RESUMO

CASO CLÍNICO: Varón de 36 años consulta tras exposición bilateral a puntero láser el día anterior. En funduscopia se evidenciaron alteraciones pigmentarias maculares dispersas y en tomografía de coherencia óptica, desestructuración de las capas retinianas. La agudeza visual fue de contaje de dedos en ambos ojos. Nueve meses después se evidenció en la angiografía mediante tomografía de coherencia óptica una alteración del flujo vascular coriocapilar en la zona lesionada por el láser. Discusión: El láser afecta principalmente al epitelio pigmentario retiniano. Sin embargo, con este caso ponemos de manifiesto la posible afectación más allá del epitelio pigmentario retiniano, apareciendo alteración vascular coriocapilar tras la exposición a puntero láser


CASE REPORT: A 36-year-old man went to the emergency department the day after exposure to a laser pointer. Funduscopy revealed dispersed macular pigment alterations, and optical coherence tomography showed a disruption of the retinal layers. The visual acuity was counting fingers in both eyes. Nine months later, optical coherence tomography angiography showed a rarefaction in the choriocapillaris vascular flow in the area of the laser injuries. DISCUSSION: The retinal layer mainly damaged by the laser injury was the retinal pigment epithelium. However, it is suggested that the laser injury could involve tissues beyond the retinal pigment epithelium, since a rarefaction of the choriocapillaris was found after exposure to the laser

RESUMO

CASO CLÍNICO: Paciente varón de 10 años, de raza negra, con antecedente de traumatismo leve en ojo izquierdo; presenta agudeza visual corregida de 0,2 en escala de Snellen, hifema con altura de 1 mm, presión intraocular (PIO) de 12 mmHg en ese ojo, con incremento de PIO a 20 mmHg a las 72 h. Con el resultado positivo del estudio de drepanocitos, se decide tratamiento médico con oxigenoterapia transcorneal. Se logra el aclaramiento de la cámara anterior, con agudeza visual corregida de 0,8 y la reducción de la PIO a 8 mmHg. DISCUSIÓN: En los pacientes con hifema persitente en el contexto de una drepanocitosis, la oxigenoterapia transcorneal es una buena alternativa terapéutica. Se obtienen resultados satisfactorios inmediatos con la disminución de la PIO y el aclaramiento de la cámara anterior

Achieving immediate and favourable results by lowering the IOP and improving the clearing of the anterior chamber

Clinical case The case concerns a 10-year-old boy of African origin, who suffered a mild ocular trauma to the left eye. Upon examination, the best visual acuity was 0.2 using the Snellen scale, with a 1mm height hyphema, intraocular pressure (IOP) of 12mmHg on left eye, with an increase up to 20mmHg within 72h. RESUMO

Objetivo: Analizar cualitativamente la evolución de las ampollas de filtración mediante tomografía de coherencia óptica de segmento anterior (OCT-SA) en pacientes con implante XEN45. Métodos: Estudio prospectivo de las ampollas de filtración de 30 ojos operados de catarata y glaucoma mediante facoemulsificación e implantación XEN45 (FACO-XEN). Se realizó OCT-SA analizando la morfología y la reflectividad de la ampolla al mes 3, 6 y 12 postoperatorio. La funcionalidad se estudió considerando una presión intraocular (PIO) ≤ 18mm Hg sin medicación hipotensora. Resultados: La PIO permitió clasificar las ampollas en no funcionales: planas (6,67%) y encapsuladas (3,33%) y funcionales (90%), las cuales dividimos según su morfología en quísticas (5/27), difusas (2/27) y laminar o en capas (20/27). Las quísticas presentaron una PIO media de 12,8, 12,6 y 14,0 mm Hg a los 3, 6 y 12 meses respectivamente. En las difusas la PIO media fue 13,0, 11,5 y 13,0 mm Hg a los 3, 6 y 12 meses respectivamente. En las de patrón en capas la PIO media fue 14,45, 14,55 y 14,8 mm Hg a los 3, 6 y 12 meses respectivamente. El porcentaje de ampollas con alta reflectividad fue de 48,15%, 62,96% y 77,78% a los 3, 6 y 12 meses, con una PIO media de 14,23, 14,59 y 15,14mmHg en cada período, respectivamente. Conclusión: El análisis de OCT-SA podría ser un buen predictor de la funcionalidad de la ampollas en la cirugía FACO-XEN. Aquellas quísticas o con menor reflectividad parecen tener mejor éxito postoperatorio. Pese a ello, se requieren estudios a mayor largo plazo (AU)
