RESUMO

After several years of waiting due to the "covid quarantine" and subsequent restrictions, we finally met. Epidemiological restrictions prevented the planned interim congress in 2021. The Third Congress on Functional and Stereotactic Neurosurgery was held in Moscow on March 15-17, 2023. Despite the widespread use of the online conference format, the Congress was attended by almost everyone who is in one way or another associated with functional and stereotactic neurosurgery in our country. International participation was also present despite the existing sanctions restrictions.

Morphometric MRI analysis improves neuroimaging of structural changes in epilepsy. OBJECTIVE: To investigate diagnostic potential of MR brain morphometry in neurosurgical epileptology. MATERIAL AND METHODS: An interdisciplinary working group reviewed the studies devoted to MR morphometry in epileptology as a part of state assignment No. 056-00119-22-00. Study subject was trials of MR-morphometry in epilepsy. Searching for literature data was conducted in international and national databases between 2017 and 2022 using certain keywords. Final analysis included 36 publications. RESULTS: Currently, MR brain morphometry allows measurement of cortical volume and thickness, surface area and depth of furrows, as well as analysis of cortical tortuosity and fractal changes. In neurosurgical epileptology, MR-morphometry has the greatest diagnostic value in MR-negative epilepsy. This method simplifies preoperative diagnosis and reduces costs. CONCLUSION: Morphometry in neurosurgical epileptology is an additional method for verifying the epileptogenic zone. Automated programs simplify application of this method.

A comparative analysis of two case histories showing the effects of COVID-19 and immunization with the Sputnik V vaccine on the dynamics of epilepsy is presented. The study was part of the "Epilepsy and COVID-19" research initiative in a cohort of patients with epilepsy divided into two groups: those experiencing COVID-19 and those immunized with vaccine against COVID-19. We present here a comparative analysis of two case histories: recurrence of seizures in COVID-19 in a young (male) patient and good tolerance of vaccine in an older (female) patient with comorbid pathology after surgical treatment of drug-resistant epilepsy. Patient K (age 32 years, male) had generalized genetic (idiopathic) epilepsy with remission of seizures for three years. On the background of mild COVID-19 the patient experienced recurrence of generalized tonic-clonic seizures with deterioration of electroencephalographic indicators. Patient F (age 59 years, female) had focal drug-resistant epilepsy with remission of seizures for three years following surgical treatment, with comorbid endocrine pathology. No side effects were seen on immunization with Sputnik V vaccine and clinical-electroencephalographic remission persisted. The results showed good tolerance of Sputnik V vaccine, with persisting remission of seizures after neurosurgical treatment of drug-resistant epilepsy in the older patient with comorbid pathology and the potential for recurrence of seizures in the young patient on the background of mild COVID-19. These results will help practicing physicians determine management tactics in epilepsy patients. Continuation of this research on the effects of COVID-19 disease and vaccination on the dynamics of epilepsy in a larger cohort is needed.

To compare an impact of coronavirus disease and Sputnik V COVID-19 vaccine on the dynamics of epilepsy. The study is part of the ongoing «Epilepsy and COVID-19¼ independent research which recruited patients with epilepsy into two groups: group 1 - COVID-19 survivors and group 2 - patients vaccinated with a COVID-19 vaccine. The study compares two clinical cases: seizure recurrence with COVID-19 disease in a young patient and good tolerability of vaccination in a female elderly patient following surgical management of pharmacoresistant epilepsy with concomitant pathology. In group 1, a 32-year-old patient with idiopathic generalized epilepsy and 3-year seizure remission had generalized seizure recurrence with electroencephalographic deterioration against the backdrop of mild COVID-19. In group 2, a 59-year-old patient, with focal pharmacoresistant epilepsy, and 3-year seizure remission after surgical management, and comorbid endocrine dysfunction showed no side-effects with Sputnik V COVID-19 vaccination and maintained clinical and electroencephalographic remission. The study revealed that the Sputnik V vaccine was well tolerated, and that seizure remission was maintained after epilepsy surgery in an elderly patient with comorbidity, as well as there was the possibility of seizure recurrence in younger patients with mild COVID-19. The findings will aid practitioners in making decisions on how to manage epilepsy patients. More study into the impact of the disease and COVID-19 vaccination on epilepsy dynamics in a larger sample is required.

BACKGROUND: To study etiopathogenesis is one of the most important tasks of modern neurology. Various types of structural changes occur in drug-resistant epilepsy (DRE); however, they are described as distinct phenomena. OBJECTIVE: To provide a comprehensive characterization of structural changes in the cortex and adjacent white matter in the electrophysiological activity zone (in the epileptic focus) in patients undergoing surgery for DRE. MATERIAL AND METHODS: Biopsy material of fragments of the temporal lobe and hippocampus from 16 patients aged 21 to 54 years (mean age, 25 years) with DRE were intraoperatively obtained at the Prof. A.L. Polenov Russian Research Institute of Neurosurgery. The investigators studied histological sections stained with H&E, toluidine blue according to the Nissl method and the Spielmeyer method, as well as the results of immunohistochemical reactions with glial fibrillary acidic protein (GFAP), vimentin, and neurofilaments (NF) (Dako antibodies, Denmark). RESULTS: Histological examination revealed a set of heterogeneous changes, reflecting the complex pathogenetic interactions that developed during the formation of an epileptic focus. Structural brain damage involved both gray and white matter. Focal cortical dysplasia was diagnosed in 14 (87.5%) cases; white matter neuronal heterotopia in 100%; neuronal reactive and destructive changes in 100%; epileptic leukoencephalopathy (vascular demyelination, microcysts, sclerosis and dystonia, gliosis) in 100%, cortical atrophy in 12.5%, and hippocampal sclerosis in 20% (in 2 out of the 10 examinees). CONCLUSION: The morphopathological heterogeneity in the structure of epileptic foci reflects the complexity of etiopathogenetic interactions, the polymorphism of epileptic manifestations, and the individual nature of formation of the epileptic system, which requires an integral approach to understanding the pathogenesis and morphogenesis of formation of the epileptic system and provides a direction for a personalized approach to epilepsy treatment.
