RESUMO

The objective of this study was to develop a standardized protocol for the capturing and analysis of endoscopy digital images for subsequent use in a Computer Aided Diagnosis (CAD) system in gynaecological cancer. Images were captured at optimum illumination and focus at 720x576 pixels using 24 bits color in the following cases: (i) for a variety of testing targets from a color palette with known color distribution, (ii) different viewing angles and distances from calf endometrium, and (iii) images from the human endometrium. Images were then gamma corrected and their classification performance was compared against that of nonqamma corrected images. No significant difference in texture features was found between the close up and panoramic views, and between angles, either before or after gamma correction. There was significant difference in certain texture features between normal and abnormal endometrium, both before and after gamma correction. Our findings suggest that proper color correction can significantly impact CAD system performance, and we recommend its application prior to quantitative texture analysis in gynaecological endoscopy.

RESUMO

The Dispersion of Air Pollution and its Penetration into the Local Environment (DAPPLE) project brings together a multidisciplinary research group that is undertaking field measurements, wind tunnel modelling and computer simulations in order to provide better understanding of the physical processes affecting street and neighbourhood-scale flow of air, traffic and people, and their corresponding interactions with the dispersion of pollutants at street canyon intersections. The street canyon intersection is of interest as it provides the basic case study to demonstrate most of the factors that will apply in a wide range of urban situations. The aims of this paper are to introduce the background of the DAPPLE project, the study design and methodology for data collection, some preliminary results from the first field campaign in central London (28 April-24 May 2003) and the future for this work. The objective of this study is to investigate the usefulness of texture analysis in the endometrium during hysteroscopy in endoscopic imaging of the uterine cavity. Endoscopy images from the endometrium from three subjects, at optimum illumination and focus, were frozen and digitized at 720x576 pixels using 24 bits color. Regions of interest (ROI) of normal (N=61) and abnormal (N=69) regions were manually selected by the physician. ROI images were converted into gray scale and statistical features (SF) and spatial gray level dependence matrix features (SGLDM) were computed. The nonparametric Wilcoxon rank sum test at a=0.05 was carried out for comparing the differences between normal and abnormal tissue. There was significant difference between normal and abnormal endometrium for the SF features variance, energy and entropy and for the SGLDM feature of angular second moment. There was no significant difference for the SF features mean, median, and SGLDM features of contrast, correlation and homogeneity.

RESUMO

The provision of effective emergency telemedicine and home monitoring solutions are the major fields of interest discussed in this study. Ambulances, Rural Health Centers (RHC) or other remote health location such as Ships navigating in wide seas are common examples of possible emergency sites, while critical care telemetry and telemedicine home follow-ups are important issues of telemonitoring. In order to support the above different growing application fields we created a combined real-time and store and forward facility that consists of a base unit and a telemedicine (mobile) unit. This integrated system: can be used when handling emergency cases in ambulances, RHC or ships by using a mobile telemedicine unit at the emergency site and a base unit at the hospital-expert's site, enhances intensive health care provision by giving a mobile base unit to the ICU doctor while the telemedicine unit remains at the ICU patient site and enables home telemonitoring, by installing the telemedicine unit at the patient's home while the base unit remains at the physician's office or hospital. The system allows the transmission of vital biosignals (3-12 lead ECG, SPO2, NIBP, IBP, Temp) and still images of the patient. The transmission is performed through GSM mobile telecommunication network, through satellite links (where GSM is not available) or through Plain Old Telephony Systems (POTS) where available. Using this device a specialist doctor can telematically "move" to the patient's site and instruct unspecialized personnel when handling an emergency or telemonitoring case. Due to the need of storing and archiving of all data interchanged during the telemedicine sessions, we have equipped the consultation site with a multimedia database able to store and manage the data collected by the system. The performance of the system has been technically tested over several telecommunication means; in addition the system has been clinically validated in three different countries using a standardized medical protocol.

Six weeks after acute myocardial infarction, 303 men were randomly divided into exercise and control groups. The exercise group attended the hospital gymnasium twice weekly for a three-month supervised exercise course. Both groups were exercise tested before and after the course and at subsequent follow-up. The exercise group increased their physical fitness greatly compared with the control group. Eight per cent of the exercise group died during the period of follow-up, compared with 14 per cent of the control group; this difference is not significant. There was an apparent improvement in mortality in those with inferior MI who completed the exercise course, which was not seen in those with MI in other sites. For many patients after MI progressive exercise is safe, improves physical fitness and may reduce mortality for those after inferior MI.

During a study investigating physiological and other effects of an exercise programme for coronary patients, a questionnaire was administered. Preliminary analysis had suggested some improvement in the patients' morale, but in view of the possible relevance of a number of psychological variables it was decided to carry out further analysis on the available data. The coefficient of discrimination was computed for 32 patients. For 19 patients correlations were computed between scores on subjective fitness, symptoms, anxiety, interest in sex, if at work, age, weight, and workload achieved at a given heart rate. The questionnaire appeared to have satisfactorily high internal and external validity. Patients with a high 'morale' score tended to achieve a greater increase in workload over the course. Although cause and effect cannot be unequivocally assigned, the association is felt to be important, and research is continuing.

Fifty-nine men with known coronary disease entered an exercise rehabilitation programme. Forty-nine had sustained recent myocardial infarctions, nine had had past myocardial infarcts, and one had had a recent internal mammary implant for severe angina. Each class lasted about 45 minutes and they were held twice weekly; the course for each patient was three months. Sixteen patients dropped out from the course, only three for reasons obviously medical. No deaths or cardiac arrests occurred during the programme. Extrasystoles and angina were detected in a minority of patients and did not constitute reasons for failure to complete the course. The main aim of the study was to show that such a programme is feasible in a district general hospital. Improvement in the morale of patients was impressive and the incidence of return to work high.
