blood plasma; this was attended with lowering of the AOS Cp-Tr activity.

There were synthesized new hemosorbents for selective removal of LDL, based on macroporous silica, immobilised by heparine (sorbent 1) and chytozane-sulphate (sorbent 2). Experimental data showed satisfactory hemo- and biocompatibility of the new sorbents. No significant alterations in biochemical parameters were observed. Sorption cholesterol (Ch) capacity of the sorbents was equal and accounted for 15 mg of Ch per 1 g of the sorbent. Pilot clinical studies in familial hypercholesterolemia (FH) patients have shown the satisfactory hemo- and biocompatibility of the sorbent 1, and its significant Ch withdrawal. We conclude that the hemosorbents can be used in the FH patients treatment.

RESUMO

Activity of lecithine-cholesterol acyltransferase (LCAT) was studied in blood of 26 patients with atherosclerosis of various localization after course of treatment involving three-fold hemosorption. The initial activity of the enzyme, decreased distinctly as compared with normal state, was dissimilarly altered during hemosorption. The system LCAT-HDLP was stabilized after the third hemosorption. These results relevant to programming hemosorption therapy of patients with atherosclerosis.

RESUMO

The inclusion of hemosorption through activated coals into the comprehensive treatment of 580 patients sick with atherosclerosis has made it possible to achieve a positive clinical effect among 81 per cent of the patients sick with the ischemic heart disease, 79 per cent of atherosclerotic discirculatory encephalopathy and 76 per cent of obliterating atherosclerosis of lower limbs. The positive dynamics is especially visible among patients who are administered the programmatic hemosorption, making it possible over a long period of time (3-5 years) to sustain remission in the clinical state and the normal level of the lipids in the blood. The direct effect is expressed in the improved microcirculation, disappearance of the sludge-syndrome and the reduction of ischemic disorders. The remote effect of hemosorption is connected with the repeated removals of cholesterin of the atherogenic lipoproteids, which leads, we believe, to the interruption or even regression of the atherosclerotic process.

RESUMO

Eighty-three patients were treated with hemosorption and essential phospholipids for coronary heart disease (CHD) and followed up for 3-5 years. The new approach to the treatment of CHD involves simultaneous removal of cholesterol-rich atherogenic lipoproteins by hemosorption and administration of essential phospholipids to increase cholesterol withdrawal from the cells. This combined treatment of coronary patients is shown to normalize cholesterol metabolism, improve microcirculation, increase stress tolerance and bring about an obvious clinical remission. The good clinical effect is corroborated by the results of bicycle ergometry and data on lipid spectrum, cholesterol metabolism, microcirculation and radio-isotope studies.

RESUMO

An affine hemosorbent, a product of covalent heparin immobilization on a solid matrix, has been proposed for the extraction of atherogenic lipoproteins. The sorbent has high capacity with respect to atherogenic lipoproteins and is selective with other blood constituents. Experimental hemosorption in animals demonstrated rapid sorbent regeneration and satisfactory hemo- and biocompatibility, therefore the new sorbent can be recommended for clinical application.
