RESUMO

ABSTRACT: The term rickets refers to insufficient or retarded mineralization of the osteoide matrix. X-linked hypophosphatemic (XLH) rickets is a rare genetic disorder characterized by biochemical changes in bone mineralization due to inactivation of the phosphate regulating gene and primary defect of the osteoblasts. The aim of this article was to report a clinical case of XLH, its oral manifestations, periapical changes and dental management. A 31-year old woman female patient was referred to the school of dentistry with pain and sensitivity in the teeth. She had a childhood history of rickets, hypophosphatemia and alteration in Vitamin D. In the oral exam, enamel hypoplasia, microdontia, fistula, caries and periapical lesions and periodontal disease were diagnosed. The radiographic and tomographic exams exhibited the presence of periapical lesions involving various teeth with radiolucent images, suggestive of granuloma or periapical cysts. The treatme nt prioritized the urgency of eliminating pain and removing the foci of infection. Endodontic treatment began in the teeth that had fistula or periapical lesions and in parallel, oral hygiene guidance was provided and periodontal treatment was performed. There was an improvement in the clinical condition with reduction in inflammation and mobility of the teeth. Dentists and health professionals must evaluate the patient as a whole, considering the relations between systemic and oral health. Knowledge of systemic diseases associated with rickets and their characteristics is essential for making a correct oral diagnosis and planning the dental treatment.


RESUMEN: El término raquitismo se refiere a la mineralización insuficiente o retardada de la matriz osteoide. El raquitismo hipofosfatémico ligado al cromosoma X (XLH) es un trastorno genético caracterizado por cambios bioquímicos en la mineralización ósea debido a la inactivación del gen regulador del fosfato y al defecto primario de los osteoblastos. El objetivo de este artículo fue reportar un caso clínico de XLH, sus manifestaciones orales, cambios periapicales y manejo dental. La paciente, una mujer de 31 años, acudió a la Clínica de Semiología de la UFPR con dolor y sensibilidad en varios dientes. Tenía antecedentes infantiles de raquitismo, hipofosfatemia y alteración de la vitamina D. En el examen oral se diagnosticó hipoplasia del esmalte, microdoontia, fístula, caries y lesiones periapicales y enfermedad periodontal. Los exámenes radiográficos y tomográficos mostraron la presencia de lesiones periapicales en varios dientes con imágenes radiolúcidas, sugestivas de granuloma o quistes periapicales. El tratamiento priorizó la urgencia de eliminar el dolor y remover los focos de infección. Se inició tratamiento de endodoncia en los dientes que presentaban fístula o lesiones periapicales y paralelamente se brindó orientación de higiene oral y se realizó tratamiento periodontal. Hubo una mejoría en la condición clínica con reducción de la inflamación y movilidad de los dientes. Los odontólogos y profesionales de la salud deben evaluar al paciente como un todo, teniendo en cuenta las relaciones entre salud sistémica y oral. El conocimiento de las enfermedades sistémicas asociadas al raquitismo y sus características es fundamental para realizar un correcto diagnóstico oral y planificar el tratamiento odontológico.

RESUMO Introdução: O uso de próteses totais tem impacto na qualidade de vida relacionada à saúde bucal e os idosos são uma parcela da população a ser pesquisada sob essa perspectiva. Objetivo: Avaliar a autopercepção do impacto do uso de próteses totais na qualidade de vida relacionada à saúde bucal de idosos institucionalizados. Métodos: Participaram 20 idosos do Lar dos Idosos Recanto do Tarumã, Curitiba, Paraná, Brasil. Foram avaliados aspectos demográficos, econômicos, de hábitos e clínicos, juntamente com aplicação do questionário Geriatric Oral Health Assessment Index. Os dados foram submetidos à análise descritiva e bivariada (Teste do Qui-Quadrado), com significância de 5 porcento. Resultados: A média de idade foi de 75,2 anos (DP= 8,8) e o tempo médio de uso de prótese foi de 27,9 anos (DP= 18,5). Renda inferior a um salário mínimo foi indicada por 80,0 porcento, e visitas ao cirurgião-dentista pela última vez há mais de um ano, por 70,0 porcento. Os piores relatos foram referentes à mastigação, representados por problemas para mastigar alimentos e desconforto ao comer, com 25 porcento de respostas positivas em cada questão; a soma das frequências das respostas "às vezes" e "sempre" quanto à insatisfação ou à infelicidade com a aparência da boca foi reportada 60 porcento dos pesquisados. A retenção insatisfatória da prótese inferior se deu em 50,0 porcento casos, enquanto os problemas com oclusão estiveram presentes também em metade da amostra. Os defeitos estiveram presentes em 70,0 porcento das próteses superiores e 45,0 porcento das inferiores. Não foi encontrada associação significativa (p> 0,05) entre as variáveis independentes e o desfecho. Conclusões: Os idosos avaliados, independentemente das condições das próteses totais, relataram qualidade de vida relacionada à saúde bucal satisfatória(AU)


RESUMEN Introducción: El uso de prótesis totales tiene un impacto en la calidad de vida relacionada con la salud bucal, y los ancianos son parte de la población que se investigará desde esta perspectiva. Objetivo: Evaluar la autopercepción del impacto del uso de prótesis totales en la calidad de vida relacionada con la salud bucal de ancianos institucionalizados. Métodos: Participaron 20 ancianos de "Lar dos Idosos Recanto do Tarumã", Curitiba, Paraná, Brasil. Los aspectos demográficos, económicos, de hábitos y clínicos se evaluaron junto con la aplicación del cuestionario Geriatric Oral Health Assessment Index. Los datos fueron sometidos a análisis descriptivo y bivariado (prueba de chi cuadrado), con significación del 5 por ciento. Resultados: La edad media fue de 75,2 años (DE= 8,8) y el tiempo medio de uso de la prótesis fue de 27,9 años (DE= 18,5). Los ingresos menores a un salario mínimo se indicaron en el 80,0 por ciento, y las visitas al cirujano dental la última vez, hace más de un año, en el 70, 0 por ciento. Los peores informes estaban relacionados con la masticación, representados por problemas para masticar los alimentos y la incomodidad al comer, con el 25 por ciento de respuestas positivas en cada pregunta. La suma de las frecuencias de las respuestas "a veces" y "siempre" sobre la insatisfacción o la infelicidad con la apariencia de la boca se informó en el 60 por ciento de los encuestados. La retención insatisfactoria de la prótesis inferior se produjo en el 50 por ciento de los casos, mientras que la oclusión también presentó problemas en la mitad de la muestra. Los defectos estuvieron presentes en el 70,0 por ciento de las prótesis superiores y en el 45,0 por ciento de las inferiores. ABSTRACT Introduction: The use of complete dentures has an impact on oral health-related quality of life. Elderly people are the population sector to be researched into from this perspective. Objective: Evaluate the self-perception of the impact of the use of complete dentures on the oral health-related quality of life of institutionalized elderly people. Methods: The study sample was 20 elderly people from Lar dos Idosos Recanto do Tarumã, Curitiba, Paraná, Brazil. Evaluation of demographic and economic details, habits and clinical aspects was based on the information collected with the Geriatric Oral Health Assessment Index questionnaire. The data were subjected to descriptive and bivariate analysis (Chi-squared test), with a significance of 5 percent. Results: Mean age was 75.2 (SD = 8.8) years and mean time of denture use was 27.9 (SD = 18.5) years. Income was below minimum wage in 80.0 percent of the participants, whereas the last visit to the dentist had occurred more than a year ago in 70.0 percent. The worst reports had to do with chewing, represented by problems to chew food and discomfort when eating, with 25 percent positive answers to each question. The sum of the frequencies of the answers "sometimes" and "always" about dissatisfaction with or unhappiness about the appearance of the mouth was reported by 60 percent of the respondents. Unsatisfactory retention of the lower denture occurred in 50.0 percent of the cases, whereas occlusion problems were present in half of the sample. Defects were present in 70.0 percent of the upper dentures and 45.0 percent of the lower dentures. No significant association (p> 0.05) was found between independent variables and outcome. Conclusions: The elderly people evaluated, regardless of the conditions of their complete dentures, reported satisfactory oral health-related quality of life(AU)

AIMS: To analyze and compare the electromyographic activity of the temporal (anterior portion) and masseter muscles among Karate-Do athletes before and after training, with and without the use of a mouthguard. METHODS: Twenty athletes (14 males and 6 females) with a mean age of 23.7 ± 7.5 years participated. They had surface electromyography recordings taken of their bilateral temporal and masseter muscles before and after training under the following conditions: no mouthguard, with a ready-made mouthguard, and with a custom-made mouthguard. Activity was examined at mandibular rest, while clenching, and at maximum voluntary contraction. The data were normalized using the mean maximum voluntary contraction. RESULTS: The right (p = 0.005) and left (p = 0.015) temporal muscles showed significantly lower electromyographic activity with a custom-made mouthguard compared with no mouthguard after training while clenching. The electromyographic activity of the temporal and masseter muscles did not show significant differences when tested at mandibular rest and while clenching before or after training with a custom-made mouthguard (p > 0.05). CONCLUSION: The use of a custom-made mouthguard preserved participants' electromyographic profiles before and after training; thus, they allow for stable muscle activity during the training of Karate-Do athletes.

OBJECTIVE: The aim of this study was to evaluate the perception of taste in HIV-infected patients. METHOD: One hundred males and females (11 to 60 years old) were divided into two groups (50 patients infected by HIV and 50 controls) and evaluated for gustatory function. RESULTS: The results revealed that the mean score in the evaluation of taste was significantly lower in individuals with HIV when compared to controls for both sides of the tongue (p < 0.05). Patients with HIV infection had difficulty recognizing the bitter taste, followed by salty and sweet. When each side of the tongue was evaluated separately and compared, the Wilcoxon test showed that there was no significant difference on the tongue of individuals with HIV. The prevalence of hypogeusia was 20% in individuals with this disease. CONCLUSIONS: Individuals with HIV infection may have a deficit in taste that can affect your general and oral health.

Completely and partially edentulous patients with Sjogren's syndrome (SS) experience severe hyposalivation, xerostomia, and considerable difficulty in using tissue-supported prosthesis. This clinical paper describes the management, treatment, and 6-year follow-up of a patient diagnosed with SS type II, who uses corticosteroids and antihyperglycemic drugs. The patient received restorative, periodontal, and surgical treatments followed by implant-supported fixed prosthesis. Radiographic evaluation and probing depth showed gingival health and no bone loss after 6 years. Treatment with implant-retained dental prosthesis greatly increased comfort and function, offering an alternative to patients with SS.

RESUMO

Oral focal mucinosis is an uncommon clinicopathologic condition that is considered the oral counterpart of cutaneous focal mucinosis. It is a disease of unknown etiology where the connective tissue undergoes a focal myxoid degeneration. A literature review disclosed 47 published cases of oral focal mucinosis. An additional case is presented; the clinical and histologic differential diagnosis, sex, age, location, treatment, and recurrence are discussed.

Celiac disease is a chronic intestinal disease caused by intolerance to gluten associated with poor digestion and absorption of the majority of nutrients and vitamins, which may affect both developing dentition and oral mucosa. The aim of this study is to make a brief review of the literature about celiac disease and to report on a clinical case, showing the impact it may have on the general and oral health. A 39-year-old woman reported the presence of diarrhea, nauseas, flatulence, colic, difficulty with falling asleep, lack of appetite and loose of 18 Kg in the last two years. She also noted the appearance of symptomatic lesions in the mouth. These lesions had a mean duration of a month and occurred in any region of the oral mucosa, particularly on the tongue. Multiples ulcerations were found in thesublingual caruncula region, at the vestibular fornix and at the lingual apex. Topical treatment was instituted for the oral lesions with immediately relief of the symptoms. The diagnosis of celiac disease was established by means of medical clinical exam, biopsy of the small intestine, and by the presence of specific antibodies in the blood. The patient has been instructed to follow a gluten-free diet. Multiprofessional action with the involvement of a gastroenterologist and other health professionals, such as dentists, is important for diagnosing the disease and guiding the patient withceliac disease to achieve a good quality of life
