RESUMO
Degenerative mitral valve disease nowadays is the most common cause of mitral insufficiency in developed countries and is associated with high morbidity and mortality. In the last decades repairing the mitral valve has become the operation of choice for treatment of the mitral valve prolapse, enabling to improve the geometry and function of the left ventricle and long-term survival. Nevertheless, the problem of choice of method of management of severe mitral regurgitation in asymptomatic patients with degenerative mitral valve disease remains unsolved. In this article we present immediate results of a prospective comparative study of mitral valve surgery in asymptomatic and symptomatic patients in dependence on NYHA class of heart failure.

The article discusses the results of reconstructive valve-preserving operations in mitral insufficiency performed under conditions of hypothermic (27-25 degrees C) protection without perfusion in 114 patients. Depending on the valve morphological changes multicomponent intervention was under taken contemplating the performance of commissurotomy, chorda fenestration, resection and sewing of the cusps, removal of fibrous deposits from the cusps, resection of peripheral and intermediate chordae. The discrepancy between the cusp area and the perimeter of the fibrous ring was corrected in 99 patients by annuloplasty with sutures. The hospital mortality was 6.1%. Six-year postoperative survival was recorded in 91% of cases; no thromboembolic complications encountered in 97.4% of patients. The condition improved in most patients and they moved to NYHA Functional Class I-II. Unfavourable results were due to a severe initial clinical and functional condition and to the operations conducted in the active stage of septic endocarditis.

Using neutron activation analysis, the concentration of 13 elements was measured in functionally different bones of mice: weight-bearing and dynamic femurs, essentially stationary temporal bones, and newly formed ectopis bones under the renal capsule developed at the site of implantation of syngenic bone marrow. It was found that the elementary composition of the above bones was different in the norm and that it varied in the course of 3-week hypokinesia produced by small-size cages. It was shown that during hypokinesia the elementary composition changed according to four different patterns. Progressive demineralization due to Ca, P, Mg and Fe losses was accompanied by compensatory incorporation of Sr. In the course of the hypokinetic stress the content of trace elements (Zn, Cr, Ru, Rb, Br, Co, Sb) varied significantly. The mechanisms of action of these elements was related to the mobilization of activating elements at different stages of the general adaptation syndrome.

The first results of the radiobiological studies obtained in a biophysical channel of the IBR-2 reactor at the Joint Institute for Nuclear Research are presented in this report. A study was made of the DNA-membrane complex sedimentation after exposure of Chinese hamster cells V79-4 to gamma-rays within a wide range of doses. The assumption, that had earlier been forwarded by the authors, of the role of double-strand DNA breaks in the alteration of a relative rate of the complex sedimentation upon irradiation of cells with doses above 50 Gy was confirmed.
