RESUMO

Objective This study aimed to identify the main knee complaints and injuries associated with baseball and their prevalence in the state of São Paulo, Brazil. Methods This epidemiological study analyzed data from an online questionnaire sent to baseball athletes from the state of São Paulo, Brazil, from 2019 to 2022. Results Ninety-eight athletes participated in the study. Their average age was 24.3 years, and 85.72% of the subjects were men. The most prevalent ethnicities were yellow (50%) and white (42.86%). Most athletes had incomplete or complete higher education (75.5%). Most (88.77%) have been training for over 1 year, and 40.82% played in more than 1 position. More than half also practiced another sport. Most (66.32%) athletes present knee complaints or symptoms, and 37.75% had suffered a knee injury playing baseball, with several mechanisms (contact with the ground, contact with another player, or no contact). More than half (59.45%) of the athletes required time away from baseball due to complaints, symptoms, or injuries. Conclusion Among the athletes interviewed, 66.32% had a knee complaint, and 37.75% had already had a knee injury, especially meniscal and ligament injuries. The injury rate was highest in the first year of practice.

Objectives The present paper aims to evaluate and compare the histological features of fresh and frozen menisci stored in a tissue bank for 1 month and for 5 years. Methods The meniscal grafts were subjected to a histological study. A total of 10 menisci were evaluated; 2 were frozen for 5 years, 4 were frozen for 1 month, and 4 were fresh, recently harvested specimens. Histological properties were evaluated in sections stained with hematoxylin and eosin and Masson trichrome methods. Results The menisci frozen for 1 month showed partially preserved collagen fiber structure and no significant hydropic tissue degeneration. The menisci frozen for 5 years presented an evident dissociation of collagen fibers and multiple foci of hydropic degeneration. Discussion Degeneration was much more significant in menisci stored for 5 years, indicating that a long freezing period results in substantial progression of tissue deterioration. This may suggest that the 5-year period, considered the maximum time for graft storage before transplant, is too long. Conclusion Grafts stored for 1 month showed a slight degenerative change in collagen fibers, whereas menisci frozen for 5 years presented significant tissue degeneration.

PURPOSE AND HYPOTHESIS: In preoperative sizing for meniscal transplantation, most authors take into consideration the length and width of the original meniscus, but not its height. This study aimed at evaluating (1) whether the meniscal height is associated with the meniscal length and width, (2) whether the heights of the meniscal segments are associated with the individual's anthropometric data, (3) whether the heights of the meniscal segments are associated with each other in the same meniscus, and (4) the degree of symmetry of the meniscal dimensions between the right and left knees. METHODS: In this cross-sectional, observational study, two independent radiologists measured the meniscal length, width and height in knee magnetic resonance imaging scans obtained from 25 patients with patello-femoral pain syndrome. Reproducibility of measurements was calculated with intraclass correlation coefficients. Associations between the anthropometric data and the meniscal measurements, the meniscal length and width versus height, and the heights of the meniscal segments in the same meniscus were examined with Pearson's correlation. RESULTS: Inter-observer reliability was excellent (>0.8) for length and height and good (0.6-0.8) for width measurements. There was also excellent agreement (>0.8) for the length and width of the menisci in the right and left knees. The heights of the horns of the lateral meniscus showed good agreement (0.6-0.8), while the heights of the other meniscal segments had excellent agreement between the sides (>0.8). There were significant associations with generally low (r < 0.5) correlation between the heights of the meniscal segments and the lengths and widths of the menisci, between the meniscal height and anthropometric data, and between the heights of the meniscal segments in the same meniscus. Correlations between anthropometric data and meniscal length and width were generally high (r > 0.7). CONCLUSIONS: There was excellent agreement between the meniscal dimensions of the right and left knees, and a weak association between the meniscal height with the meniscal width and length, between the height of the menisci with anthropometric data and between the heights of the segments in the same meniscus. The height of the meniscal segments may be a new variable in preoperative meniscal measurement.

OBJECTIVES: To ascertain thickness measurements on the anterior cruciate ligament (ACL) in its middle third on magnetic resonance imaging (MRI) scans and to assess whether there is any association between variations in ligament thickness and patients' heights and ages, along with variations in the anatomical measurements on the knee. METHODS: MRI scans on 48 knees were evaluated. The anteroposterior size of the femoral condyles, interepicondylar distance, intercondylar distance and anteroposterior and mediolateral thicknesses of the ACL were measured. It was assessed whether there was any statistical relationship between ACL thickness and the patients' age, height or other measurements evaluated. RESULTS: The mean thickness of the middle third of the ACL was 4.5 mm in the sagittal plane and 4.3 mm in the frontal plane. The anteroposterior thickness of the ACL in its middle third had a positive relationship with the size of the lateral condyle. The mediolateral thickness of the ACL in its middle third had a positive relationship with the size of the lateral condyle and with the intercondylar distance in the axial plane. There was no relationship between the thickness of the ACL and the patients' age or height. CONCLUSION: The thickness of the ACL presented positive associations with the size of the lateral femoral condyle and the intercondylar distance.


OBJETIVO: Obter as medidas da espessura do ligamento cruzado anterior (LCA) em seu terço médio em exames de ressonância magnética e avaliar se existe associação entre a variação da espessura do ligamento com a altura e a idade dos pacientes, bem como com as variações das medidas anatômicas do joelho. MÉTODOS: Foram avaliados os exames de ressonância magnética de 48 joelhos, aferidas as medidas do tamanho anteroposterior dos côndilos femorais, distância interepicondilar, distância intercondilar e as espessuras anteroposterior e mediolateral do LCA e avaliamos se existe relação estatística entre a espessura do LCA e a idade ou a altura dos pacientes e as demais medidas avaliadas. RESULTADOS: A média da espessura no terço médio do LCA foi de 4,5 mm no plano sagital e 4,3 mm no plano frontal. A espessura anteroposterior do LCA no seu terço médio tem relação positiva com o tamanho do côndilo lateral. A espessura mediolateral do LCA no seu terço médio tem relação positiva com o tamanho do côndilo lateral e com a distância intercondilar no plano axial. Não encontramos relação entre a espessura do LCA e a idade ou a altura dos pacientes. CONCLUSÃO: A espessura do LCA apresenta uma associação positiva com o tamanho do côndilo femoral lateral e a distância intercondilar.

OBJECTIVE: This was an epidemiological study on fractures of the proximal third of the femur in elderly patients who were treated at a teaching hospital in the central region of São Paulo. METHODS: The subjects were patients over the age of 60 years who were attended over a 1-year period. A questionnaire seeking basic sociodemographic data and information on comorbidities presented and medications used was drawn up. The circumstances of the fractures and their characteristics, the treatment instituted and the intra-hospital mortality rate were evaluated. RESULTS: The 113 patients included in the study presented a mean age of 79 years. The ratio between the sexes was three women to each man. Only 30.4% of the patients reported having osteoporosis and only 0.9% had had treatment for the disease. Low-energy trauma was the cause of 92.9% of the fractures. Femoral neck fractures accounted for 42.5% of the fractures and trochanteric fractures, 57.5%. Five patients did not undergo operations; 39 underwent joint replacement; and 69 underwent osteosynthesis. The mean length of hospital stay was 13.5 days and the mean length of waiting time until surgery was 7 days. The intra-hospital mortality rate was 7.1%. CONCLUSION: The patients attended at this institution presented an epidemiological profile similar to what is found in the Brazilian literature. Chronic kidney failure is a significant factor with regard to intra-hospital mortality. Preventive measures such as early diagnosis and treatment of osteoporosis and regular physical activity practices were not implemented.


OBJETIVO: Estudo epidemiológico das fraturas do terço proximal do fêmur em pacientes idosos, tratados em hospital-escola na região central de São Paulo. MÉTODOS: Pacientes a partir 60 anos atendidos no período de um ano. Questionário foi elaborado com informações sociodemográficas básicas, comorbidades apresentadas e medicações em uso. Foram avaliadas circunstâncias da fratura e suas características, tratamento instituído e taxa de mortalidade intra-hospitalar. RESULTADOS: Os 113 pacientes incluídos no estudo apresentavam 79 anos em média. A proporção entre os sexos foi de três mulheres para cada homem. Somente 30.4% dos pacientes relataram osteoporose e somente 0.9% tratavam a doença. Trauma de baixa energia foi a causa de 92,9% das fraturas. Fraturas do colo do fêmur representaram 42,5% das fraturas e trocantéricas 57,5%. Cinco pacientes não foram operados, 39 foram submetidos a substituição articular e 69 foram submetidos a osteossíntese. O tempo médio de internação foi de 13,5 dias e de espera até a cirurgia sete dias. A taxa de mortalidade intra-hospitalar foi de 7,1%. CONCLUSÃO: Pacientes atendidos na instituição apresentam perfil epidemiológico semelhante àqueles encontrados em literatura nacional. Insuficiência renal crônica é um fator significativo para mortalidade intra-hospitalar. Medidas preventivas como diagnóstico precoce e tratamento da osteoporose e prática regular de atividades físicas não são adotadas.

OBJECTIVES: To study the measurements and anatomical relationships of the patellofemoral joint using magnetic resonance imaging, and to evaluate the variation in the morphology of the medial patellofemoral ligament (MPFL) according to patients' heights and ages and the variation in measurements on other structures that are known to be involved in predisposition to patellar instability. METHOD: Twenty-three knees (18 patients) underwent magnetic resonance imaging and their interepicondylar distance, patellar height, trochlear depth, ventral trochlear prominence, trochlear groove angle, lateral facet tilt, lateral patellar tilt and size of the lateral and medial facets and their ratio were measured. These measurements were compared with the length and thickness of the MPFL. RESULTS: The average length of the MPFL was 46.4 mm, while the average thicknesses of its patellar insertion, middle third and femoral insertion were, respectively, 1.7 mm, 1.4 mm and 1.2 mm. The thickness of the MPFL correlated positively with the lateral condyle and interepicondylar distance measurements, and negatively with the patients' ages. CONCLUSION: The morphology of the MPFL varies with the interepicondylar distance and the lateral condyle distance, and with patients' ages.
