RESUMO

OBJECTIVE: The aim: To determine the dynamics of galectin-3 in the formation of the patient's clinical profile and the impact on structural cardiac and vascular remodeling in patients with myocardial infarction after reperfusion therapy. PATIENTS AND METHODS: Materials and methods: 140 patients with class II A CHF syndrome according to Vasilenko-Strazhesko FC II-III (NYHA), which occurred in patients with MI, and 22 patients with MI without signs of CHF were examined. RESULTS: Results: The average serum galectin-3 level of the studied patients was analyzed. The average concentration of galectin-3 in patients with stenting without signs of CHF was (10.87±1.65) ng/ml comparing to (6.57±0.21) ng/ml in healthy subjects (p<0.05). An increase in the concentration of this indicator was observed in the group of patients with CHF after MI with reperfusion - (35.2±1.76) ng/ml, which is significantly higher in comparison with practically healthy subjects, and in patients without revascularization it was (38.94±1.97) ng/ml (p<0.001). Positive correlations of medium strength were found between left ventricular end-systolic volume (ESV), left ventricular end-diastolic volume (LVEDV) and serum galectin-3 concentration in the examined patients. The correlation coefficient between the abovementioned indicators was 0.60 (p<0.01) and 0.53 (p<0.05), respectively. CONCLUSION: Conclusions: It has been determined that in patients with CHF with postinfarction cardiosclerosis there is a direct correlation of medium strength between the values of indicators of LV myocardial remodeling with plasma galectin-3 concentration, including ESV, EDV, in examined patients. The role of galectin-3 as a marker of postinfarction remodeling in patients with CHF for early diagnosis, prognosis, and risk stratification has been presented.

OBJECTIVE: The aim: To increase the treatment effectiveness of CHF patients after MI with stenting by using magnesium and potassium salts of gluconic acid, eplerenone, and rivaroxaban in complex therapy. PATIENTS AND METHODS: Materials and methods: The research was performed at the premises of Ivano-Frankivsk Regional Clinical Cardiology Centre, Ukraine. 84 patients with CHF after past MI were examined. RESULTS: Results and conclusions: A more pronounced anti-ischemic effect has been linked to the use of combination therapy with rivaroxaban on the background of basic therapy (BT) in patients with CHF after MI, compared with the use of magnesium and potassium salts of gluconic acid or eplerenone. The use of eplerenone in the complex treatment of these patients on the background of BT has been proven to provide a pronounced reverse remodeling of the left myocardium in the postinfarction period.

RESUMO

Introduction: Cardiovascular diseases remain a problem of the modern medical community. Mortality from cardiovascular disease is at the forefront in the structure of the causes of mortality, so active support for the prevention and treatment of this group of diseases is the most effective measure as it will affect the continuation of life expectancy. The aim: To analyze features and patterns of cardiovascular disease in the Carpathian region. Material and methods: The features of the course and treatment of acute coronary syndrome in the period 2014-2018 in the Ivano-Frankivsk region are analyzed. Clinicalanamnestic, geographical, demographic, the effectiveness of patient care, statistical and acute coronary syndrome registry data were studied. Conclusions: Preventive activities, both primary and secondary, with the promotion of knowledge on the preservation and strengthening of health, are a guarantee of positive progress in overcoming cardiovascular diseases. Creation of rehabilitation centers with a multidisciplinary approach, along with medical and physical interventions of psychological support and training, will contribute to the improvement and prevention of complications of cardiovascular disease. The cooperation of cardiological communities between the regions has a social and medical significance, namely the training of specialists, the exchange of experience, scientific projects with the medical community of Opole Voivodship is one of the directions of increasing the efficiency of treatment of cardiological patients.
