The elderly are predisposed to adverse drug reactions because of age-related physiologic changes, interaction of one drug with another, and interaction between a drug and a coexisting medical condition. Recognition of such adverse reactions is essential, because many are reversible and many are even preventable. Assessment of clinical disorders in the elderly should include evaluation of drugs as a causal or aggravating factor. Careful choice and dosage of drugs in the elderly will minimize the likelihood of drug-induced illness.

7-Hydroxymethotrexate is a major metabolite of methotrexate in patients undergoing chemotherapy with this drug. It has now been demonstrated that when cultures of CCRF-CEM, a human lymphoblastic cell line, are incubated in the presence of 100 microM methotrexate, significant quantities of 7-hydroxymethotrexate are formed. This finding is relevant to clinical use of the drug since it is now clear that during the treatment of acute lymphocytic leukemia conversion of methotrexate to the relatively inactive 7-hydroxy derivative probably occurs not only in the liver but also in the target cells.

This controlled study, utilizing matched pairs, evaluates the relative cost effectiveness of day hospitalization to inpatient treatment in a group of 18- to 24-year-old black schizophrenic males. Global Assessment Scale scores were measured upon intake, at the time of discharge, and at the three-month follow-up to determine the level of patient functioning for each treatment group. Cost outcome measures were made for each program and a side-by-side cost effective analysis was conducted between the two. Results reveal evidence that day treatment was superior to inpatient care at a lower cost for a longer period of time.

The experience of a solo practitioner suggests that the incidence of multiple myeloma is higher than the widely reported figure of 2.7/100,000. Only by early recognition and treatment will the survival rate be increased. Measurement of the sedimentation rate may be helpful in spotting the disease, for a rapid rate in patients with bone pain and anemia suggests multiple myeloma.
