RESUMO

The paper shows the value of radionuclide cysternomyelography in the diagnosis of internal hydrocephalus concurrent with basal liquorrhea. The authors examined 1522 patients of whom 105 were found to have the pathology, this yielded radionuclide differential diagnostic criteria for this nosological entity. They proposed new technological conditions for enhancing its diagnostic value.

RESUMO

Radionuclide cisterno-myelography was conducted in 155 patients in the posttraumatic period. Five variants of the radiological picture of liquordynamic disorders were distinguished. The signs of open internal hydrocephalus were determined. The authors note the frequency of the liquor path radiological sign--registration of radiopharmaceutical beyond the liquor channels, which points to the presence of nasal or auricular liquorrhea. Various changes of liquor circulation and resorption were revealed in posttraumatic periods. Disorders of resorption were manifested either by its acceleration or by its reduction or by sharp deceleration. Children most frequently had a radiological picture of open internal hydrocephalus. Signs of liquorrhea were a frequent component of a posttraumatic state in children. Different variants of the radiological picture were revealed in adults in the posttraumatic period: diminished draining function of the cerebral submeningeal space with compensatory drainage of the liquor in the form of nasal liquorrhea was a constant finding. Signs of open hydrocephalus were a commonly encountered component of the pathological picture of posttraumatic state in adults.

RESUMO

An analysis of the radiological signs is presented proceeding from the results of examination of the visualization of the CSF tract in 123 patients in whom radionuclide cisternography with 99mTc-DTPA was performed 1,3,6 and 24 h after administration of a radiopharmaceutical. The radiological appearance of pathology of the CSF system in a long-term period after penetrating craniocerebral wound was developed.

For purposes of defining the adequacy of using REG registrations in clinical practice, the authors conducted 4 series of studies on normals (16 cases), in patients of a neurosurgical ward (39 cases) and in experimental animals (12 dogs). The experimental and clinical studies displayed that the mechanical pulse fluctuation of the skin was not the determining factor in the genesis of a REG. The authors have come to the conclusion that the time of distribution of the pulse wave by itself can not serve as an indicator of an extra-or intracranial origination of REG waves inasmuch as it is different in different brain areas and depending upon its remoteness from the heart and the state of the vascular tone. Using the occlusional cuff the authors calculated the portion of the extracranial item in the total amount of the wave amplitude in the REG in norms (in average 14%). Thus, the achieved data confirmed the adequacy of the REG as a method of studying the pulse blood flux and cerebral vascular tone.
