RESUMO

The efficacy of the calcium antagonist bepridil given in a dose of 200-400 mg/day was evaluated in 22 patients with Functional Classes II-III stable angina pectoris. In addition to clinical assessments, repeated bicycle ergometric exercise tests were performed in the patients. The agent was found to be highly potent, well tolerated, and to cause no grave adverse effects.

RESUMO

Diagnostic indices based on various sets of signs were developed to define functional classes of angina pectoris. At the same simultaneous application of clinical parameters (age, conditions for onset of anginal episodes and their arrest, the number of weekly anginal episodes, the level of daily exercise, over 1-mm ST segment depression, resting ECG signs of left ventricular hypertrophy, etc.) and bicycle ergometer testing data (cardiac output, peak exercise heart rate, etc.) augmented the accuracy and reliability of identification of a functional class in patients with stable angina. The diagnostic indices are highly reproducible and may be useful in the assessment of the course of the disease and its prognosis in patients with coronary heart disease.

RESUMO

The study was undertaken to define the clinical and instrumental signs that significantly divide coronary heart disease patients with stable angina into functional classes. Diagnostic criteria constructed on the basis of various clinical and instrumental evidence were developed with regard to the results of the examination of 163 patients with stable angina by using the mathematical statistical methods. The authors provide a procedure for determining and assessing the informative value of the diagnostic index in the long-term prospective survey of patients with coronary heart disease. Concurrent application of clinical parameters and bicycle ergometric findings as part of the index enhances the accuracy and validity of definition of a functional class in patients with stable angina.

RESUMO

The results of 5-year pharmacologic treatment of 234 coronary patients with stable angina of varying functional classes are reported. Coronarography detected stenosis of major coronary arteries in all cases; ventriculography demonstrated no disorders of left-ventricular contractility. Clinical improvement and stabilization occurred in 75.2% of patients, deterioration, in 24.8%. Of the latter group, nonfatal myocardial infarction occurred in 13.7%, and signs of heart failure developed in 9.8%. Repeat bicycle ergometry demonstrated increased or unchanged physical stress tolerance in 67.1%. Working capacity was intact after five years in 61.1%. Therefore, long-term medication of coronary patients with stable angina and normal left-ventricular contractility has proved efficient.

RESUMO

The diagnostic test with dipyridamol was carried out in 83 patients with ischaemic heart disease (IHD) and cardialgias of various genesis, with diagnosis verified by coronarography and veloergometry. IHD patients after dipyridamol had angina pectoris attacks with or without ischaemic changes on ECG, which points to latent coronary insufficiency. The appearance of an anginal attack with the depression of the ST segment is typical of IHD with stenosing coronary atherosclerosis, as a rule, with developed collateral vessels. As to sensitivity and specificity in detecting IHD the dipyridamol test is equal to the bicycle ergometry tests. With the results of both tests considered the frequency rate of detection of IHD is increased. The dipyridamol test is recommended for diagnosis of IHD in therapeutic and cardiological hospitals.
